Özet
AMAÇ: Çalışmamızın amacı Agresif Büyük B Hücreli Lenfoma (ABBHL) tanılı hastalarda retiküloendotelyal sistem bileşenlerinde (karaciğer, dalak ve kemik iliği) diffüz artmış F-18 FDG tutulumunun hemogramdan elde edilen inflamatuar belirteçler ile olan ilişkisini CBC skoru (tam kan sayımı skoru) modeli üzerinden incelemek ve bu parametrelerin prognoz üzerine olan etkisini değerlendirmektir. GEREÇ VE YÖNTEM: Mayıs 2020–Ekim 2024 tarihleri arasında kliniğimizde evreleme amacıyla PET-BT çekilen 51 ABBHL hastası retrospektif olarak değerlendirildi. Anemisi, enfeksiyonu, sekonder malignitesi olanlar ve PET-BT'de fokal aktivite tutulumu veya biyopsiyle doğrulanmış kemik iliği tutulumu olan hastalar çalışmaya dahil edilmedi. Kliniğimizde yapılan evreleme PET görüntülemesi üzerinde dalak, vertebral kolon, iliak kemik ve femur proksimaline ait SUVmean değerleri ölçüldü. Alınan ölçümler karaciğerden alınan SUVmean değerine oranlanarak standardize edildi. Tedavi öncesi hemogramdan Nötrofil/Lenfosit, Platelet/Lenfosit ve Lenfosit/Monosit oranları hesaplanarak CBC skoru elde edildi. Hastalar progresif/non progresif, survival/non survival hasta grubu olarak gruplandırıldı. Elde edilen verilerin Progresyonsuz sağkalım (PS) ve Genel sağkalım (GS) ile ilişkisi incelendi. BULGULAR: Çalışmaya 31 erkek ve 20 kadın olmak üzere toplam 51 hasta dahil edildi. Hastaların yaş ortalaması 51,1 (dağılım: 6–86), ortalama takip süresi ise 19,9 ay (dağılım: 1–50 ay) idi. ECOG performans skorlamasına göre 39 hastanın performansı iyi (ECOG <2), 11 hastanın ise kötü (ECOG ≥2) olarak değerlendirildi; 1 hastanın verisine ulaşılamadı. Ann Arbor evrelemesine göre hastaların 12'si evre 1 (%23,5), 22'si evre 2 (%43,1) , 11'i evre 3 (%21,6) ve 6'sı evre 4 (%11,8) hastalık grubundaydı. B semptomu 10 hastada (%22,6), ekstranodal tutulum 12 hastada (%26,7), bulky kitle ise 17 hastada (%33,3) mevcuttu. Hastaların büyük çoğunluğu standart R-EPOCH veya R-CHOP kemoterapi rejimleriyle tedavi edildi. Tedavi sonrası PET/BT değerlendirmesinde 42 hastada tam yanıt, 2 hastada kısmi yanıt saptandı. 7 hastada progresyon gözlenirken, 3 hastada en az 6 aylık hastalıksız dönemin ardından nüks gelişti. Takip süresi boyunca toplam 11 hasta exitus ile sonuçlandı. Evreleme PET/BT incelemesinde Dalak/karaciğer SUVmean oranı progresyon gelişen grupta anlamlı olarak daha yüksekti (progresif grup ort: 0,96; non-progresif grup ort: 0,82; p=0,003). Aynı parametre, exitus gözlenen hastalarda da anlamlı şekilde daha yüksek bulundu (non-survival grup: ort. 0,92; survival grup: ort. 0,84; p=0,03). Kemik iliği/karaciğer SUVmean oranı da progresif hastalarda tedaviye yanıtlı olanlara kıyasla anlamlı yüksek saptandı. Ekstranodal tutulum ve bulky kitle varlığı, exitus gözlenen grupta daha sık olup genel sağkalım açısından olumsuz prognostik faktörler olarak değerlendirildi. R-IPI skoruna göre risk grupları arasında progresyon açısından istatistiksel olarak anlamlı fark izlenmedi (p=0,050), ancak exitus açısından sınırda anlamlılık saptandı (p=0,048). Laboratuvar verilerinde yüksek lenfosit düzeyleri, progresyonsuz sağkalım ve genel sağkalım açısından koruyucu prediktör olarak saptanırken (HR: 0,413 [0,186–0,918], p=0,030), yüksek nötrofil düzeyleri olumsuz prediktör olarak belirlendi (HR: 1,75 [1,257–2,435], p=0,001). Nötrofil/Lenfosit oranı (HR: 1,046 [1,017–1,075], p=0,002) ve Platelet/Lenfosit oranı (HR: 1,002 [1,001–1,004], p=0,000) da her iki sağkalım ölçütü açısından olumsuz prediktörlerdi. Bu parametreler kullanılarak oluşturulan CBC skoru modelinde progresyon ve exitus açısından CBC skorları arasında anlamlı fark olduğu tespit edildi (sırasıyla p:0,017, p:0,007). CBC skoru median değeri; tedaviye yanıtlı hastalarda 0 puan, progresyon gelişenlerde 2 puan, exitus olmayanlarda 0 puan, exitus olanlarda ise 3 puan olarak belirlendi. CBC skoru ile tedavi yanıtı arasında orta düzeyde negatif korelasyon saptandı (p=0,012; r=-0,350). Kaplan–Meier analizlerine göre CBC skoru arttıkça sağkalım süresi anlamlı şekilde azalmaktaydı. SONUÇ: Elde ettiğimiz sonuçlar dalaktaki diffüz artmış aktivite tutulumunun, tam kan sayımından elde ettiğimiz inflamatuar belirteçler ile korele olduğunu ve prognostik öneme sahip olabileceği hipotezimizi desteklemektedir. Ayrıca hemogram parametrelerinden oluşturulan CBC skoru modelinin progresyon veya exitus saptanan hastalarda anlamlı derecede yüksek saptanması, CBC skoru arttıkça progresyonsuz sağkalım ve genel sağkalımın azalması bu modelin prognostik gösterge olarak kullanılabileceğini düşündürmektedir. Ancak bu modelin klinik pratikte prognostik gösterge olarak kullanılabilmesi için daha yüksek hasta sayılı ve daha uzun takip süreleri çalışmalar ile desteklenmesi gerekmektedir.

AIM: The aim of this study was to investigate the relationship between diffusely increased F-18 FDG uptake in components of the reticuloendothelial system (liver, spleen, and bone marrow) and inflammatory markers derived from complete blood count (CBC) in patients diagnosed with Aggressive Large B-Cell Lymphoma (ALBCL), using the CBC score model, and to evaluate the prognostic impact of these parameters. MATERİALS AND METHODS: Fifty-one patients diagnosed with ALBCL who underwent staging PET/CT in our clinic between May 2020 and October 2024 were retrospectively analyzed. Patients with anemia, infection, secondary malignancy, focal uptake on PET/CT, or bone marrow involvement confirmed by biopsy were excluded from the study. SUVmean values were measured from the spleen, vertebral column, iliac bone, and proximal femur on staging PET/CT and standardized by calculating the ratio to liver SUVmean. Pre-treatment blood counts were used to calculate Neutrophil/Lymphocyte, Platelet/Lymphocyte, and Lymphocyte/Monocyte ratios, from which the CBC score was derived. Patients were categorized into progressive/non-progressive and survival/non-survival groups. The relationship between obtained parameters and progression-free survival (PFS) and overall survival (OS) was assessed. RESULTS: The study included 51 patients, 31 male and 20 female. The mean age was 51.1 years (range: 6–86), and the mean follow-up duration was 19.9 months (range: 1–50). According to ECOG performance status, 39 patients had good performance (ECOG <2) and 11 had poor performance (ECOG ≥2); data for one patient were unavailable. Based on Ann Arbor staging, 12 patients (23.5%) were stage 1, 22 (43.1%) stage 2, 11 (21.6%) stage 3, and 6 (11.8%) stage 4. B symptoms were present in 10 patients (22.6%), extranodal involvement in 12 (26.7%), and bulky disease in 17 (33.3%). Most patients received standard R-EPOCH or R-CHOP chemotherapy regimens. Post-treatment PET/CT revealed complete response in 42 patients, partial response in 2, and progression in 7. Three patients experienced relapse after at least six months of remission. A total of 11 patients died during follow-up. The spleen/liver SUVmean ratio was significantly higher in the progression group compared to the non-progressive group (mean 0.96 vs. 0.82, p=0.003). Similarly, this ratio was significantly higher in deceased patients than in survivors (mean 0.92 vs. 0.84, p=0.03). The bone marrow/liver SUVmean ratio was also significantly higher in the progressive group compared to those with treatment response. Extranodal involvement and presence of bulky disease were more frequent in the deceased group and were considered poor prognostic factors for overall survival. No statistically significant differences were observed among R-IPI risk groups in terms of progression(p=0.050), though mortality showed borderline significance (p=0.048). No significant difference was found between R-IPI scores and treatment response (complete, partial, or progressive disease) (p=0.11), but a weak negative correlation was observed between R-IPI and treatment response (p=0.018; r=-0.333). In laboratory parameters, higher lymphocyte counts were identified as protective predictors for both PFS and OS (HR: 0.413 [0.186–0.918], p=0.030), while elevated neutrophil counts were adverse predictors (HR: 1.75 [1.257–2.435], p=0.001). Neutrophil/Lymphocyte (HR: 1.046 [1.017–1.075], p=0.002) and Platelet/Lymphocyte ratios (HR: 1.002 [1.001–1.004], p=0.000) were also found to be negative predictors for both survival metrics. The CBC score model created using these parameters revealed significant differences in CBC scores between progressive and non-progressive patients (p=0.017) and between deceased and surviving patients (p=0.007). Median CBC scores were: 0 in treatment-responsive patients, 2 in progressive cases, 0 in surviving patients, and 3 in deceased cases. A moderate negative correlation was found between CBC score and treatment response (p=0.012; r=-0.350). Kaplan–Meier survival analysis demonstrated that increasing CBC scores were significantly associated with shorter survival duration. CONCLUSİON: Our findings support the hypothesis that diffusely increased splenic FDG uptake correlates with inflammatory markers derived from complete blood count and may carry prognostic significance. Furthermore, the CBC score model, found to be significantly higher in patients with progression or mortality, appears to be a potential prognostic indicator. However, validation in larger cohorts with longer follow-up is necessary before it can be implemented in clinical practice.
