Özet

Amaç: Bu çalışmanın amacı, eklemeli ve eksiltmeli sistemlerle üretilen overlay restorasyonlarda oluşabilecek mikrosızıntının in-vitro olarak incelenmesidir. Üç farklı üretim tekniği (eksiltmeli ve 2 çeşit eklemeli sistem) ile elde edilen ve iki farklı rezin siman ile simante edilen overleylerde oluşabilecek mikrosızıntı değerleri arasında istatistiksel olarak anlamlı bir fark olmadığına dair sıfır hipotezi kurulmuştur. Materyal ve Metod: İn-vitro olarak yürütülen bu çalışmada 66 adet molar diş kullanıldı ve dişlerde çürük, çatlak veya restorasyon olmamasına dikkat edildi. Sınırları mezial ve distalde mine-sement birleşiminin 1 mm üzerinde olan bir overlay preparasyonu yapıldı. Dişler 2 farklı eklemeli (DLP ve SLA) ve 1 eksiltmeli yöntem ile üretilen overlay restorasyonların yerleştirileceği rastgele 3 gruba ayrıldı. Her grup ise kendi içinde iki farklı rezin siman (G-Cem One ve Panavia Universal) ile simante edilecek şekilde 2 alt gruba ayrıldı (n=11). Restorasyonların üretimi ve simantasyonundan sonra dişler termal siklusa tabi tutuldu (5000 siklus). Hazırlanan örnekler ışık geçirmeyen ortamda %0,5'lik bazik fuksin solüsyonu içerisinde 24 saat bekletildi. Daha sonra dişler Micracut 202 hassas kesim cihazı ile su soğutması altında meziodistal yönde 2 eşit parçaya ayrıldı. Kesitler Zeiss Axiocam 503 Color Stereomikroskop altında incelenerek dişler ve restorasyonlar arasındaki boya sızıntısı skorlandı. Boyanın en derin penetrasyonuna sahip olan bölüm, stereomikroskop altında 6 puanlık (0-5) bir ölçeğe göre incelenerek belirlendi. Bulgular: Sonuçlar yorumlanırken anlamlılık düzeyi p<0,05 olarak belirlenmiş olup; p<0,05 için istatistiksel olarak anlamlı kabul edilmiştir. Çalışmamızda, eklemeli ve eksiltmeli üretim yöntemlerinin kendi arasında, materyallere göre ve simanlara göre mezial basamaktaki, distal basamaktaki ve ortalama mikrosızıntı miktarları karşılaştırılmıştır. Çalışmamızda, mezial basamakta, distal basamakta ve bunların ortalaması alındığında; Cerasmart bloktan CAD/CAM frezeleme yöntemiyle üretilip G-Cem One rezin siman ile simante edilen grup en düşük mikrosızıntı değerlerini göstermiştir. En yüksek mikrosızıntı değerlerini gösteren grup ise Crowntec reçineden eklemeli DLP yöntem ile Asiga Max UV cihazında üretilip Panavia SA rezin siman ile simante edilen gruptur (p<0,05). Sonuç: Çalışmamızın sonuçlarına göre sıfır hipotezi reddedilmiştir. Çalışmamızda, Cerasmart 270 bloktan CAD/CAM frezeleme yöntemiyle üretilip G-Cem One rezin siman ile simante edilen grup en düşük mikrosızıntı değerlerini göstermiştir. En yüksek mikrosızıntı değerlerini gösteren grup ise Crowntec reçineden eklemeli DLP yöntem ile Asiga Max UV cihazında üretilip Panavia SA rezin siman ile simante edilen gruptur (p<0,05). Kullandığımız iki adeziv rezin simandan en fazla mikrosızıntı gösteren Panavia SA Universal adeziv rezin simandır. Hiçbir üretim yöntemi ve materyal diş-restorasyon arayüzünde gözlenen mikrosızıntının tamamen elimine edilmesini sağlayamamıştır.

Objective: The aim of this study is to investigate in vitro the microleakage that may occur in overlay restorations produced with additive and subtractive systems. The null hypothesis was established that there is no statistically significant difference between the microleakage values that may occur in overlays obtained with three different production techniques (subtractive and 2 types of additive systems) and cemented with two different resin cements. Materials and Methods: In this in-vitro study, 66 molars were used and the teeth were free of caries, cracks or restorations. An overlay preparation was made with borders 1 mm above the enamel-cementum junction mesially and distally. The teeth were randomly divided into 3 groups in which 2 different additive (DLP and SLA) and 1 subtractive overlay restorations were placed. Each group was divided into 2 subgroups to be cemented with two different resin cements (G-Cem One and Panavia Universal) (n=11). After fabrication and cementation of the restorations, the teeth were subjected to thermal cycling (5000 cycles). The prepared specimens were kept in 0.5% basic fuchsin solution for 24 hours in a light-proof environment. The teeth were then cut into 2 equal sections in the mesiodistal direction under water cooling with a Micracut 202 precision cutting device. The sections were examined under Zeiss Axiocam 503 Color Stereomicroscope and the dye leakage between the teeth and restorations was scored. The section with the deepest penetration of the dye was identified by examining it under the stereomicroscope according to a 6-point (0-5) scale. Results: Analysis results are presented as frequency and percentage. While interpreting the results, the significance level was determined as p<0.05; p<0.05 was considered statistically significant. In our study, the amounts of microleakage in the mesial step, distal step and average were compared between additive and subtractive production methods, according to materials and cements. In our study, the group produced from Cerasmart block by CAD/CAM milling method and cemented with G-Cem One resin cement showed the lowest microleakage values in the mesial step, distal step and their average. The group with the highest microleakage values was the group produced from Crowntec resin with the additive DLP method in Asiga Max UV device and cemented with Panavia SA resin cement (p<0.05). There was no significant difference between the group cemented with G-cem one resin cement, produced with the subtractive method from Cerasmart block and the group produced with the additive SLA method from Permanent Crown resin in Formlabs Form 3B device when the values at the mesial and distal were averaged (p>0.05). Conclusion: According to the results of our study, the null hypothesis was rejected. In our study, the group produced from Cerasmart 270 block by CAD/CAM milling method and cemented with G-Cem One resin cement showed the lowest microleakage values. The group with the highest microleakage values was the group produced from Crowntec resin with the additive DLP method in Asiga Max UV device and cemented with Panavia SA resin cement (p<0.05). Of the two adhesive resin cements we used, Panavia SA Universal adhesive resin cement showed the highest microleakage. No production method and material could completely eliminate the microleakage observed at the tooth-restoration interface.
