Özet

ÖZET Pankreas Cerrahisi Sonrası Egzokrin Pankreas Yetmezliği ve Tip 3 C Diyabet İçin Risk Faktörlerinin Araştırılması Amaç: Bu çalışmanın amacı, pankreas cerrahisi sonrası gelişen ekzokrin pankreas yetmezliği ve tip 3c diyabet için risk faktörlerini değerlendirmek ve postoperatif dönemde rutin biyokimyasal bulgular ve semptomlar ile ilişkili belirleyicileri ortaya koymaktır. Gereç ve Yöntem: Çalışmaya, pankreas cerrahisi geçiren 48 hasta dahil edilmiştir. Katılımcıların demografik özellikleri, preoperatif diyabet varlığı, uygulanan cerrahi prosedür tipi (distal pankreatektomi, total pankreatektomi, Whipple), tümörün TNM evrelemesi ve çeşitli biyokimyasal parametreleri (CA19-9, CA125, CEA, H-indeks, preoperatif pankreas çapı, BMI) değerlendirilmiştir. Postoperatif dönemde ise hastaların HbA1c, C-peptit ve elastaz düzeyleri ile hipoglisemik atak görülme sıklığı analiz edilmiştir. Preoperatif değişkenlerin postoperatif biyokimyasal sonuçlara etkisi regresyon analizleri ile araştırılmıştır. Bulgular: Katılımcıların yaş ortalaması 59.6±11.0 yıl, beden kitle indeksi ortalaması 24.1±4.5 kg/m² olarak bulunmuştur. Erkek oranı %56.3, kadın oranı %43.8'dir. Olguların %22.9'unda preoperatif diyabet saptanmıştır. En sık uygulanan cerrahi prosedür Whipple (%64.6) olurken, hastaların ortalama postoperatif takip süresi 21.9±27.2 aydır. Preoperatif değişkenler arasında, CA19-9 düzeyi postoperatif C-peptit düzeyi ile anlamlı pozitif ilişkili bulunmuştur (p = 0.014). Postoperatif HbA1c düzeyini öngören başlıca faktörler ise preoperatif diyabet varlığı (p < 0.001) ve CA19-9 düzeyidir (p = 0.045). Ayrıca BMI'nin postoperatif HbA1c üzerine anlamlı etkisi bulunmuştur (p = 0.030). Postoperatif dönemde hipoglisemik atak insidansı %4.2 olarak belirlenmiş ve hipoglisemik atak varlığı ile postoperatif HbA1c düzeyleri arasında anlamlı fark saptanmamıştır (p = 0.333). Postoperatif C-peptit düzeyinin tek anlamlı belirleyicisi postoperatif elastaz düzeyi (p = 0.048), serum elastaz düzeyinin ise yaş (p = 0.035) ve postoperatif C-peptit düzeyi (p = 0.048) olarak tespit edilmiştir. Çeşitli biyokimyasal parametreler arasında (HbA1c, C-peptit, elastaz) yapılan korelasyon analizinde anlamlı ilişki gözlenmemiştir. Sonuç: Pankreas cerrahisi sonrası gelişen ekzokrin pankreas yetmezliği ve tip 3c diyabet için preoperatif ve postoperatif dönemde çeşitli biyokimyasal ve klinik belirteçler viii kullanılabilir. Özellikle CA19-9 düzeyleri, preoperatif diyabet öyküsü ve BMI, postoperatif diyabet ve pankreatik fonksiyon kaybının öngörülmesinde önemli rol oynayabilir. Postoperatif elastaz düzeyi, pankreatik endokrin rezervin bağımsız belirleyicilerinden biridir. Bu bulgular, klinik pratikte hasta izleminde biyokimyasal parametrelerin dikkate alınmasının önemini göstermektedir.

ABSTRACT Investigation of Risk Factors for Exocrine Pancreatic Insufficiency and Type 3 C Diabetes After Pancreatic Surgery Aim: The aim of this study was to evaluate the risk factors for the development of exocrine pancreatic insufficiency and type 3c diabetes after pancreatic surgery, and to identify the predictors associated with routine biochemical findings and symptoms in the postoperative period. Materials and Methods: A total of 48 patients who underwent pancreatic surgery were included in the study. Demographic characteristics, presence of preoperative diabetes, type of surgical procedure (distal pancreatectomy, total pancreatectomy, Whipple procedure), TNM tumor staging, and various biochemical parameters (CA19-9, CA125, CEA, H-index, preoperative pancreatic diameter, BMI) were evaluated. In the postoperative period, HbA1c, C-peptide, and elastase levels, as well as the incidence of hypoglycemic attacks, were analyzed. The effects of preoperative variables on postoperative biochemical outcomes were investigated using regression analyses. Results: The mean age of the participants was 59.6±11.0 years, and the mean body mass index was 24.1±4.5 kg/m². The proportion of male and female patients was 56.3% and 43.8%, respectively. Preoperative diabetes was present in 22.9% of cases. The most common surgical procedure was the Whipple procedure (64.6%), and the mean postoperative followup period was 21.9±27.2 months. Among the preoperative variables, CA19-9 level was found to be significantly and positively associated with postoperative C-peptide level (p = 0.014). The main predictors of postoperative HbA1c were preoperative diabetes (p < 0.001) and CA19-9 level (p = 0.045). Additionally, BMI was found to have a significant effect on postoperative HbA1c (p = 0.030). The incidence of hypoglycemic attacks in the postoperative period was 4.2%, and there was no significant difference in postoperative HbA1c levels between patients with and without hypoglycemic attacks (p = 0.333). The only significant predictor of postoperative C-peptide level was postoperative elastase level (p = 0.048), whereas the significant predictors of serum elastase were age (p = 0.035) and postoperative C-peptide (p = 0.048). Correlation analyses among various biochemical parameters (HbA1c, C-peptide, elastase) revealed no significant associations. x Conclusion: Various biochemical and clinical markers can be used in both preoperative and postoperative periods to predict the development of exocrine pancreatic insufficiency and type 3c diabetes after pancreatic surgery. In particular, CA19-9 levels, preoperative history of diabetes, and BMI may play important roles in predicting postoperative diabetes and pancreatic functional loss. Postoperative elastase level is an independent determinant of pancreatic endocrine reserve. These findings highlight the importance of considering biochemical parameters in the clinical follow-up of patients after pancreatic surgery.
