Özet

Amaç: Dünyada en sık ölüme neden olan kanser türü olan akciğer kanserinde prognoz ve sağ kalımı öngörebilecek parametreleri değerlendirmeyi amaçladık. Gereç ve Yöntemler: 2010-2019 yılları arasında Dokuz Eylül Üniversitesi Tıp Fakültesi Hastanesi Göğüs Cerrahisi Anabilim Dalı'nda akciğer kanseri nedeniyle cerrahi rezeksiyon uygulanan evre 1 KHDAK tanılı 256 hastadan, kriterlere uyan 81 olgu çalışmaya dahil edildi. Tüm olgularda preoperatif, postoperatif 3. ay, 6. ay ve nüks dönemlerinde NLR, PLR ve mGPS değerleri ve sağ kalım süreleri retrospektif olarak incelendi. Veriler SPSS.25 ile istatistiksel olarak analiz edildi. Anlamlılık düzeyi olarak p<0.05 değeri kabul edildi. Bulgular: Hastaların 65'i (%80.2) erkek, 16'sı (%19.8) kadındı. 66 hastaya (%81.5) lobektomi, 8 hastaya (%9.9) lobektomi ve wedge, 3 hastaya (%3.7) sağ üst bilobektomi, 2 hastaya (%2.5) pnömonektomi ve 2 hastaya (%2.5) sağ alt bilobektomi uygulandı. Preop, postop 3.ay, 6.ay ve nüks NLR değerlerinin sağ kalım süresi üzerinde istatistiksel olarak anlamlı fark oluşturmadığı görüldü (p>0.05). Preop NLR değeriyle, postop 6.ay NLR değerleri kıyaslandığında sağ kalım süresi istatistiksel olarak anlamlı olduğu görüldü. (p: 0.003) vi Preop, postop 6.ay ve nüks NLR değerlerinin sağ kalım süresi üzerinde istatistiksel olarak anlamlı fark oluşturmadığı görüldü (p>0.05). Postop 3.ay PLR sağ kalım süresi üzerinde istatistiksel olarak anlamlı fark oluşturduğu saptandı (p<0,039). Preop ile postop 6.ay PLR değerleri kıyaslandığında sağ kalım süresi istatistiksel olarak anlamlı olduğu görüldü. (p: 0.047) Preop, postop 6.ay ve nüks mGPS arasında sağ kalım süresi üzerinde istatistiksel olarak anlamlı bir fark oluşturmadığı görüldü (p>0.05).Postop 3.ay mGPS değerinin sağ kalım süresi üzerinde istatistiksel olarak anlamlı olduğu görüldü (p< 0.05). Sonuç: Akciğer kanserinde prognozu öngörebilecek spesifik bir tümör belirteci mevcut değildir. Son zamanlarda inflamasyon biyobelirteçleri, tedavinin etkinliği, prognoz ve mortaliteyi tahmin etmek için kullanılmaya başlanmıştır. Yapılan çalışmalarda NLR, PLR ve mGPS'ın ileri evre kanserli hastalarda kötü prognoz ve sağ kalım ile ilişkili oldukları gösterilmiştir. Bildiğimiz kadarıyla post-operatif NLR, PLR, mGPS'in prognoz, sağkalım, nüks ile ilişkisine dair çok az çalışma bulunmaktadır. Bizim çalışmamızda ise erken evre akciğer kanserinde postop 6.ay NLR-PLR, postop 3. ay mGPS sağ kalım ile ilişkili olduğu saptanmıştır. Diğer değerlerin sağ kalım ile ilişkisiz olduğu saptanmıştır. Bu durum hastaların erken evre olması nedeniyle kansere karşı yeterince inflamatuar yanıt oluşamamış olabileceğini düşündürmüştür. Ayrıca çalışmamızın retrospektif olması nedeni ile bazı kısıtlılıklar bulunmaktadır. Bu konuda daha geniş merkezli ve prospektif çalışmaya ihtiyaç vardır.

Background: We aimed to evaluate the parameters that can predict prognosis and survival in lung cancer, which is the most common cancer type causing death in the world. Material and Methods: Among 256 patients diagnosed with stage 1 NSCLC who underwent surgical resection due to lung cancer in the Department of Chest Surgery at Dokuz Eylül University Medical Faculty Hospital between 2010-2019, 81 cases that met the criteria were included in the study. In all cases, preoperative, postoperative 3rd month, 6th month and recurrence periods NLR, PLR and mGPS values and survival times were retrospectively examined. Data were statistically analyzed with SPSS.25. P<0.05 was accepted as the significance level. Results: 65 (80.2%) of the patients were male and 16 (19.8%) were female. Lobectomy was performed in 66 patients (81.5%), lobectomy and wedge in 8 patients (9.9%), right upper bilobectomy in 3 patients (3.7%), pneumonectomy in 2 patients (2.5%), and right lower bilobectomy in 2 patients (2.5%). It was observed that there was no difference in survival time between preoperative, postoperative 3rd month, 6th month and recurrence NLR values viii (p>0.05). When the preoperative NLR value was compared with the postoperative 6th month NLR values, it was seen that there was a difference in survival time. (p: 0.003) It was observed that there was no difference in survival time between preoperative, postoperative 6th month and recurrence NLR values (p>0.05). There was a difference in survival time between postoperative 3rd month PLR (p<0.039). When the preoperative and postoperative 6th month PLR values were compared, it was seen that the survival time was permanently significant. (s: 0.047) It was observed that there was no permanent difference in survival time between preoperative, postoperative 6th month and relapse mGPS (p>0.05). It was observed that the 3rd month mGPS had a significant effect on survival time (p< 0.05). Conclusion: There is no specific tumor marker that can predict prognosis in lung cancer. Recently, inflammation biomarkers have been used to predict treatment efficacy, prognosis and mortality. Studies have shown that NLR, PLR and mGPS are associated with poor prognosis and survival in patients with advanced stage cancer. To our knowledge, there are very few studies on the relationships between postoperative NLR, PLR, mGPS and prognosis, survival, and recurrence. In our study, postoperative 6th month NLR-PLR and postoperative 3rd month mGPS were found to be associated with survival in early stage lung cancer. Other values were found to be unrelated to survival. This suggests that patients may not have developed an adequate inflammatory response to cancer due to their early stage. In addition, there are some limitations due to the retrospective nature of our study. A larger-scale and prospective study is needed on this subject.
