Özet
Menopoz, bir kadının adet döngülerinin kalıcı olarak sona erdiği ve doğal üreme kapasitesinin bittiği biyolojik bir süreçtir. Çalışmamızın amacı postmenopozal obezitede pilates egzersizlerinin menopozal semptomlar üzerine etkisini belirlemek ve kontrol grubu ile kıyaslamaktır. Çalışma tek kör randomize kontrollü çalışma olarak tasarlandı. Çalışmaya postmenopozal dönemde olan 45 obez birey dahil edildi. Hastalar randomize olarak 3 gruba ayrıldı ve 2 gruba da 8 hafta boyunca, haftada 2 gün toplam 16 seans tedavi uygulandı. 1. Gruba reformer pilates egzersizleri , 2. Gruba mat pilates egzersizleri uygulandı. 3. Grup Kontrol grubuna ise yaşam tarzı değişikliklerini içeren fiziksel aktivite temelli broşür dağıtıldı. Tedavi öncesi ve sonrası toplam iki kere değerlendirme yapıldı. Değerlendirmede menopozal semptomlar Menopozal Semptomları Değerlendirme Ölçeği (MSDÖ) , yaşam kalitesi Menopoza Özel Yaşam Kalitesi Değerlendirme Ölçeği (MÖYKÖ) , depresyon Beck Depresyon Ölçeği (BDÖ) , uyku kalitesini Pittsburgh Uyku Kalitesi İndeksi (PUKİ) ile ölçüldü. Aerobik kapasite Altı Dakika Yürüme Testi ( 6 DYT) , vücut kompozisyonu vücut analiz baskülü ( TANİTA) , kan lipit profili ise alınan kan laboratuvar analizleri sonuçları kullanıldı. Verilerin karşılaştırılması için istatistiksel analizler IBM SPSS Statistics 21.0 ile yapıldı. Yapılan değerlendirmeler sonucunda hem reformer hem mat pilates grubunda MSDÖ ve MÖYKÖ puanlarında istatistiksel olarak anlamlı bir farklılık tespit edildi (p<0.001). Tüm gruplarda BDÖ ve PUKİ puanlarının ölçümleri arasında istatistiksel olarak anlamlı bir farklılık tespit edildi (p<0.05). Vücut kompozisyonu analizinde reformer pilates grubu mat pilates grubuna göre istatistiksel olarak daha etkili olduğu görüldü (p<0.05). Kan lipit profilinde ve aerobik kapasite analizleri sonucunda her iki egzersiz grubunun istatistiksel olarak etkili olduğu görüldü ancak grupların birbirine göre herhangi bir üstünlüğü sağlanmadı. Çalışmamıza göre, pilates egzersizlerinin postmenopozal obez bireylerde genel sağlık durumu, ruhsal iyi oluş, uyku kalitesi ve metabolik göstergeler üzerinde çok yönlü olumlu etkileri bulunmaktadır. Bu doğrultuda, reformer pilates egzersizlerinin menopozal semptomlar üzerinde de etkili olabileceği görüşündeyiz.

Menopause is a biological process in which a woman's menstrual cycles permanently end and her natural reproductive capacity ends. The aim of our study was to determine the effects of pilates exercises on menopausal symptoms in postmenopausal obesity and to compare them with the control group. The study was designed as a single-blind randomized controlled trial. 45 obese individuals in the postmenopausal period were included in the study. The patients were randomly divided into 3 groups and both groups received a total of 16 treatment sessions, 2 days a week for 8 weeks. Reformer pilates exercises were applied to Group 1, and mat pilates exercises were applied to Group 2. A physical activity-based brochure including lifestyle changes was distributed to the Control Group in Group 3. A total of two evaluations were made before and after the treatment. In the evaluation, menopausal symptoms were measured with the Menopausal Symptoms Assessment Scale (MRS), quality of life with the Menopause-Specific Quality of Life Assessment Scale (MENNQL), depression with the Beck Depression Inventory (BDI), and sleep quality with the Pittsburgh Sleep Quality Index (PSQI). Aerobic capacity was measured with the Six-Minute Walk Test (6 MWT), body composition with the body analysis scale (TANITA), and blood lipid profile was measured with the results of laboratory analysis of the blood. Statistical analyses were performed using IBM SPSS Statistics 21.0 to compare the data. As a result of the evaluations, a statistically significant difference was found in the MRS and MENNQL scores in both the reformer and mat pilates groups (p<0.001). A statistically significant difference was found between the measurements of BDI and PSQI scores in all groups (p<0.05). In body composition analysis, the reformer pilates group was found to be statistically more effective than the mat pilates group (p<0.05). As a result of blood lipid profile and aerobic capacity analysis, both exercise groups were found to be statistically effective, but no superiority was achieved between the groups. According to our study, pilates exercises have multifaceted positive effects on general health status, mental well-being, sleep quality and metabolic indicators in postmenopausal obese individuals. In this context, we believe that reformer pilates exercises may also be effective on menopausal symptoms.
