Özet

Giriş: Gelişimsel kalça displazisi hafif displaziden, proksimal femoral ve asetabular deformitelerin eşlik ettiği kalça ekleminin tamamen çıkık olduğu morfolojik bozuklulara kadar seyreder. Erken yaşta hastalarda ağrı, fonksiyonel kısıtlılık ve bacak uzunluk farkında şikayetleri ile başvururlar. Crowe tip 3-4 gelişimsel kalça displazisi asetabular kemik defektleri, proksimal ve diafizer femurda ki deformiteler, pelvik sagittal eğimde ki bozulmalar ve yumuşak doku kontraktürleri nedeniyle total kalça artroplastisi teknik olarak zordur. Amaç: Crowe tip 3-4 gelişimsel kalça displazisi nedeniyle total kalça artroplastisi uygulanan hastaların operasyon sonrası klinik ve radyolojik olarak sonuçlarının değerlendirilmesi, fonksiyonel sorunlarını azaltmak ve yaşam kalitelerini arttırmak. Yöntem: Ekim 2011- Mart 2024 tarihleri arasında Manisa Celal Bayar Üniversitesi Tıp Fakültesi Ortopedi ve Travmatoloji Kliniğinde Crowe 3-4 Gelişimsel Kalça Displazisi nedeniyle total kalça protezi uygulanan 47 hastanın dahil edilmesi planlandı. Hastaların 8'una ulaşılamadı.39 hastanın (9 bilateral) 48 kalçasına ait bulgular retrospektif olarak değerlendirildi. Verilerin analizi IBM SPSS Statistics 27 paket programında yapıldı. Nominal değişkenler independent samples t test, paired sample t test nonnominal değişkenler Mann whitney u testi ile yapıldı ayrıca korelasyon analizi ve frekans analizi kullanıldı, p<0,001 istatiksel olarak anlamlı kabul edildi. Bulgular: Ortalama takip süresi 6,5 yıl idi. Son değerlendirmede HHS ortalama 84,57 ameliyat öncesi değerlere göre istatistiksel olarak anlamlı derecede artmıştır (p<0,001). Merle D Aubigne kalça skoru ortalama 15,76 ameliyat öncesi değerlere göre istatistiksel olarak anlamlı derece artmıştır (p<0,001). Modifiye JOA skoru ortalama 16,46 ameliyat öncesi değerlere göre istatistiksel olarak anlamlı derece artmıştır (p<0,001), Modifiye JOA iyileşme yüzdesi ortalama %55,76. VAS ağrı skalası %38,5 i ağrı şikayetinin olmadığını (0), %20 si hafif ağrı (1-3), %4 ü orta şiddette ağrı şikayetinin (4-6) olduğunu bildirdi. Kalça merkezinin ortalama medializasyonu 0,60 mm literatürde belirtilen ideal rotasyon merkezi (COR'un) sınırları içerisindeydi, yukarı çıkma 10,83 mm idi. Asetabular komponent ortalama inklinasyonu 45,5 dereceydi, literatürde genel savunulan görüşe göre 45 derece ve üzeri polietilen aşınmasının arttırdığıydı . Femoral ofseti ortalama 41 mm idi, bu literatürde röntgen (40 ± 0,5) ve tomografi (42,2) ile ölçülen ideal femoral ofset mesafesine çok yakındı. Pelvik sagittal eğimin bilateral Crowe tip 4 hastalarda postoperatif değiştiğini gösteren ölçümlerden H/W oranı (sağ) ortalaması postoperatif son kontrolünde 0,77 ameliyat öncesi değerlere göre istatistiksel olarak anlamlı derece artmıştır ( p değeri 0,004), H/W oranı (sol) ortalaması postoperatif son kontrolünde 0,76 çıktı ameliyat öncesi değerlere göre istatistiksel olarak anlamlı derece artmıştır ( p değeri 0,012), SSc mesafesi ortalaması postoperatif son kontrolünde 37,44 ölçülerek ameliyat öncesi değerlere göre istatistiksel olarak anlamlı derece azalmıştır ( p değeri 0,008), SC mesafesi ortalaması postoperatif son kontrolünde 16,22 ölçülerek ameliyat öncesi değerlere göre istatistiksel olarak anlamlı derece azalmıştır ( p değeri <,001), Ssi mesafesi ortalaması postoperatif son kontrolünde 80,33 ölçülerek ameliyat öncesi değerlere göre istatistiksel olarak anlamlı derece azalmıştır. ( p değeri ,010). Sonuç: Crowe tip 3-4 asetabular, femoral, pelvik ve yumuşak doku deformiteleri nedeniyle total kalça artroplastisi cerrahi teknik olarak zor bir prosedürdür ancak sonuçları bakımından yüz güldürücüdür. Cerrahi teknik ve kullanılan implant bakımından literatürde farklı görüşler mevcuttur.

Introduction: Developmental dysplasia of the hip (DDH) ranges from mild dysplasia to severe morphological abnormalities, including complete dislocation of the hip joint accompanied by proximal femoral and acetabular deformities. Patients typically present at an early age with complaints of pain, functional limitation, and leg length discrepancy. In Crowe type 3-4 DDH, total hip arthroplasty is technically challenging due to acetabular bone defects, proximal and diaphyseal femoral deformities, alterations in pelvic sagittal alignment, and soft tissue contractures. Aim: To evaluate the postoperative clinical and radiological outcomes of patients who underwent total hip arthroplasty due to Crowe type 3–4 developmental dysplasia of the hip, in order to reduce functional problems and improve their quality of life. Methods: Between October 2011 and March 2024, a total of 47 patients who underwent total hip arthroplasty due to Crowe type III–IV developmental dysplasia of the hip at the Department of Orthopaedics and Traumatology, Manisa Celal Bayar University Faculty of Medicine, were initially considered for inclusion in the study. Eight patients could not be reached, and data from 39 patients (9 bilateral cases), involving 48 hips in total, were retrospectively evaluated. Data analysis was performed using IBM SPSS Statistics version 27. Nominal variables were analyzed using independent samples t-test and paired samples t-test, while non-nominal variables were assessed using the Mann–Whitney U test. Additionally, correlation and frequency analyses were conducted. A p-value of <0.001 was considered statistically significant. Results: The mean follow-up period was 6.5 years. At the final evaluation, the mean Harris Hip Score (HHS) significantly increased to 84.57 compared to preoperative values (p < 0.001). The mean Merle d'Aubigné hip score also showed a statistically significant improvement, reaching 15.76 postoperatively (p < 0.001). Similarly, the mean modified Japanese Orthopaedic Association (JOA) score significantly improved to 16.46 compared to preoperative values (p < 0.001), with a mean modified JOA recovery rate of 55.76%. According to the Visual Analog Scale (VAS), 38.5% of patients reported no pain (score of 0), 20% reported mild pain (score 1–3), and 4% reported moderate pain (score 4–6). The mean medialization of the hip center was 0.60 mm, which remained within the ideal limits for the center of rotation (COR) as reported in the literature. Vertical displacement of the hip center averaged 10.83 mm. The mean inclination angle of the acetabular component was 45.5°, which slightly exceeds the commonly referenced threshold of 45° in the literature associated with increased polyethylene wear. The mean femoral offset was 41 mm, which closely approximates the ideal offset values reported in the literature based on radiographic (40 ± 0.5 mm) and CT (42.2 mm) measurements. In patients with bilateral Crowe type IV hips, pelvic sagittal alignment showed significant postoperative changes. The mean H/W ratio (right) increased significantly to 0.77 at final follow-up compared to preoperative values (p = 0.004). The mean H/W ratio (left) also increased significantly to 0.76 (p = 0.012). The mean SSc distance decreased significantly to 37.44 mm postoperatively (p = 0.008). The mean SC distance significantly decreased to 16.22 mm (p < 0.001), and the mean SSi distance decreased significantly to 80.33 mm (p = 0.010). Conclusion: Due to the presence of acetabular, femoral, pelvic, and soft tissue deformities in Crowe type III–IV cases, total hip arthroplasty is a technically demanding surgical procedure. However, the outcomes are generally satisfactory and encouraging. There are varying opinions in the literature regarding the optimal surgical techniques and implant choices for these complex cases.
