Özet

Amaç: HIV enfeksiyonu olanlarda son dönemde antiretroviral tedavi (ART) kullanımı ile malignite insidansının arttığı görülmüştür. Çalışmamızda HIV ve lenfoma tanılı hastaların tedavileri ve toksisitelerini, sağkalımlarını HIV negatif lenfoma hastaları ile karşılaştırılarak geriye dönük olarak incelenmesi amaçlanmıştır. Gereç ve Yöntem: Çalışmamızda 6 farklı merkezde 2010 – 2024 yılları arası tanı almış tüm HIV dosyaları geriye dönük taranmıştır. HIV (+) ve lenfoması olan hastaların demografik verileri, serum belirteçleri, tedavi yanıtları, toksisitesi, genel sağkalım (OS) ve ve progresyonsuz sağkalım (PFS) analizi yapılarak aynı yaş grubu ve tarihlerde tanı alan kontrol grupları ile karşılaştırılmıştır. Bulgular: Çalışmaya dahil edilen 61 HIV (+) hastanın 20'si (%33) Burkitt lenfoma (BL), 16'sı (%26) diffüz büyük B hücreli lenfoma (DBBHL), 15'i (%25) Hodgkin lenfoma (HL), 7'si (%11) plazmoblastik lenfoma (PBL) ve 3'ü (%5) primer merkezi sinir sistemi lenfıması (PMSSL) idi. 290 HIV (-) hastanın ise 19'u (%6,5) BL, 165'i (%56,9) DBBHL, 106'sı (%36,6) HL idi. Hastaların yaşlarının ortancaları arasında fark saptanmadı (p=0,100). Kadın sıklığı PMSSL'lerde fazlaydı (p=0,034). Sağkalım açısından; PBL'lerin OS ve PFS'si daha kısaydı (sırasıyla, p=0,004, p=0,001). HIV (+) BL, DBBHL ve HL'ler HIV (-)'lerle karşılaştırıldığında HIV (+)'lerde erkek oranı anlamlı olarak daha fazlaydı (BL'ler %100 vs. %42,1, DBBHL'ler %93,8 vs. %56,4, HL'ler %100 vs. %52,8; sırasıyla p<0,001, p=0,004, p<0,001). Tedavi yanıtında BL ve HL'lerde HIV gruplarına göre fark yoktu (sırasıyla, p=0,683, p=0,239); DBBHL'lerde tam yanıt oranı HIV (-)'lerde (%73,2) HIV (+)'lere (%37,5) göre anlamlı olarak daha yüksekti (p=0,001). Herhangi bir toksisite varlığı açısından ise BL ve DBBHL'lerde HIV (+) ve HIV (-)'ler arasında fark yokken, HL'lerde HIV (-)'lerde anlamlı olarak daha fazlaydı (%30,2 vs. %14,3; p=0,012). Tek değişkenli analizde OS'yi etkileyen faktörler; BL'lerde evre, ekstranodal tutulum, ECOG, LDH; DBBHL'lerde HIV (+)'liği idi. PFS'yi etkileyen faktörler; BL'lerde evre, ekstranodal tutulum; DBBHL'lerde HIV (+)'liği, patolojide EBV DNA (+)'liği, ekstranodal tutulum ve ECOG; HL'lerde ECOG olduğu belirlendi. Çok değişkenli analizlerde anlamlı bulgu saptanmadı. Sonuç: Çalışmamızda HIV (+)'liğinin DBBHL grubunda OS'yi etkilediği gösterilmiştir. Hastaların tedavi yanıtları ve toksisitelerinin HIV (+)'liği ile ilişkisi lenfoma gruplarına göre farklı olup, bu verilerin HIV ile ilişkisinin gösterilmesi açısından daha fazla hasta verisine ve çalışmaya ihtiyaç vardır.

Introduction: In recent years, an increase in the incidence of malignancies has been observed with the use of antiretroviral therapy (ART) in those with HIV infection. Our study aims to retrospectively analyze the treatments, toxicities, and survival outcomes of patients diagnosed with HIV and lymphoma, compared to HIV-negative lymphoma patients. Materials and Methods: In our study, all HIV patients diagnosed between 2010 and 2024 in six different centers were retrospectively reviewed. The demographic data, serum markers, treatment responses, toxicity, overall survival (OS), and progression-free survival (PFS) of HIV-positive patients with lymphoma were analyzed and compared with control groups of the same age group and diagnosed within the same time period. Results: Among the 61 patients with HIV positivity, 20 (33%) had Burkitt lymphoma (BL), 16 (26%) had diffuse large B-cell lymphoma (DLBCL), 15 (25%) had Hodgkin lymphoma (HL), 7 (11%) had plasmablastic lymphoma (PBL) and 3 (5%) had primary central nervous system lymphoma (PCNSL). Among the 290 HIV-negative patients, 19 (6.5%) had BL, 165 (56.9%) had DLBCL and 106 (36.6%) had HL. No significant difference was found in the median ages of the patients (p=0.100). The frequency of females was higher in PCNSL cases (p=0.034). In terms of survival outcomes, PBL cases had shorter OS and PFS (p=0.004 and p=0.001, respectively). When HIV-positive BL, DLBCL and HL cases were compared with HIV-negative ones, the ratio of males was significantly higher in the HIV-positive group (BL: 100% vs. 42.1%, DLBCL: 93.8% vs. 56.4%, HL: 100% vs. 52.8%; p<0.001, p=0.004 and p<0.001, respectively). For treatment response, no difference was observed between HIV groups in BL and HL cases (p=0.683 and p=0.239, respectively); however, the complete response rate in DLBCL was significantly higher in HIV-negative patients (73.2%) compared to HIV-positive patients (37.5%) (p=0.001). In terms of the presence of any toxicity, no significant difference was found between HIV-positive and HIV-negative patients in BL and DLBCL cases, whereas in HL, any toxicity was significantly more frequent in HIV-negative patients (30.2% vs. 14.3%; p=0.012). In univariate analysis, factors affecting OS included stage, extranodal involvement, ECOG and LDH levels in BL cases; HIV-positivity in DLBCL cases. Factors affecting PFS were stage and extranodal involvement in BL; HIV-positivity, EBV DNA positivity in pathology, extranodal involvement and ECOG in DLBCL cases with ECOG was the determinant in HL cases. No significant findings were detected in multivariate analyses. Conclusion: In our study, HIV positivity was shown to affect OS in the DLBCL group. The relationship between treatment responses and toxicities with HIV positivity varied across lymphoma subgroups. Further patient data and studies are needed to better elucidate the association of these findings with HIV.
