Özet
AMAÇ: Meme kanseri kadınlar arasında görülme sıklığına göre dünyada ilk sırada yer almaktadır. Fiziksel ve mental açıdan hastaların huzuruna etki eden bir hastalık olması sebebiyle tedavi yöntemlerinin hem sağkalımı artırmasına hem de hastaların iyi oluş düzeylerine de etki etmesi beklenmektedir. Hastaların kozmetik beklentilerinin artmasıyla birlikte hem hastaların memnuniyet düzeylerini yükseltecek hem de uygun onkolojik prensipleri sağlayacak cerrahi teknikler geliştirilmiştir. Meme cerrahisi sonrası hasta memnuniyeti, cerrahinin şekli, rekonstrüksiyon yöntemi ve demografik özelliklere bağlı olarak değişiklik göstermektedir. Cerrahi tedavi şekli olarak ana hatlarıyla iki türlü yaklaşım mevcuttur. Birincisi memenin kendi dokusunun korunması ve tümörün etraf sağlam dokudan da alınarak çıkarılmasıyla karakterize meme koruyucu cerrahi, diğeri ise meme dokusunun çıkarıldığı mastektomidir. Mastektomi kararında hastaların kozmetik beklentileri daha da belirginleşmektedir. Bu nedenle geliştirilen cerrahi tekniklerden biri de mastektomi ve implant ile rekonstrüksiyon yöntemidir. Endikasyon olduğu sürece meme koruyucu cerrahi yapılması gerekliyken, çeşitli nedenlerle implant ile rekonstrüksiyon da yapılabilmektedir. Cerrahi yöntemler arasında hasta memnuniyet düzeylerini karşılaştıran kanıta dayalı verilerin bulunması, hem ameliyat öncesi hasta beklentilerini yönetmede, hem de ameliyat şeklini belirlemede katkı sağlamaktadır. Memnuniyet düzeyini değerlendirmede etkili araçlardan biri de Breast Q ölçeğidir. Çalışmamızda subkutan mastektomi ve implant ile rekonstrüksiyon yapılan hastalarla meme koruyucu cerrahi yapılanların memnuniyet düzeylerini Breast Q ölçeği ile değerlendirerek karşılaştırmak amaçlanmıştır. GEREÇ ve YÖNTEM: Meme kanseri nedeniyle meme koruyucu cerrahi ya da subkutan mastektomi ve implant ile rekonstrüksiyon yapılan, eş zamanlı onkoplastik prosedür uygulanmayan, 18 yaşından büyük 200 kadın hasta çalışmaya dahil edildi. Herhangi nedenle implant kaybı görülen hastalar çalışma dışında bırakıldı. Meme kanseri nedeniyle ameliyat olmuş hastalar çalışma hakkında detaylı bilgilendirilip aydınlatılmış onamı imzaladıktan sonra Breast Q ölçeğini doldurdular. Mevcut ölçek kullanılarak hastaların ameliyat sonrası memnuniyet düzeyleri değerlendirildi. Breast Q alt boyut memnuniyet değerlendirilmesi dışında hastaların klinik verileri, patoloji sonuçları ve demografik özellikleri incelendi. Her alt boyut memnuniyet düzey sonucu, 0-100 arasında puanlamaya tabi tutuldu. Çalışmada meme koruyucu cerrahi grubundakı hastalar ile subkutan mastektomi ve implant ile rekonstrüksiyon grubundakı hastaların memnuniyet oranları karşılaştırıldı. 1 BULGULAR: Çalışmaya toplam 200 hasta dahil edildi. Bunlardan 115'i meme koruyucu cerrahi, 85'i ise subktan mastektomi ve implant ile rekonstrüksiyon yapılan hastalardı. Subkutan mastektomi vakalarının 67'sine meme başı koruyucu mastektomi, 18'ne ise cilt koruyucu mastektomi yapılmıştır. Hastaların yaş ortalaması 50.3(27-74) idi. Meme koruyucu cerrahi grubunda 27'si(%23,5) duktal karsinoma in situ ve evre I, 88'i(%76,5) ise evre II-III olarak saptanmıştır. Subkutan mastektomi ve implant ile rekonstrüksiyon grubunda 27'si(%31,8) duktal karsinoma in situ ve evre I iken, 58(%68,2) hasta ise evre II – III idi. Meme koruyucu cerrahi grubuna kıyasla mastektomi grubunda 50 yaş altı (%44, p=0,001) ve ideal vücut kitle indeksi(%20, p=0,024) düzeyinde olan hasta oranı istatistiksel olarak anlamlı derecede daha fazla görülmüştür. Meme koruyucu cerrahi grubunda hastaların tamamına yakınına (%98) radyoterapi uygulanırken, mastektomi grubunda hastaların yaklaşık yarısına (%54, p<0,001) radyoterapi uygulanmıştır. Mastektomi ve implant ile rekonstrüksiyon grubuna göre meme koruyucu cerrahi yapılan hastaların Breast-Q ölçeği psikososyal iyi oluş (p<0,001), cinsel iyilik hali (p=0,002) ve meme görünümünden memnuniyet (p<0,001) alt boyut memnuniyet düzeyleri istatistiksel olarak anlamlı derecede daha yüksek olduğu saptanmıştır. Meme koruyucu cerrahi yapılan hastalarda herhangi demografik ya da klinik özelliklerine göre meme memnuniyet düzeylerinde fark saptanmaz iken, subkutan mastektomi ve implant ile rekonstrüksiyon grubunda sigara kullanımı, radyoterapi etkisi ve kronik hastalığın meme memnuniyetini olumsuz etkilediği görülmüştür. Yaş, vücut kitle indeksi ve kronik hastalık etkilerine göre hasta memnuniyet puanları düzeltildiğinde yine memnuniyeti artıran tedavi yönteminin meme koruyucu cerrahi olduğu sonucuna varılmıştır. SONUÇ: Meme koruyucu cerrahi yapılan hastalarda Breast Q ölçeği alt boyut memnuniyet düzeyleri mastektomi grubuna göre anlamlı olarak daha yüksek saptanmıştır. Meme koruyucu cerrahi doğal meme dokusu nedeniyle kozmetik memnuniyet sağlayarak tüm parametrelerde üstün düzeyde memnuniyet oranları göstermiştir. Subkutan mastektomi ve implant ile rekonstrüksiyon grubunda radyoterapi, sigara ve kronik hastalık öyküsünün meme görünümünden memnuniyet oranlarını düşürdüğü saptanırken, meme koruycu cerrahi yapılan hastaların memnuniyet oranlarının ise herhangi klinik veya demografik özellikden etkilenmediği saptanmıştır.

AIM: Breast cancer ranks first among women worldwide in terms of prevalence. As it is a disease that affects both the physical and mental well-being of patients, treatment methods are expected to enhance both survival rates and the overall well-being of patients. With increasing cosmetic expectations among patients, surgical techniques have been developed to improve patient satisfaction while adhering to appropriate oncological principles. Patient satisfaction following breast surgery varies depending on the type of surgery, reconstruction method, and demographic characteristics. Broadly speaking, there are two main surgical approaches. The first is breast-conserving surgery, characterized by preserving the native breast tissue while excising the tumor along with a margin of healthy tissue. The second is mastectomy, in which the entire breast tissue is removed. In decisions regarding mastectomy, patients' cosmetic expectations become more pronounced. One of the surgical techniques developed for this purpose is subcutaneous mastectomy with implant-based reconstruction. While breast-conserving surgery should be performed whenever indicated, implant-based reconstruction may also be performed for various reasons. Having evidence-based data comparing patient satisfaction levels among different surgical methods contributes to managing preoperative patient expectations and determining the appropriate surgical approach. One of the effective tools for evaluating satisfaction levels is the Breast-Q scale. Our study aims to compare the satisfaction levels of patients who underwent subcutaneous mastectomy with implant-based reconstruction and those who underwent breast-conserving surgery using the Breast-Q scale. MATERIALS AND METHODS: The study included 200 female patients over the age of 18 who underwent either breast-conserving surgery or subcutaneous mastectomy with implant-based reconstruction due to breast cancer, without simultaneous oncoplastic procedures. Patients who experienced implant loss for any reason were excluded from the study. Patients who had undergone surgery for breast cancer were thoroughly informed about the study, signed an informed consent form, and then completed the Breast-Q questionnaire. Using this tool, postoperative satisfaction levels of the patients were assessed. In addition to evaluating satisfaction through the subscales of the Breast-Q questionnaire, clinical data, pathology results, and demographic characteristics of the patients were examined. 3 Each sub-dimension satisfaction level result was scored between 0-100. The study compared the satisfaction levels of patients in the breast-conserving surgery group with those in the subcutaneous mastectomy and implant-based reconstruction group. RESULTS: A total of 200 patients were included in the study, of whom 115 underwent breast-conserving surgery and 85 underwent subcutaneous mastectomy with implant-based reconstruction. Among the subcutaneous mastectomy cases, 67 underwent nipple-sparing mastectomy, and 18 underwent skin-sparing mastectomy. The mean age of the patients was 50.3 years (range: 27–74). In the breast-conserving surgery group, 27 patients (23.5%) were diagnosed with ductal carcinoma in situ or stage I breast cancer, while 88 patients (76.5%) were diagnosed with stage II–III breast cancer. In the subcutaneous mastectomy and implant based reconstruction group, 27 patients (31.8%) had ductal carcinoma in situ or stage I disease, whereas 58 patients (68.2%) were diagnosed with stage II–III breast cancer. Compared to the breast-conserving surgery group, the mastectomy group had a significantly higher proportion of patients under 50 years of age (44%, p=0.001) and those with an ideal body mass index (20%, p=0.024). While nearly all patients (98%) in the breast-conserving surgery group received radiotherapy, approximately half of the mastectomy group did (54%, p<0.001). It was found that patients who underwent breast-conserving surgery had statistically significantly higher satisfaction levels in the Breast-Q scale subdimensions of psychosocial well-being (p<0.001), sexual well-being (p=0.002), and satisfaction with breast appearance (p<0.001) compared to those who underwent mastectomy and implant-based reconstruction. No significant differences in breast satisfaction levels were observed among the breast conserving surgery group based on any demographic or clinical characteristics. However, in the subcutaneous mastectomy and implant-based reconstruction group, smoking, radiotherapy effects, and chronic diseases were found to negatively impact breast satisfaction. After adjusting satisfaction scores for factors such as age, body mass index and the chronic diseases, breast-conserving surgery was concluded to be the treatment method that most effectively enhanced patient satisfaction. CONCLUSION: In patients who underwent breast-conserving surgery, satisfaction levels in the subdimensions of the Breast-Q scale were found to be significantly higher compared to the mastectomy group. Breast-conserving surgery provided superior satisfaction rates across all parameters by ensuring cosmetic satisfaction due to the preservation of natural breast tissue. In the group that underwent subcutaneous mastectomy and implant-based reconstruction, it was found that radiotherapy, smoking, and a history of chronic disease reduced satisfaction with breast appearance. However, the satisfaction rates of patients who underwent breast-conserving surgery were not affected by any clinical or demographic characteristics.
