Özet
ÖZET Amaç: Preterm doğum yenidoğan morbidite ve mortalitesinin başlıca nedenidir. Preterm doğumların büyük bir kısmı ise geç preterm bebeklerdir. Son yıllardaki gelişmeler nedeniyle geç preterm doğumlarda artış görülmektedir. Bu grupta meydana gelen artış kısa, orta ve uzun dönemde sağlık hizmet vericileri üzerinde ekonomik ve zaman bakımından ciddi bir yük artışına sebep olmuştur. Çalışmamızda yenidoğan yoğun bakım ünitesine yatırılarak takibi gereken geç preterm bebeklerin sosyal ve demografik özellikleri ile yatış nedenleri, yatış süresi ve mortalite arasındaki ilişkilerin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Çalışmamızda Konya Şehir Sağlık Uygulama ve Araştırma Merkezi Konya Şehir Hastanesi Çocuk Sağlığı ve Hastalıkları Yenidoğan Yoğun Bakım Ünitesi'ne (YDYBÜ) 01.09.2023-01.09.2024 tarihleri arasında yatan geç preterm bebekler prospektif olarak takip edilmiştir. Bilgiler yatış anında anneden ve takibinde günlük vizitlerden ve hemşire gözleminden kaydedilmiştir. Çalışmaya dahil edilen, aile onamı alınan ve prospektif olarak takip edilen 385 geç preterm hastanın anne yaşı, doğum haftası, gestasyon haftası, YDYBÜ'ne yatış saati, yatış nedeni, cinsiyet, doğum şekli, gebelik haftasına göre doğum ağırlığı, yardımcı üreme teknikleri varlığı, çoğul gebelik, antenatal steroid, anne morbiditeleri (gestasyonel diyabet, preeklampsi/eklampsi, erken membran rüptürü), konjenital anomali varlığı ve tanısı, resüsitasyon varlığı, respiratuar hastalık, mekanik ventilasyon ihtiyacı, serbest akım veya hood ile verilen oksijen gün süresi, apne, hipokalsemi, hipoglisemi, beslenme bozukluğu, indirekt hiperbilirubinemi (İHB), erken neonatal sepsis veya geç sepsis, kan kültüründe üreme, nekrotizan enterekolit evre 2 ve üzeri, bronkopulmoner displazi, mortalite, yatış süresi ve yeniden yatış bilgileri takip süresi boyunca veri toplama formuna kaydedildi Bulgular: Çalışmada yer alan 385 hastanın ortanca gebelik haftası 35 (34-36) hafta, ortalama doğum kiloları ise 2570,6±537,4 (1360-4280) gramdı. Doğum ağırlığı gebelik yaşına uygun olan (AGA) hasta sayısı 307'idi (%79,7). Yatış zamanı ilk 24 saat olan hasta sayısı 288'dir (%74,8). Yatış nedeni solunum sıkıntısı olan 205 (%53,2), İHB olan 79 (%20,5), konjenital anomali olan 19 (%4,9), beslenme sorunu olan 19 (%4,9) hasta saptandı. Hastaların 222'sinde (%57,7) respiratuar hastalık bulundu. Yatış süresi 1-171 gün aralığındaydı ve medyan yatış süresi 6 gün olarak saptandı. Yeniden yatış 18 (%4,7) hastada gerçekleşti. Yeniden yatış nedeni İHB olan hasta sayısı 8 (%2,1), sepsis olan hasta sayısı 2 (%0,5), idrar yolu enfeksiyonu olan hasta sayısı 1'idi (%0,3). Yatış nedenine göre mortalite dağılımları istatistiksel olarak farklılık göstermekteydi. Genetik nedenler ile yatan çocukların %44,4'ü, konjenital kalp hastalığı nedeni ile yatan çocukların %28,6'sı, diğer nedenler ile yatan çocukların %10'u ve konjenital anomali nedeni ile yatan çocukların %5,3'ü exitus olmuştu. Yatış süresi üzerinde nihai olarak konjenital anomali, beslenme bozukluğu, geç sepsis ve mekanik ventilatör ihtiyacı etkili bulundu (p<0.05). Sonuç: Geç preterm bebekler genellikle term bebekler gibi ele alınmakta ve aynı şekilde yönetilmektedir. Ancak, yapılan çalışmalar geç preterm bebeklerin solunum sıkıntısı, hipotermi, hipoglisemi ve hiperbilirubinemi gibi kısa vadeli olumsuz sonuçlar açısından daha yüksek risk taşıdığını ve gelişimsel gecikme, okul performansı ve sosyal güvenlik yardımları gibi uzun vadeli sonuçlar açısından da dikkatli bir şekilde değerlendirilmesi gerektiğini göstermektedir. Geç preterm bebekler, solunum sıkıntısı, hipoglisemi, sepsis ve mortalite gibi birçok kısa vadeli tıbbi komplikasyona yatkınlık göstermektedir. Bu nedenle, obstetrisyenlerin doğum zamanlamasını değerlendirirken dikkatli olmaları ve 34 haftadan sonra maternal veya fetal komplikasyonlar geliştiğinde gebeliği sonlandırma konusunda daha titiz olmaları önemlidir. Neonatologlar ise bu bebekleri düşük riskli servislerde veya anne yanında izlemeye eğilim göstermemeli ve erken taburculuktan kaçınmalıdır.

ABSTRACT CLINICAL AND DEMOGRAPHIC CHARACTERISTICS OF LATE PRETERM INFANTS ADMITTED TO THE NEONATAL INTENSIVE CARE UNIT Objective: Preterm birth is the leading cause of neonatal morbidity and mortality. The majority of preterm births are late preterm babies. Due to recent developments, an increase in late preterm births has been observed. The increase in this group has caused a severe economic and time burden on healthcare providers in the short, medium, and long term. Our study aimed to evaluate the relationships between the social and demographic characteristics of late preterm babies who need to be hospitalized and followed up in the neonatal intensive care unit, the reasons for hospitalization, the duration of hospitalization, and mortality. Materials and Methods: In our study, late preterm babies who were hospitalized in the Konya Health Application and Research Center Konya City Hospital Child Health and Diseases Neonatal Intensive Care Unit (NICU) between September 2023 and 2024 were followed up prospectively. Information was recorded from the mother during hospitalization, daily visits, and nurse observations during follow-up. The maternal age, week of birth, week of gestation, time of admission to the NICU, the reason for admission, gender, mode of delivery, birth weight according to gestational week, presence of assisted reproductive techniques, multiple pregnancies, antenatal steroids, maternal morbidities (gestational diabetes, preeclampsia/eclampsia, premature membrane rupture), presence and diagnosis of congenital anomalies, presence of resuscitation, respiratory disease, need for mechanical ventilation, duration of oxygen given by free flow or hood, apnea, hypocalcemia, hypoglycemia, malnutrition, indirect hyperbilirubinemia (IHB), early neonatal sepsis or late sepsis, growth in blood culture, necrotizing enterocolitis stage 2 and above, bronchopulmonary dysplasia, mortality, length of stay and readmission information of 385 late preterm patients included in the study were recorded in the data collection form throughout the follow-up period. Results: Of the 385 patients included in the study, the median gestational week was 35 (34-36) weeks, and the mean birth weight was 2570.6±537.4 (1360-4280) grams. The number of patients with appropriate birth weight for gestational age (AGA) was 307 (79.7%). The number of patients whose hospitalization time was day 0 was 288 (74.8%). The reason for hospitalization was 205 (53.2%) patients with respiratory distress, 79 (20.5%) with IHB, 19 (4.9%) with congenital anomalies, and 19 (4.9%) with feeding problems. Respiratory disease was found in 222 (57.7%) of the patients. The duration of hospitalization ranged from 1 to 171 days, and the median duration was 6 days. Readmission occurred in 18 (4.7%) patients. The number of patients with IHB as the reason for readmission is 8 (2.1%), the number of patients with sepsis is 2 (0.5%), and the number of patients with urinary tract infections is 1 (0.3%). Mortality distributions differ statistically according to the reason for admission. 44.4% of children hospitalized for genetic reasons, 28.6% of children hospitalized for congenital heart disease, 10% of children hospitalized for other reasons, and 5.3% of children hospitalized for congenital anomalies exited. Congenital anomaly, malnutrition, late sepsis, and the need for mechanical ventilation were found to be the ultimate factors affecting the length of admission (p<0.05). Conclusion: Late preterm infants are generally treated and managed in the same way as term infants. However, studies have shown that late preterm infants are at higher risk for short-term adverse outcomes such as respiratory distress, hypothermia, hypoglycemia, and hyperbilirubinemia and should be carefully evaluated for long-term outcomes such as developmental delay, school performance, and social security benefits. Late preterm infants are prone to many short-term medical complications, including respiratory distress, hypoglycemia, sepsis, and mortality. Therefore, it is important for obstetricians to be careful when assessing the timing of delivery and to be more meticulous about terminating pregnancy after 34 weeks when there are maternal or fetal complications. Neonatologists should not tend to monitor these infants in low-risk wards or with the mother and should avoid early discharge.
