Özet
Bu çalışma, ebe liderliğinde doğum öncesi, doğum ve doğum sonrası dönemde aldıkları sürekli bakımın perinatal sonuçlara etkisini incelemek amacıyla gerçekleştirilmiştir. Araştırma müdahale-kontrol gruplu son test tasarımlı deneysel tiptedir, 01.10.2024-20.03.2025 tarihleri arasında yürütüldü. Çalışmanın evrenini araştırmacının çalıştığı özel hastanenin gebe polikliniğine gelen gebeler oluşturmaktadır. Örneklem büyüklüğü hesaplanarak 50 çift (deney: 25, kontrol: 25) ile çalışma yürütüldü. Deney grubuna Ebe Liderli Sürekli Bakım Modeliyle temellendirilen ebelik bakımı verilirken kontrol grubuna rutin ebelik bakımı verildi. Araştırmada veri toplamak için Bilgilindirlmiş Gönüllü Onam Formu, Tanıtıcı Bilgi Formu, Doğum Bilgi Formu, Doğumda Verilen Destekleyici Bakıma İlişkin Kadının Algısı Ölçeği, Doğum Sonu Bilgi Formu, Annelerin Doğum Sonu Güvenlik Hisleri Ölçeği, LATCH Emzirme Tanımlama ve Değerlendirme Ölçeği, Edinburgh Doğum Sonu Depresyon Ölçeği kullanıldı. Araştırma bulgularında Doğum sonrası dönemde bebeğin bakımında desteği en çok kimden aldığı sorulduğunda deney grubunda en çok (%48) ebe desteği alındığı kontrol grubunda ise (%44) ailesinde destek aldığı, deney grubunda Doğumda Verilen Destekleyici Bakıma İlişkin Kadının Algısı Ölçeği toplam puan ortalaması 123,04±8,69; Kontrol grubunda Doğumda Verilen Destekleyici Bakıma İlişkin Kadının Algısı Ölçeği toplam puan ortalaması 105,52±15,42'dir. Deney grubunda Annelerin Doğum Sonu Güvenlik Hisleri Ölçeği toplam puan ortalaması 62,84±7,18; Kontrol grubunda Annelerin Doğum Sonu Güvenlik Hisleri Ölçeği toplam puan ortalaması 44,64±8,32'dir.Deney grubunda LATCH Emzirmeyi Tanılama ve Değerlendirme Ölçeği toplam puan ortalaması 7,92±2,01; Kontrol grubunda LATCH Emzirmeyi Tanılama ve Değerlendirme Ölçeği toplam puan ortalaması 6,12±1,53'tür.Deney grubunda Edinburgh Doğum Sonu Depresyon Ölçeği toplam puan ortalaması 7,88±5,92'dir. Kontrol grubunda Edinburgh Doğum Sonu Depresyon Ölçeği toplam puan ortalaması 9,56±7,59 olarak saptanmıştır. Doğumda Verilen Destekleyici Bakıma İlişkin Kadının Algısı ile Annelerin Doğum Sonu Güvenlik Hisleri arasında pozitif yönlü, orta düzeyde ve anlamlı bir ilişki bulunmuştur (r=0,427; p=0,033). Bu bulgu, doğum sırasında algılanan desteğin, doğum sonrası güvenlik hissini artırdığını göstermektedir. Annelerin Doğum Sonu Güvenlik Hisleri ile Edinburgh Doğum Sonu Depresyon Ölçeği puanları arasında negatif yönlü ve anlamlı bir ilişki (r=-0,512; p=0,009) saptanmıştır. Bu bulgu, güven hissi yüksek olan annelerde depresyon düzeylerinin daha düşük olduğunu göstermektedir. Sonuç olarak, bu çalışma ebe liderliğinde sunulan sürekli ve bütüncül bakımın yalnızca fiziksel sağlık sonuçlarını değil, aynı zamanda annelerin duygusal, bilişsel ve psikososyal iyilik hallerini de olumlu yönde etkilediğini göstermektedir. Bulgular, ebelik bakımında sürekliliğin artırılmasının kadın sağlığına olan katkısını vurgulamakta; doğumda ve sonrasında kadın merkezli, empatik ve güven temelli bakım modellerinin yaygınlaştırılması gerektiğine işaret etmektedir.

This study was conducted to examine the effect of midwife-led continuous care in the prenatal, birth and postpartum period on perinatal outcomes. The study was of experimental type with intervention-control group post-test design and was conducted between 01.10.2024-20.03.2025. The population of the study consisted of pregnant women who came to the pregnancy outpatient clinic of the private hospital where the researcher worked. The sample size was calculated and the study was conducted with 50 couples (experimental: 25, control: 25). The experimental group received midwifery care based on the Midwife-Led Continuous Care Model, while the control group received routine midwifery care. Informed Voluntary Consent Form, Introductory Information Form, Birth Information Form, Women's Perception of Supportive Care Given at Birth Scale, Postpartum Information Form, Mothers' Postpartum Feelings of Security Scale, LATCH Breastfeeding Identification and Assessment Scale, Edinburgh Postpartum Depression Scale were used to collect data in the study. In the findings of the study, when asked from whom the baby received the most support in the care of the baby in the postpartum period, the experimental group (48%) received the support of the midwife and the control group (44%) received support from the family, the total mean score of the Women's Perception of Supportive Care Provided at Birth Scale in the experimental group was 123.04±8.69; the total mean score of the Women's Perception of Supportive Care Provided at Birth Scale in the control group was 105.52±15.42. In the experimental group, the mean total score of the Postpartum Feelings of Safety Scale of Mothers was 62.84±7.18; in the control group, the mean total score of the Postpartum Feelings of Safety Scale of Mothers was 44.64±8.32. The mean total score of the LATCH Breastfeeding Identification and Assessment Scale in the experimental group was 7.92±2.01; the mean total score of the LATCH Breastfeeding Identification and Assessment Scale in the control group was 6.12±1.53.The mean total score of the Edinburgh Postpartum Depression Scale in the experimental group was 7.88±5.92. In the control group, the mean total score of the Edinburgh Postpartum Depression Scale was 9.56±7.59.A positive, moderate and significant relationship was found between Women's Perception of Supportive Care During Childbirth and Mothers' Feelings of Postpartum Security (r=0.427; p=0.033). This finding shows that the support perceived during labor increases the sense of postpartum security. A negative and significant relationship (r=-0.512; p=0.009) was found between the mothers' sense of postpartum safety and Edinburgh Postpartum Depression Scale scores. This finding indicates that mothers with a high sense of security have lower levels of depression. In conclusion, this study shows that midwife-led continuous and holistic care positively affects not only physical health outcomes but also emotional, cognitive and psychosocial well-being of mothers. The findings emphasize the contribution of increasing continuity in midwifery care to women's health, and point to the need for the dissemination of woman-centered, empathic and trust-based care models during and after childbirth.
