Özet
Giriş ve Amaç: Kemik ve yumuşak doku tümörleri yüksek mortaliteye ve morbiditeye neden olabilmektedir. Tümör cerrahisinde adjuvan ve neoadjuvan tedavilerin gelişmesi ve yeni endoprotez tasarımları sayesinde ekstremite koruyucu cerrahi yöntemleri ampütasyonların önüne geçmiştir. Bu çalışmadaki amacımız, diz çevresi tümörleri nedeniyle kliniğimizde tümör rezeksiyon protezi ameliyatı yapılan hastaların klinik ve radyolojik sonuçlarını değerlendirmektir. Aynı zamanda rotasyonel menteşeli ve menteşeli protezlerin birbirlerine olan üstünlüklerini değerlendirmektir. Hastalar ve Yöntem: Kliniğimizce diz bölgesi tümörleri nedeniyle 2000-2018 yılları arasında ameliyat edilen hastalar çalışmaya alındı. Tek bir hekim tarafından tümör rezeksiyon proteziyle rekonstrüksiyon cerrahisi uygulanan 31 hasta kontrole çağırıldı. MSTS skoru kullanılarak klinik değerlendirme ve ISOLS skoru kullanılarak radyolojik değerlendirme yapıldı. Bulgular: Protezlerin 5 yıllık sağkalımı %46,2'dir. Ortalama MSTS skoru %84,65±9,94 (%54-97) (orta-mükemmel) ve ortalama ISOLS skoru 86,60±10,47 (%45-100) (kötü-mükemmel) olarak saptanmıştır. Protez tiplerine göre skorlar arasında anlamlı bir fark saptanmazken, her iki protez tipi için düz bacak kaldırma kuvveti arttıkça duygusal kabulleniş artmaktadır. Sonuç: Tümör rezeksiyon protezleri diz çevresi tümörleri için sağkalımı kısaltmadan hastaya fonksiyon sağlayan avantajlı bir tedavi yöntemidir. Hastanın tedavi ihtiyacına göre menteşeli veya rotasyonel menteşeli tümör rezeksiyon protezi tercih edilebilir.

Introduction and Aim: Bone and soft tissue tumors have high mortality and morbidity rates. With new improvements in adjuvant-neoadjuvant therapies and new endoprosthesis designs, limb salvage surgery methods became more popular than amputations. The aim of our study was to evaluate clinical and radiological results of patients who underwent surgery due to tumors around knee with megaprosthesis. Also we compared hinged and rotational hinged megaprosthesis whether they have advantage over each other. Patients and Methods: Thirty one patients, who had been operated between 2000-2018 by a single surgeon with knee megaprosthesis were evaluated using International Limb Salvage Symposium criterias (MSTS and ISOLS scores). Results: Five years survival for the prosthesis were 46,2%. Overall MSTS score was 84,65%±9,94 (54%-97) (fair-excellent) and ISOLS score was 86,60%±10,47 (45%-100) (poor-excellent). There were no significant differences between the scores according to prosthesis types. As the strenght of straight leg lift increases so was the emotional acceptance for both types of prosthesis. Conclusion: For tumors around the knee megaprosthesis' are good choice of treatment without decreasing life expectancy. Both hinged and rotational hinged prosthesis can be used according to clinical needs.
