Özet
Amaç: Ateş organizmayı çeşitli etkenlerden korumak için gelişmiş bir savunma mekanizmasıdır. Literatürde ateş oluşum mekanizmalarını aydınlatmak için yapılan deneylerde genellikle gram negatif bakterilerin dış membranının ana komponenti olan LPS verilmektedir. LPS, vücutta TLR-4 tarafından tanınmaktadır. Bu reseptörün aktivasyonu sonucunda çeşitli proinflamatuvar sitokinler salıverilmekte ve immün sistem aktive olmaktadır. Bu aktivasyonun bir sonucu da ateştir. TLR-4 sinyal blokürleri kullanarak LPS' nin vücutta tanınmasını engelleyerek ateş yanıtının inhibe edilebileceği gibi bir hipotezi test etmek istedik. Gereç ve yöntem: Wistar albino, yetişkin sıçanlara LPS enjekte ederek ateş oluşturduk. Vücut sıcaklıklarını periton boşluğuna yerleştirdiğimiz telemetrik implantlarla ölçtük. Plazma IL-6 seviyesini ELISA yöntemiyle değerlendirdik. Bulgular: Ön deneylerde LPS ile oluşturduğumuz ateş yanıtının, antipiretik olduğu bilinen metamizolle inhibe olduğunu göstererek modelimizi valide ettik. Ardından LPS ile oluşturduğumuz ateş yanıtı üzerine TLR-4 sinyal blokürlerinin etkilerini inceledik. LPS-RS ateş yanıtını inhibe etmedi. Bu durumun dozdan, uygulanış yerinden ve uygulanış zamanından bağımsız olduğu görüldü. Dişi sıçanlarda da farklı bir sonuç elde edemedik. IAXO-102 veya naltrekson uygulamasıyla da ateş yanıtı inhibe olmadı. LPS verilmesi sonrası kanda artması beklenen IL-6 seviyesini ölçtük. LPS ile beklenen IL-6 artışı gözlendi; ama LPS-RS bu artışa da etki göstermedi. LPS' nin dozunun azaltılması veya suşunun değiştirilmesi de LPS-RS' nin ateş yanıtını inhibe etmesine neden olmadı. Hava kesesi modeliyle lokal bir alanda inflamasyon oluşturup aynı bölgeye LPS-RS enjekte etmemiz de ateş yanıtını inhibe etmedi. Son olarak endojen bir yolla sistemik inflamasyonu uyararak ateş yanıtı elde etmek için yaptığımız çekal ligasyon ponksiyon modeli ile ateş yanıtı gözleyemedik ve bu modelde TLR-4 sinyal blokürlerinin etkililiklerini deneyemedik. Sonuç: Sıçanlarda sistemik ve lokal LPS enjeksiyonu sonucunda oluşan ateş yanıtı ve ateş yanıtıyla beraber artan IL-6 seviyesi TLR-4 sinyal blokürleriyle inhibe olmamaktadır. TLR-4 in vivo LPS ile oluşan sistemik inflamatuvar yanıtların inhibisyonu bağlamında uygun bir farmakolojik hedef olmayabilir.

Aim: Fever is an advanced defense mechanism to protect the organism from various factors. In experiments to elucidate the mechanisms of fever formation, LPS, which is the main component of the outer membrane of gram-negative bacteria, has usually been administered. LPS is recognized by TLR-4 in the organism. As a result of the activation of this receptor, various proinflammatory cytokines; such as IL-6 are released and the immune system is activated. One consequence of this activation is fever. We aimed to test whether we could inhibit the fever response by preventing the recognition of LPS in the body by using TLR-4 signaling inhibitors. Materials and methods: We induced fever by injecting LPS to Wistar, albino, adult rats. We measured the body temperatures with telemetric implants placed in the abdominal cavities. We measured the plasma IL-6 level by ELISA method. Results: First, we found that the fever response induced by LPS was inhibited by metamizole, which is known to be an antipyretic. Meanwhile, LPS-RS did not inhibit the fever response. It was observed that this situation was independent of dose, administration route and time of administration. LPS-RS did not also abolish the fever in female rats. We subsequently used other TLR-4 signaling inhibitors such as IAXO-102 or naltrexone and the fever response was not inhibited by these agents. We measured the level of IL-6, which is expected to increase after LPS administration. The expected increase in IL-6 was observed with LPS; but LPS-RS did not affect this increase either. Reducing the dose or changing the strain of LPS also did not result any inhibition of fever by LPS-RS. As an alternative model, fever was induced by local injection of LPS into an air pouche. LPS-RS into the same area did not inhibit the fever response. Finally, we intended to produce fever by endogenously released pyrogen substances with the cecal ligation puncture model. However, we observed a very prominent hypothermic response rather than fever and we were not able to evaluate the effect of these agents on this model. Conclusions: The fever response resulting from systemic and local LPS injection and accompanied IL-6 elevation in rats has not been blocked by TLR-4 signaling inhibitors. Thus, TLR-4 may not be a suitable pharmacological target in the context of inhibition of systemic inflammatory responses induced by LPS in vivo.
