Özet
Şizofreni, dünyanın her yerine yayılan, tüm sosyal sınıfları, ırksal ve etnik grupları etkileyen ağır bir hastalıkdır. Şizofreni, kişinin nasıl hissettiğine ve düşündüğüne etki eden, pozitif semptomları, negatif belirtileri ve bilişsel semptomları karakterize eden zihinsel hastalıktır. Çalışmada,“Şizofreni hastalarında CYP3A4 *1B, *2 ve *22 polimorfizmlerinin incelenmesi”incelenmiştir. Araştırmanın amacı, bu polimorfzimlerin şizofreni ile ilişkisini belirlemektir. Sitokrom P450 3A4 (CYP3A4), insanlarda ilaçları metabolize eden en önemli enzimdir. CYP3A4, ilaç metabolizmasında merkezi bir rol oynar ve aynı zamanda, en bol bulunan CYP enzimidir. Şu anda kullanılan ilaçların %45-60'ının metabolize edilmesinde rol oynar. CYP3A gen kümesinin insan lokusu, 7q21-q22 kromozomunda yer alır. CYP3A4, ağırlıklı olarak bağırsakta ve karaciğerde bulunur. CYP3A4 geninde 30'dan fazla SNP (Tek nükleotid polimorfizmi) vardır. İstanbul NP Beyin hastanesinin Psikiyatri polikliniğine başvuran ve şizofreni tanısı konulan, 11-59 yaşları arasında olan toplam 50 hasta çalışmaya dahil edilmiştir. Çalışmada, önce DNA (deoksiribonükleik asit) izolasyonu ardından, Thermo Fisher Quanti stution 5 Real-Time PCR (polimeraz zincir reaksiyonu) kullanılarak Şizofreni tanısı konulmuş toplam 50 hastanın CYP3A4 *1B, *2 ve *22 polimorfizmlerinin genotipleme analizleri yapılmıştır. Çalışmamızda Hastaların büyük bir kısmında (n=36) *1/*1 genotipi ve EM (Normal Metabolizma) fenotipinin olduğunu belirlenmiştir. Toplam 50 hastanın yalnızca birinde (n=1) *1B/*22 genotipi ve EM/IM fenotipi, birinde ise (n=1) *1B/*1B genotipi ve IM (Orta Dereceli Metabolizma) olduğu bulunmuştur. Bireylerin bir kısmının (n=9) *1/*22 genotipi olduğu ve fenotipin ise IM/EM olduğu gözlemlenmiştir. Hastaların yalnızca üçünde CYP3A4 genotipinin (n=3) *1/*1B ve uygun olarak fenotipinin EM (Normal Metabolizma) olduğu bulunmuştur. Sonuç olarak, CYP3A4 genotip ve fenotiplerin tedavi stratejisi dikkate alınmalıdır. Daha çok, *1/*22 (IM/EM) ve *1B/*22 (EM/IM) olan hastalarda doz ayarlamaları ve ilaç seçimi kişiye özel hale getirilmelidir. ii

Schizophrenia is a severe illness that spreads all over the world and affects all social classes, racial and ethnic groups. Schizophrenia is a mental illness characterized by positive symptoms, negative symptoms, and cognitive symptoms that affect how a person feels and thinks. In the study, the examination of CYP3A4 *1B, *2 and *22 polymorphisms in schizophrenia patients were examined. The aim of the research is to determine the relationship of these polymorphisms with schizophrenia. Cytochrome P450 3A4 (CYP3A4) is the most important enzyme that metabolizes drugs in humans. CYP3A4 plays a central role in drug metabolism and is also the most abundant CYP enzyme. It plays a role in the metabolism of 45-60% of currently used drugs. The human locus of the CYP3A gene cluster is located on chromosome 7q21-q22. CYP3A4 is found predominantly in the intestine and liver. There are more than 30 SNPs (Single nucleotide polymorphisms) in the CYP3A4 gene. A total of 50 patients, aged between 11 and 59, who applied to the Psychiatry outpatient clinic of Istanbul NP Brain Hospital and were diagnosed with schizophrenia, were included in the study. In the study, first DNA (deoxyribonucleic acid) isolation was followed by genotyping analyzes of CYP3A4 *1B, *2 and *22 polymorphisms of a total of 50 patients diagnosed with Schizophrenia using Thermo Fisher Quanti study 5 Real-Time PCR (polymerase chain reaction). In our study, it was determined that the majority of patients (n = 36) had *1/*1 genotype and EM (Normal Metabolism) phenotype. Out of a total of 50 patients, only one (n=1) was found to have *1B/*22 genotype and EM/IM phenotype, and one (n=1) was found to have *1B/*1B genotype and IM (Intermediate Metabolism). It was observed that some of the individuals (n=9) had *1/*22 genotype and the phenotype was IM/EM. Only three of the patients were found to have CYP3A4 genotype (n=3) *1/*1B and correspondingly EM (Normal Metabolism) phenotype. Consequently, treatment strategy of CYP3A4 genotypes and phenotypes should be taken into account. Rather, dose adjustments and drug selection should be personalized in patients with *1/*22 (IM/EM) and *1B/*22 (EM/IM). iv
