Özet

Giriş: Gestasyonel geçici tirotoksikoz(GGT), genellikle hiperemezis gravidarumlu kadınlarda görülmekte olup, dolaşımdaki yüksek insan koryonik gonadotropin (hCG) düzeyi ile ilişkilidir. Gebeliğin ilerleyen dönemlerinde hCG seviyelerinin düşmesiyle birlikte tirotoksikoz tablosu genellikle kendiliğinden düzelmektedir. Bizim bu çalışmada amacımız; gebelik süresince gestasyonel geçici tirotoksikozun klinik ve biyokimyasal seyrini analiz etmek, anti-tiroid tedavinin etkisini belirlemektir. Bulgular: Çalışmaya Ocak 2016 ve Nisan 2022 tarihleri arasında Karadeniz Teknik Üniversitesi Tıp Fakültesi Farabi Hastanesi Endokrinoloji ve Metabolizma Hastalıkları Polikliniği'ne başvuran ve gestasyonel tirotoksikoz tanısı alan 98 hasta dahil edildi. Hastaların yaş ortalaması 32,1±5,5 yıldı. Hastaların ortalama gebelik sayısı 2,6±1,5 idi ve %22,5'u (n=22) nullipar, %5,1'i (n=5) multipl gebelikti. TSH düzeyi; birinci trimesterda 0,2±0,2 mIU/L, ikinci trimesterda 0,3±0,4 mIU/L, üçüncü trimesterda 0,5±0,5 mIU/L idi. sT3 değerleri birinci trimesterda 4,1±1,0 mIU/L, ikinci trimesterda 3,6±1,8 mIU/L ve üçüncü trimesterda 3,2±0,5 mIU/L idi. sT4 değerleri birinci trimesterda 1,2±0,6 mIU/L, ikinci trimesterda 0,8±0,6 mIU/L ve üçüncü trimesterda 0,7±0,2 mIU/L idi. Hastaların %9,9'unda (n=8) anti-TPO, %8,8'inde (n=7) anti-TG pozitifliği vardı. Hastaların %33,7'sinin (n=33) USG verisine ulaşılamadı, %37,8'inin (n=37) USG'de tiroid nodülü vardı.Birinci trimesterda aşikar tirotoksikozu olan hastaların tirotoksikoz süresi subklinik tirotoksikozu olanlara göre anlamlı şekilde uzundu (p:0,025). Tiroid nodülü olan hastaların ikinci trimesterdaki TSH değeri (0,25±0,35) tiroid nodülü olmayanlara göre anlamlı şekilde düşüktü (p=0,049). Tiroid nodülü olan hastaların birinci trimesterdaki sT3 değeri (4,39±0,93) tiroid nodülü olmayanlara göre anlamlı şekilde yüksekti (p=0,009). Hastalardan ultrasonografide tiroiditi olanların birinci trimesterdaki sT3 değeri (4,63±0,8) tiroiditi olmayanların sT3 değerinden (4,01±1) anlamlı şekilde yüksekti (p=0,046). Hastalardan tiroiditi olanların üçüncü trimesterdaki sT4 değeri (0,63±0,11) tiroiditi olmayanların sT4 değerinden (0,74±0,18) anlamlı şekilde düşüktü (p=0,025). Multipar gebelerin birinci ve üçüncü trimesterlardaki TSH değeri nullipar gebelere göre anlamlı şekilde düşüktü (p=0,006 ve p=0,008). ATİ kullanan hastaların hipertiroidi süresi ortalama 21,8±11,5 hafta iken kullanmayan hastaların 16,0±9,5 haftaydı ve ATİ kullanan hastaların hipertiroidi süresi kullanmayanlara göre anlamlı şekilde uzundu (p=0,035). Sonuç: GTT tanılı hastalarda tiroid nodülü varlığı serbest T3 düzeyi yüksekliği ile ilişkili bulundu. Özellikle, serbest T3 düzeyi belirgin yüksek olan gebelerde tiroid nodülü varlığı açısından ultrasonografik değerlendirme yapılmalıdır. Multiparite varlığı olan hastalarda gebelik süresince TSH düzeyi nullipar gebelere göre daha düşük bulunmuştur. Buna göre multipar gebelerin ötroidizme ulaşma süresi daha uzun olup tirotoksikozun olası riskleri açısından takibi gereklidir. Gestasyonel diyabet, ATİ tedavisi almayan hastalarda ATİ alan hastalara göre daha sık olduğu görülmüştür. Çalışmamızın sonuçlarının prospektif çok merkezli çalışmalar ile incelenmesi GTT olan hastalara ek katkı sağlayacağı düşüncesindeyiz.

Introduction: Gestational transient thyrotoxicosis is commonly observed in pregnant women with hyperemesis gravidarum and is associated with elevated circulating levels of human chorionic gonadotropin (hCG). As pregnancy progresses, the decline in hCG levels typically leads to a spontaneous resolution of the thyrotoxicosis.The aim of this study was to retrospectively evaluate the etiological and clinical characteristics of patients diagnosed with hyperthyroidism during pregnancy, to analyze the clinical and biochemical course of gestational transient thyrotoxicosis throughout pregnancy, and to determine the impact of anti-thyroid treatment on the course of the disease. Findings: A total of 98 patients who were diagnosed with gestational thyrotoxicosis and admitted to the Endocrinology and Metabolism Outpatient Clinic of Karadeniz Technical University Farabi Hospital between January 2016 and April 2022 were included in the study. The mean age of the patients was 32.1±5.5 years, and the average number of pregnancies was 2.6±1.5. Of the participants, 22.5% (n=22) were nulliparous, and 5.1% (n=5) had multiple pregnancies. Hormonal evaluations during pregnancy revealed the following:TSH levels: 0.2±0.2 mIU/L in the first trimester, 0.3±0.4 mIU/L in the second trimester, and 0.5±0.5 mIU/L in the third trimester.sT3 levels: 4.1±1.0 pg/mL in the first trimester, 3.6±1.8 pg/mL in the second trimester, and 3.2±0.5 pg/mL in the third trimester.sT4 levels: 1.2±0.6 ng/dL in the first trimester, 0.8±0.6 ng/dL in the second trimester, and 0.7±0.2 ng/dL in the third trimester.Autoimmune markers were evaluated, and 9.9% (n=8) were positive for anti-TPO, while 8.8% (n=7) were positive for anti-TG. Among patients with available ultrasound data, 37.8% had nodules, 7.1% had both nodules and thyroiditis, 4.1% had thyroiditis alone, and 3.1% had both nodules and goiter. Ultrasonography (USG) data were unavailable for 33.7% (n=33) of the patients, while 37.8% (n=37) had thyroid nodules detected by USG. The duration of thyrotoxicosis was significantly longer in patients with overt thyrotoxicosis during the first trimester compared to those with subclinical thyrotoxicosis (p=0.025). The second trimester TSH level of patients with thyroid nodules (0.25±0.35) was significantly lower than that of patients without thyroid nodules (p=0.049). The first trimester sT3 level of patients with thyroid nodules (4.39±0.93) was significantly higher than in those without thyroid nodules (p=0.009). Patients with thyroiditis had significantly higher first trimester sT3 levels (4.63±0.8) than those without thyroiditis (4.01±1) (p=0.046).Third trimester sT4 levels were significantly lower in patients with thyroiditis (0.63±0.11) compared to those without thyroiditis (0.74±0.18) (p=0.025).Nulliparous women had significantly higher TSH levels in both the first and third trimesters compared to multiparous women (p=0.006 and p=0.008, respectively).The average duration of hyperthyroidism was significantly longer in patients receiving anti-thyroid drugs (21.8±11.5 weeks) than in those who did not receive treatment (16.0±9.5 weeks) (p=0.035). Conclusion: The presence of thyroid nodules in patients diagnosed with gestational glucose intolerance (GTT) was found to be associated with elevated free T3 levels. In particular, ultrasonographic evaluation should be performed to assess the presence of thyroid nodules in pregnant women with markedly elevated free T3 levels. In multiparous patients, TSH levels during pregnancy were found to be lower compared to those in nulliparous pregnant women. Accordingly, the time to achieve euthyroidism is longer in multiparous women, and follow-up is necessary due to the potential risks of thyrotoxicosis. Gestational diabetes was observed more frequently in patients who did not receive antithyroid immunotherapy (ATI) compared to those who did. We believe that our study results could contribute further to the management of patients with GTT when investigated in prospective multicenter studies.
