Özet

Amaç: Bu çalışmada, tiroid cerrahisi sonrasında meydana gelebilen insidental paratiroidektominin, insidansını ve risk faktörlerini belirlemek ve postoperatif hipokalsemi ile ilişkisini ortaya koymak amaçlanmıştır. Giriş: Tiroid cerrahisi, modern gelişmeler sonrasında majör komplikasyon oranı ve mortalite oranı çok düşük bir cerrahi haline gelmiştir. Ancak rekürren laringeal sinir felci ve postoperatif hipokalsemi hala tiroidektomilerden sonra morbiditeye yol açması nedeni ile önemini korumaktadır. Postoperatif hipokalsemi, litaratürde değişen oranlarda bildirilmekte ve %50 ye kadar görülme riski bulunmaktadır. Postoperatif hipokalseminin risk faktörleri arasında insidental paratiroidektomi tartışmalı da olsa yer almaktadır. İP insidansının yapılan çalışmalarda, %2,3 ila %21,6 arasında değiştiği görülmektedir. İP için risk faktörlerini belirlemek ve ortaya çıkabilecek postoperatif hipokalsemi için hangi hastaların daha fazla risk altında olduğunu görmek cerrahlar açısından klinik olarak önemli yer tutmaktadır. Gereç Yöntem: Bu retrospektif kohort çalışması, Ocak 2008 ile Ekim 2020 tarihleri arasında üniversite hastanemiz genel cerrahi bünyesinde ameliyat kodu olarak tiroidektomi kodları girilmiş 3612 hastanın dosyalarının taranmasıyla oluşturulmuştur. 9 hasta, eşlik eden eş zamanlı tersiyer hiperparatiroidi nedeniyle çalışma dışında bırakılmıştır. İP için yapılan analizler 3603 hasta ile yapılmıştır. Bu analizlerde hastalar, İP grubu ve İP olmayan grup olarak ikiye bölünmüş ve bu gruplar hastaların yaşı, cinsiyeti, preoperatif tanıları, gerçekleştirilen cerrahi prosedürler, postoperatif patolojik tanıları, piyes ağırlıkları gibi özellikler açısından karşılaştırılmışlardır. Postoperatif 48 saat içinde düzeltilmiş kalsiyum değeri 8mg/dl'den düşük değeri olan vakalar hipokalsemi olarak değerlendirildi. Hipokalsemi için yapılan analizlerde eşlik eden paratiroid cerrahisi olan 164 hasta çıkarılmış ve hastalar geçici, kalıcı hipokalsemi ve normokalsemi grubu olarak 3 gruba ayrılarak aynı karşılaştırmalar yapılmıştır. Bunun yanı sıra İP ve hipokalsemi, ototransplantasyon ve hipokalsemi ilişkileri de değerlendirilmiştir ve p<0,05 anlamlı olarak kabul edilmiştir. Bulgular: Hastaların 2833'ü (%78,6) kadın, 770'i (%21,4) erkekti. Kadınların yaş ortalaması (ort.) 47.2, erkeklerin yaş ort. 49.3, genel yaş ort. ise 47,6±13 olarak hesaplandı. 3603 vakada 554 (%15,4) tane İP vakası saptandı. Bu vakalarda toplam 605 bez çıkarılmış olup bu bezlerin %24,8'i intratiroidaldi. İP için kadın cinsiyet, malignite, santral ve fonksiyonel boyun diseksiyonu, timektomi, lenf nodu metastaz varlığı ve düşük piyes ağırlığı risk faktörü olarak belirlendi. Eşlik eden paratiroid cerrahileri çıkarıldıktan sonra, 3439 hasta üzerinde geçici hipokalsemi oranı %35,6, kalıcı hipokalsemi oranı %1,8 olarak hesaplandı. Hipokalsemi için ise kadın cinsiyet, nüks MNG, bilateral santral diseksiyon, fonksiyonel boyun diseksiyonu, timektomi, lenfositik tiroidit varlığı, malignite ve kapsül invazyonu risk faktörü olarak belirlendi. İP olan vakalarda ve ototransplantasyon olan vakalarda, geçici ve kalıcı hipokalsemi oranlarının anlamlı derecede yüksek olduğu saptandı (p<0,001). Sadece ototransplantasyon olan vakalar ile sadece İP olan vakalar birbirleri ile karşılaştırıldıklarında ise sadece İP olan vakalardaki hipokalsemi oranının anlamlı derecede yüksek olduğu izlendi(p=0.028). Sonuç: Çalışmamıza göre İP ve ototransplantasyon, postoperatif hipokalsemi gelişimde önemli yer tutmaktadır. Bu nedenle paratiroid fonksiyonlarının düzenli tutulması ve hipokalsemiden kaçınmak için, cerrahi esnasında titiz bir diseksiyon gerekmekte ve cerrahi bitiminde piyesin İP açısından detaylı olarak incelenmesi gerekmektedir. Bu incelemelerde İP saptanır ise; ototransplantasyon, postoperatif hipokalsemi gelişimini önlemek amacıyla akılda bulundurulması gereken bir yöntem olarak karşımıza çıkmaktadır. Ayrıca hipokalsemi riski taşıyan hastaların erken teşhisi, cerrahlar için önem arz etmektedir.

Objective: In this study, it was aimed to determine the incidence and risk factors of incidental parathyroidectomy that may occur after thyroid surgery and to reveal its relationship with postoperative hypocalcemia. Introduction: Thyroid surgery has become a surgery with a very low major complication and mortality rate after recent advancements in surgical techniques. However, recurrent laryngeal nerve palsy and postoperative hypocalcemia still maintain their importance due to the morbidity after thyroidectomies. Postoperative hypocalcemia has been reported at varying rates in the literature and has a risk of up to 50%. Incidental parathyroidectomy is among the risk factors of postoperative hypocalcemia, albeit controversial. Studies show that the incidence of IP ranges from 2.3% to 21.6%. It is clinically essential for surgeons to determine the risk factors for IP and to notice which patients are at higher risk for postoperative hypocalcemia that may occur. Methods: This retrospective cohort study was performed by reviewing the files of 3612 patients, whose thyroidectomy codes were registered as the operation code in the general surgery service of our university hospital, between January 2008 and October 2020. Nine patients were excluded because of concomitant tertiary hyperparathyroidism. Analyzes for IP were conducted with 3603 patients. In these analyzes, patients were divided into two groups as IP and non-IP groups. These groups were compared with their age, gender, preoperative diagnoses, procedures, postoperative pathological diagnoses, and specimen weight. Cases with a corrected calcium value less than 8 mg/dl within 48 hours postoperatively were considered as hypocalcemia. In the analyzes performed for hypocalcemia, 164 patients with concomitant parathyroid surgery were excluded and the same comparisons were made by dividing the patients into 3 groups as transient hypocalcemia, permanent hypocalcemia, and normocalcemia groups. In addition, the relationships between IP-hypocalcemia and autotransplantation-hypocalcemia were evaluated. For analyzes, a p value of <0.05 was considered to indicate statistical significance. Results: 2833 of the patients (78.6%) were female, 770 (21.4%) were male. The mean age of women was 47.2, the mean age of men was 49.3, and the total mean age was 47.6 ± 13. 554 (15.4%) IP cases were detected in 3603 cases. In these cases, a total of 605 glands were removed, and 24.8% of these glands were intrathyroidal. Female gender, malignancy, central and functional neck dissection, thymectomy, presence of lymph node metastasis, and low specimen weight were determined as risk factors for IP. After the concomitant parathyroid surgeries were excluded, the rate of transient hypocalcemia was 35.6% and the rate of permanent hypocalcemia was 1.8% on 3439 patients. For hypocalcemia, female gender, recurrent multinodular goiter, bilateral central dissection, functional neck dissection, thymectomy, presence of lymphocytic thyroiditis, malignancy, and capsule invasion were determined as risk factors. Transient and permanent hypocalcemia rates were detected to be significantly higher in patients with IP and with autotransplantation (p <0.001). When the cases with only autotransplantation and cases with only IP were compared with each other, it was observed that the rate of hypocalcemia was significantly higher in cases with only IP (p = 0.028). Conclusion: According to our study, IP and autotransplantation play an important role in the development of postoperative hypocalcemia. For this reason, meticulous dissection is required during surgery and the specimen should be examined in terms of IP at the end of the surgery to keep parathyroid functions regular and to avoid hypocalcemia. If IP is detected in these examinations; autotransplantation emerges as a method that should be kept in mind in order to prevent the development of postoperative hypocalcemia. In addition, early diagnosis of patients at risk of hypocalcemia is important for surgeons.
