Özet
Amaç:Primer diz artroplastisi sonrası patellofemoral sorunlar memnuniyetsizlik sebeplerinin başında gelmektedir.Patellar komponent uygulaması veya patella koruyucu teknikler (osteofit temizliği, denervasyon, lateral gevşetme) sıklıkla patellofemoralsorunlardan kaçınmak için kullanılsa da tüm vakalarda tam memnuniyet sağlamamaktadır. Parsiyel lateral fasetektomi lateral kompartman basıncını azaltarak maltraking ve diz önü ağrısından korunmada etkili yöntemler arasında tarif edilmiştir. Amacımız kendi kliniğimizde total diz artroplastisi uygulamaları esnasında osteofit temizliğineek olarak lateral patellar fasetektomi yapılanlar ile diğer patella koruyucu cerrahi teknikleri (osteofit temizliği± denervasyon) uygulanan hastaların klinik, fonksiyonel sonuçlarını karşılaştırmaktır. Gereçve Yöntemler: Çalışmaya kliniğimizde 2018-2023 yılları arasında primer diz artroplastisi uygulananve en az 12 aylık takibi olan hastalar dahil edildi. Hastalar osteofit temizliği+ lateral patellarfasetektomi uygulananlar ve osteofit temizliği ± denervasyon yapılanlar olarakdeğerlendirildi. Herhangi bir sebeple revizyon cerrahisi, patellar komponent uygulananlar, protezcerrahisi öncesi patellar dizilim ve deformite düzelme cerrahisi uygulanan hastalar çalışmaya dahiledilmedi. İki grup arasındaki klinik sonuçlar Knee Society Score (Postoperatif 2011), Oxford Knee Score ve Kujalapatellofemoral skor kriterlerine göre değerlendirildi. Hastaların preop ve postop ayakta basarak diz anterior-posterior, lateral ve Merchant grafileri incelenip, patellar dizilimi, patellar lateral tilt ve lateral patellar yer değiştirme ölçülerek değerlendirildi. Bulgular:Çalışma toplamda 203 hasta ile yapıldı. Bu hastaların 99'u (%48.8) lateral patellar fasetektomi uygulanan, 104'ü (%51.2)lateral patellar fasetektomi uygulanmayan gruptaydı. Çalışma toplam 175 (%86.2) kadın, 28 (%13.8) erkek hastadan oluşuyordu. Lateral patellar fasetektomi olan grupta patellardeplasman, lateral patellar deplasman, congruence vepatellar tilt açısılateral patellar fasetektomi olmayan gruptan anlamlı (p<0.05) olarak daha düşüktü. Lateral patellar fasetektomi olan hastalarınfonksiyonel KSS, Oxford Knee Score, Kujala patellofemoraldeğerilateral patellar fasetektomi olmayan gruptan anlamlı (p<0.05) olarak daha yüksekti. Sonuç: Lateral patellar fasetektominin; patellofemoral uyumu hem radyolojik hem klinik olarak artırdığı ve fonksiyonel KSS, Oxford ve Kujala klinik skorlarına anlamlı pozitif katkı sağladığı tespit edildi.

Aim:Patellofemoral problems are the leading causes of dissatisfaction after primary knee arthroplasty. Although patellar component application or patella-preserving techniques (osteophyte removal, denervation, lateral release) are frequently used to avoid patellofemoral problems, they do not provide complete satisfaction in all cases. Partial lateral facetectomy has been described as an effective method for reducing lateral compartment pressure and preventing maltracking and anterior knee pain. Our aim is to compare the clinical and functional results of patients who underwent lateral patellar facetectomy in addition to osteophyte removal during total knee arthroplasty applications in our clinic with those who underwent other patella-preserving surgical techniques (osteophyte removal± denervation). Material and Methods:The study included patients who underwent primary knee arthroplasty in our clinic between 2018 and 2023 and had at least 12 months of follow-up. Patients were evaluated as those who underwent osteophyte removal + lateral patellar facetectomy and those who underwent osteophyte removal ± denervation. Patients who underwent revision surgery for any reason, patellar component application, patellar alignment and deformity correction surgery before prosthesis surgery were excluded from the study. Clinical results between the two groups were evaluated according to Knee Society Score (Postoperative 2011), Oxford Knee Score and Kujala patellofemoral score criteria. Preoperative and postoperative standing knee anterior-posterior, lateral and Merchant radiographs of the patients were examined, and patellar alignment, patellar lateral tilt and lateral patellar displacement were measured. Results: The study was conducted with a total of 203 patients. Of these patients, 99 (48.8%) were in the group that underwent lateral patellar facetectomy and 104 (51.2%) were in the group that did not undergo lateral patellar facetectomy. A total of 175 (86.2%) were female and 28 (13.8%) were male. In the group with lateral patellar facetectomy, patellar displacement, lateral patellar displacement, congruence and patellar tilt angle were significantly (p<0.05) lower than in the group without lateral patellar facetectomy. Functional KSS, Oxford Knee Score, Kujalapatellofemoral value of patients with lateral patellar facetectomy were significantly (p<0.05) higher than in the group without lateral patellar facetectomy. Conclusion: It was found that lateral patellar facetectomy increased patellofemoral congruence both radiologically and clinically and contributed significantly to functional KSS, Oxford and Kujala clinical scores.
