Özet

ÖZET Amaç: İskemik inme sonrası subakut ve kronik dönemde, hemiplejik ekstremitede görülen kompleks bölgesel ağrı sendromunun (KBAS) tedavisinde, konvansiyonel rehabilitasyonla birlikte uygulanan repetitif transkraniyal manyetik stimülasyonun (rTMS) ağrı, motor fonksiyon ve yaşam kalitesi üzerindeki etkilerini araştırmaktır. Gereç ve Yöntem: Çalışmaya dahil edilen 40 kronik iskemik inmeli hasta randomizasyon şemasına göre, aktif rTMS grubu (n=15), taklit rTMS grubu (n=15) ve kontrol grubu (n= 10) olmak üzere üç gruba ayrıldı. Repetetif TMS grubuna konvansiyonel rehabilitasyon programı ile kombine etkilenen hemisfer üst ekstremite motor (M1) alanı hedeflenerek 10 seans 10 Hz'lik yüksek frekanslı aktif veya taklit rTMS uygulandı. Hastalar tedavi öncesinde, tedavi bitiminde ve tedavi bitiminden 4 hafta sonra olmak üzere toplamda 3 defa değerlendirmeye alındı. Değerlendirmede ağrı için Vizuel Analog Skala (VAS), Brunnstrom Evrelemesi (BR) ve Fugl-Meyer Üst Ekstremite Motor Değerlendirme Ölçeği (FMDÖ) ve İnmeye Özgü Yaşam Kalitesi Ölçeği (İYKÖ) kullanıldı. Bulgular: Tedavi bitiminde, konvansiyonel rehabilitasyon programı ile birlikte uygulanan aktif rTMS'nin, subakut ve kronik iskemik inmeli hastalarda gelişen KBAS üzerinde ağrı azaltılmasında etkili olduğu görülmüştür (p˂0.05). Ancak tedaviden 4 hafta sonraki değerlendirmede, bu etkinin devam etmediği ve gruplar arasındaki VAS ölçümleri arasında anlamlı bir fark bulunmadığı görülmüştür (p> 0.05). Üst ekstremite fonksiyonel iyileşmesinde, aktif rTMS grubunda elde edilen sonuçlar, diğer gruplara kıyasla daha iyi bulunmuştur. Hastaların gruplarına göre yapılan değerlendirmelerde, aktif rTMS grubundaki hastaların tedavi öncesi ve tedavi sonrası Brunnstrom ve Fugl-Meyer total ölçümlerindeki değişim miktarları, diğer gruplardan anlamlı düzeyde daha yüksek bulunmuştur (p < .05). Ayrıca, tedaviden 4 hafta sonraki Brunnstrom ölçüm ortalamaları, tedavi öncesi ve tedavi sonrasına göre daha yüksek saptanmıştır. Gruplar arası yapılan değerlendirmelerde, tedavi sonrasında ve tedavi bitiminden 4 hafta sonra yaşam kalitesinde anlamlı bir iyileşme gözlemlenmemiştir. Sonuç: Bu çalışma, subakut ve kronik iskemik inmeli hastalarda gelişen KBAS'ın tedavisinde, konvansiyonel rehabilitasyon ile birlikte uygulanan rTMS'nin ağrı kontrolü üzerinde erken dönemde olumlu bir etki sağladığını ortaya koymuştur. Ancak, bu etkinin tedavi bitiminden 4 hafta sonra kaybolduğu ve yaşam kalitesi üzerinde anlamlı bir iyileşme sağlamadığı görülmüştür. Buna rağmen, rTMS uygulaması ile elde edilen üst ekstremite fonksiyonel iyileşmeleri, diğer gruplara göre belirgin şekilde daha yüksek bulunmuş olup rTMS'nin hem ağrı yönetimine katkısı hem de üst ekstremite motor fonksiyon iyileşmesine olan olumlu etkileri nedeniyle, inmeli hastalarda KBAS tedavisinde yeni bir tedavi yöntemi olarak uygulanmasının avantaj sağlayabileceği; bu nedenle rTMS için kontrendikasyonu olmayan hastalarda tedavi programında yer alabileceği düşünülmektedir.

Aim: The aim is to investigate the effects of repetitive transcranial magnetic stimulation, applied in combination with conventional rehabilitation, on pain control, motor function improvement, and quality of life in the treatment of complex regional pain syndrome in the hemiplegic extremity during the subacute and chronic phases following ischemic stroke. Material and Methods: A total of 40 chronic ischemic stroke patients included in the study were divided into three groups according to the randomization scheme: the active rTMS group (n=15), the sham rTMS group (n=15), and the control group (n=10). The rTMS group received 5 sessions of high-frequency active or sham rTMS at 10 Hz, targeting the motor area (M1) of the affected hemisphere, combined with conventional rehabilitation. Patients were evaluated three times: before treatment, immediately after treatment, and 4 weeks after treatment. The evaluations included the Visual Analog Scale, Brunnstrom Stage,Upper Extremity Fugl-Meyer Motor Function Scale, and the Stroke-Specific Quality of Life Scale. Results: At the end of the treatment, it was observed that active rTMS, applied in combination with conventional rehabilitation, was effective in reducing pain in patients with complex regional pain syndrome (CRPS) developing after subacute and chronic ischemic stroke (p <0.05). However, in the evaluation conducted 4 weeks after treatment, it was found that this effect did not persist, and there was no significant difference in the VAS measurements between the groups (p> 0.05). In terms of upper extremity functional recovery, the results in the active rTMS group were found to be better compared to the other groups. In the group-based evaluations, the changes in the Brunnstrom and Fugl-Meyer total scores before and after treatment were significantly different in the active rTMS group compared to the other groups (p <0.05). Additionally, 4 weeks after treatment, the Brunnstrom scores were found to be higher compared to both the pre-treatment and post-treatment scores. In the group-based evaluations, no significant improvement in quality of life was observed either after treatment or 4 weeks post-treatment. Conclusion: Our study has demonstrated that repetitive transcranial magnetic stimulation (rTMS), when applied in conjunction with conventional rehabilitation for the treatment of complex regional pain syndrome (CRPS) in subacute and chronic ischemic stroke patients, has a positive effect on pain control. However, this effect was found to diminish four weeks after the treatment, and no significant improvement in quality of life was observed. Nevertheless, the functional improvements in the upper extremity achieved through rTMS were significantly higher than those in the other groups. Due to rTMS's contributions to pain management and its positive effects on upper extremity motor function, it may offer advantages as a new treatment option for CRPS in stroke patients; therefore, it is suggested that rTMS could be included in the treatment regimen for patients without contraindications for TMS.
