Özet

DİZ ÇEVRESİ YUMUŞAK DOKU KALINLIĞININ VE VÜCUT KİTLE İNDEKSİNİN TOTAL DİZ ARTOPLASTİSİ SONRASI FONKSİYONEL SONUÇLARA VE HAYAT KALİTESİNE ETKİSİ Giriş: Yüksek vücut kitle indeksi diz osteoartriti için bir risk faktörüdür. Total diz artroplastisi sonrası da komplikasyonlarla ilişkilendirilmiştir. Vücut kitle indeksi ve total diz artroplastisi fonksiyonel sonuçlar ve komplikasyonlar açısından irdelenmiş fakat yüksek vücut kitle indeksine sahip hastalarda alt ekstremitedeki antropometrik değişiklikler göz ardı edilmiştir. Bu çalışmanın amacı total diz artroplastisi sonrası diz çevresi lokal ölçümlerinin fonksiyonel sonuçlarla ve hayat kalitesi ile ilişkisini prospektif olarak araştırmaktır. Gereç ve Yöntem: Bu prospektif gözlemsel çalışmaya, primer diz osteoartriti nedeniyle total diz artroplastisi uygulanacak hastalar dahil edildi. Hastalar ameliyat öncesi vücut kitle indeksine göre dört gruba ayrıldı (25-29,9, 30-34,9, 35-39,9 ve 40-44,9). Hastaların ameliyat öncesi vücut kitle indeksleri, kiloları, diz ve bel çevreleri ölçüldü. Hastaların ameliyat öncesi Knee Society Score (KSS), Forgotten Joint Score (UES) skorları ve Short Form-36 (SF-36) değerleri kayıt altına alındı. Diz çevresi lokal ölçümleri için ön arka grafilerde diz medial kenarı, diz lateral kenarı, yumuşak doku kalınlıkları ve kemik kalınlıkları ölçüldü. Diz yan grafilerde eklem seviyesi 8 cm üstü yağ/kas oranı, patella önü yumuşak doku kalınlığı ölçüldü. Hastaların vücut yağ oranları, ekstremite yağ oranları ise Tanita BC-418 ile ölçülerek kayıt altına alındı. Ameliyat öncesi ve ameliyat sonrası 6, 12 ve 24. aylarda UES, KSS ve SF-36 değerleri ölçülerek karşılaştırıldı. Ayrıca yüzeyel enfeksiyon, yatış süresi, ameliyat süresi gibi değerler kayıt altına alınarak değerlendirildi. Hastalar ortalama diz çevresi kalınlığına göre tekrar gruplara ayrıldı. Hastaların ameliyat öncesi ve ameliyat sonrası 6, 12 ve 24. aylardaki UES, KSS, SF-36 değerleri karşılaştırıldı. Vücut kitle endeksine göre ayrılan gruplar ile ortalama diz çevresi kalınlığına göre ayrılan gruplar arasındaki değişen hasta sayıları değerlendirildi. Bulgular: Çalışmaya Aralık 2022 - Nisan 2023 arasında ameliyat edilen 121 hasta dahil edildi. Hastaların ortanca yaşı 67 idi (48-84). Hastaların %84,3'ü (102) kadın, %15,7'si (19) erkekti. Hastaların ortanca vücut kitle indeksi 35,1 (25,2-44,5), ortanca kilosu 82,6 (56-124) idi. Hastaların ameliyat öncesi KSS ve KS-F değerleri vücut kitle indeksi arttıkça düşmekteydi. Bütün gruplarda ameliyat sonrası kontrollerde KSS ve KS-F skorlarında ameliyat öncesi döneme göre anlamlı düzelme vardı (p<0,001). Gruplar arasında ameliyat sonrası dönemde 6, 12 ve 24. aylarda KSS skorları arasında anlamlı fark vardı (p<0,001). KS-F skorları 6. aydan itibaren gruplar arasında farklılık göstermedi. UES skorları 6,12 ve 24. aylarda gruplar arasında farklılık gösterdi (p<0,001). UES skorları gruplar içinde kontrollerde anlamlı farklılık göstermedi. SF-36 ölçeğinin bütün alt boyutlarında gruplar içerisinde anlamlı düzelme görüldü. SF-36'nın bütün alt birimlerinde gruplar arasında 6, 12 ve 24. aylarda anlamlı bir farklılık görülmedi. Ortalama diz çevresi kalınlığına göre yapılan yeni gruplandırmada, vücut kitle indeksine göre yapılan gruplandırmaya göre hastaların gruplarının değiştiği görülmüştür. Ortalama diz çevresi kalınlığına göre yapılan gruplandırmada her bir grubun ameliyat öncesi ve sonrası 6, 12 ve 24. aylardaki KS-F, KSS skorları değerlendirildiğinde bütün gruplarda anlamlı skor yükselmesi görülmüştür (p< 0,001). Ortalama diz çevresi kalınlığına göre ayrılan gruplarda, her bir grubun kendi içindeki ameliyat öncesi, ameliyat sonrası 6, 12 ve 24. ay SF-36 bütün alt birimlerinde anlamlı bir yükselme elde edilmiş ve 24. ayın sonunda gruplar arasında SF-36 açısından bir fark saptanmamıştır (p<0,001). 24. ay sonunda ortalama diz çevresi kalınlığına göre ayrılan gruplar ile vücut kitle indeksine göre ayrılan grupların KSS, KS-F skorları birbirine benzer bulunmuştur. Ortalama diz çevresi kalınlığı, vücut kitle indeksi, bel çevresi, kemik/kemik+yumuşak doku oranı, eklem seviyesi 8 cm üstü yağ/kas oranı, vücut yağ/kas oranı, ekstremite yağ oranı KS-F, KSS ve UES ile korelasyon gösterdi. Yüzeyel enfeksiyon açısından VKİ anlamlı değil iken ortalama diz çevresi kalınlığı, kemik/kemik+yumuşak doku oranı ve eklem seviyesi 8 cm üstü yağ/kas oranı anlamlı olarak değerlendirildi (p<0,05). Ameliyat süresi vücut kitle indeksi, ortalama diz çevresi kalınlığı, kemik/kemik+yumuşak doku kalınlığı ve eklem seviyesi 8 cm üstü yağ/kas oranı ile korelasyon gösterdi. Yatış süresi, diz medial ve lateral yumuşak doku kalınlığı, ortalama diz çevresi kalınlığı, patella önü yumuşak doku kalınlığı, ekstremite yağ oranı, vücut yağ oranı ve diz çevresi kemik/kemik+yumuşak doku oranıyla korelasyon gösterdi. Sonuç: Yüksek vücut kitle indeksine sahip hastalar total diz artroplastisi sonrası daha düşük vücut kitle indeksine sahip hastaların ameliyat sonrası elde ettiği fonksiyonel skorlara ulaşamasalar da bu skorlarda anlamlı düzelme gösterirler. Yaşam kalitesi açısından bakıldığında yüksek vücut kitle indeksine sahip hastalarla düşük vücut kitle indeksine sahip hastalar arasında total diz artroplastisi sonrası fark yoktur. Diz çevresi lokal ölçümleri total diz artoplastisi yapılacak hastalarda yüzeyel enfeksiyon, ameliyat süresi ve yatış süresi gibi durumları etkilemektedir. Vücut kitle indeksi total diz artoplastisi sonrası fonksiyonel skorları öngörmede hala önemli bir yer tutmaktadır. Total diz artoplastisi uygulanacak hasta değerlendirilirken vücut kitle indeksinin yanında diz çevresi lokal ölçümleri de değerlendirilmeli, göz ardı edilmemelidir.

THE EFFECT OF PERI-KNEE SOFT TISSUE THICKNESS AND BODY MASS INDEX ON FUNCTIONAL OUTCOMES AND QUALITY OF LIFE AFTER TOTAL KNEE ARTROPLASTY Introduction: The high body mass index is a risk factor for knee osteoarthritis. It has also been associated with complications after total knee arthroplasty. While body mass index and total knee arthroplasty have been examined in terms of functional outcomes and complications, the anthropometric changes in the lower extremities in patients with a high body mass index have been overlooked. The aim of this study is to prospectively investigate the relationship between local measurements of knee circumference after total knee arthroplasty and functional outcomes, as well as quality of life. Materials and Methods: This prospective observational study included patients undergoing total knee arthroplasty due to primary knee osteoarthritis. The patients were divided into four groups according to their preoperative body mass index (25-29.9, 30-34.9, 35-39.9 and 40-44.9). Preoperative body mass index, weight, knee and waist circumferences of the patients were measured. Preoperative Knee Society Score (KSS), Forgotten Joint Score (UES) scores and Short Form-36 (SF-36) values of the patients were recorded. For local measurements of knee circumference, medial knee edge, lateral knee edge, soft tissue thickness and bone thickness were measured on anteroposterior radiographs. On lateral knee radiographs, the fat/muscle ratio above the joint level 8 cm and the soft tissue thickness in front of the patella were measured. The body fat ratios and extremity fat ratios of the patients were measured with Tanita BC-418 and recorded. UES, KSS and SF-36 values were measured and compared before and at 6, 12 and 24 months after surgery. In addition, values such as superficial infection, hospitalization time, and surgery time were recorded and evaluated. The patients were divided into groups again according to the mean knee circumference thickness. The UES, KSS, SF-36 values of the patients before and at 6, 12 and 24 months after surgery were compared. The changing number of patients between the groups separated according to body mass index and the groups separated according to mean knee circumference thickness were evaluated. Results: The study included 121 patients who underwent surgery between December 2022 and April 2023. The median age of the patients was 67 (48-84). 84.3% (102) of the patients were female and 15.7% (19) were male. The median body mass index of the patients was 35.1 (25.2-44.5) and the median weight was 82.6 (56-124). The preoperative KSS and KS-F values of the patients decreased as the body mass index increased. There was a significant improvement in KSS and KS-F scores in the postoperative controls in all groups compared to the preoperative period (p<0.001). There was a significant difference between the KSS scores at 6, 12 and 24 months after surgery between the groups (p<0.001). KS-F scores did not differ between the groups after the 6th month. UES scores differed between the groups at 6, 12 and 24 months (p<0.001). UES scores did not show significant differences in the controls within the groups. Significant improvements were seen within the groups in all sub-dimensions of the SF-36 scale. No significant differences were seen between the groups in all sub-dimensions of the SF-36 at 6, 12 and 24 months. In the new grouping made according to the mean knee circumference thickness, it was seen that the patient groups changed compared to the grouping made according to the body mass index. When the KS-F, KSS scores of each group were evaluated before and at 6, 12 and 24 months after the surgery in the grouping made according to the mean knee circumference thickness, a significant increase in scores was seen in all groups (p<0.001). In the groups separated according to the mean knee circumference thickness, a significant increase was obtained in all SF-36 subunits within each group before surgery, 6, 12 and 24 months after surgery and no difference was found between the groups in terms of SF-36 at the end of the 24th month (p<0.001). At the end of the 24th month, the KSS, KS-F scores of the groups separated according to the mean knee circumference thickness and the groups separated according to body mass index were found to be similar. The mean knee circumference thickness, body mass index, waist circumference, bone/bone+soft tissue ratio, fat/muscle ratio above the joint level 8 cm, body fat/muscle ratio, extremity fat ratio showed correlation with KS-F, KSS and UES. While BMI was not significant in terms of superficial infection, the mean knee circumference thickness, bone/bone+soft tissue ratio and fat/muscle ratio above the joint level 8 cm were evaluated as significant (p<0.05). Duration of surgery was correlated with body mass index, mean knee circumference thickness, bone/ bone + soft tissue thickness, and fat/muscle ratio above the joint level 8 cm. Duration of hospitalization was correlated with knee medial and lateral soft tissue thickness, mean knee circumference thickness, anterior patella soft tissue thickness, extremity fat ratio, body fat ratio, and knee circumference bone/ bone + soft tissue ratio. Conclusion: Patients with high BMI may not achieve the same postoperative functional scores as those with lower BMI after TKA; however, they still show significant improvements. In terms of quality of life, there is no difference between high- and low-BMI patients following TKA. While peri-knee local measurements impact factors such as superficial infection, surgery duration, and hospital stay, they do not establish causality with functional scores. BMI remains a crucial predictor of functional outcomes after TKA. Therefore, peri-knee local measurements should be considered alongside BMI during preoperative evaluation for TKA candidates.
