Özet
Bu çalışma, 6 Şubat 2023'te meydana gelen Kahramanmaraş merkezli depremlerden etkilenen bireylerde EMDR 2.0 grup müdahalesinin travma sonrası stres bozukluğu (TSSB), depresyon, anksiyete ve stres semptomları üzerindeki etkilerini incelemeyi amaçlamaktadır. EMDR 2.0, klasik EMDR protokolüne göre daha kısa sürede daha etkin sonuçlar elde etmek amacıyla geliştirilmiş, bilişsel ve motor görevlerle çalışma belleğini daha yoğun biçimde meşgul eden, çok katmanlı bir müdahale modelidir. Çalışma, yarı deneysel ön-test, son-test ve izlem testli desenle yürütülmüştür. Katılımcılar Malatya ilinde konteyner kentlerde yaşayan ve TSSB riski taşıyan kadın bireylerden oluşmaktadır. Başlangıçta 70 kişiyle başlanan çalışmada, süreç içinde yaşanan örneklem kayıpları nedeniyle deney grubunda 31, kontrol grubunda 27 olmak üzere toplam 58 katılımcı ile analizler tamamlanmıştır. Müdahale üç oturumdan oluşan EMDR 2.0 grup protokolü şeklinde uygulanmıştır. Veriler, Travma Sonrası Stres Bozukluğu Kontrol Listesi (PCL-5) ve Depresyon Anksiyete Stres Ölçeği (DASS-21) kullanılarak toplanmıştır. Bulgular, deney grubunda hem TSSB hem de depresyon, anksiyete ve stres semptomlarında anlamlı düzeyde azalma olduğunu ortaya koymuştur. PCL-5 toplam puanlarındaki azalma η²=0.915 gibi yüksek bir etki büyüklüğüyle desteklenmiştir. Yeniden yaşantılama, kaçınma ve aşırı uyarılma alt boyutlarında da izlem ölçümlerinde belirgin düşüşler gözlenmiştir. DASS-21 bulguları ise, depresyon (t=5.387, p=.000), anksiyete (t=5.769, p=.000) ve stres (t=4.938, p=.000) alt boyutlarında anlamlı düzelmeler olduğunu göstermiştir. Bu sonuçlar, EMDR 2.0'ın travma sonrası duygudurum semptomları üzerinde transdiagnostik bir etkiye sahip olabileceğini düşündürmektedir. Çalışmanın özgün katkılarından biri, EMDR 2.0'ın grup formatında, kısa sürede ve afet sonrası sahada uygulanabilirliğini test eden ilk araştırmalardan biri olmasıdır. Müdahale, zaman ve kaynak kısıtlılığı olan kriz koşullarında dahi etkin bir psikososyal destek sunabilmektedir. Sınırlılıklar arasında yalnızca kadın katılımcıların yer alması, kısa izlem süresi ve özbildirim temelli veri kullanımı bulunmaktadır. Bununla birlikte, elde edilen bulgular, EMDR 2.0'ın afet sonrası ruh sağlığı müdahalelerinde bütüncül, kısa süreli ve etkili bir yaklaşım olabileceğini göstermektedir.

This study investigates the effectiveness of the EMDR 2.0 group intervention on post-traumatic stress disorder (PTSD), depression, anxiety, and stress symptoms among adult women affected by the earthquakes that struck Kahramanmaraş, Türkiye, on February 6, 2023. EMDR 2.0 is a recent adaptation of the standard EMDR protocol, incorporating cognitive and motor dual-tasking strategies to intensively tax working memory, thereby facilitating the desensitization and reprocessing of traumatic memories. Designed to produce therapeutic outcomes in fewer sessions, EMDR 2.0 has gained attention for its applicability in acute crisis contexts. The study utilized a quasi-experimental, pre-test, post-test, and follow-up design. Participants were selected from women living in temporary container housing in the city of Malatya, who had been identified as at risk for PTSD. The final sample consisted of 58 participants, with 31 in the intervention group and 27 in the control group, following attrition. The intervention comprised three EMDR 2.0 group sessions administered over one week. Data were collected using the PTSD Checklist for DSM-5 (PCL-5) and the Depression Anxiety Stress Scales (DASS-21) at three time points. The findings revealed significant symptom reduction in the intervention group across all measured domains. PTSD scores declined markedly with an effect size of η² = 0.915. Notably, the subscales of re-experiencing, avoidance, and hyperarousal showed consistent improvements at follow-up. In terms of mood symptoms, significant reductions were observed in depression (t=5.387, p=.000), anxiety (t=5.769, p=.000), and stress (t=4.938, p=.000), underscoring the transdiagnostic potential of the EMDR 2.0 approach. One of the unique contributions of this research lies in its application of EMDR 2.0 in a post-disaster context through a group-based, short-term, and field-ready format. This protocol proved to be both time- and cost-effective, especially in crisis environments with limited access to individualized care. Limitations of the study include a gender-restricted sample, limited geographic representation, and reliance on self-report measures. Nevertheless, the results provide preliminary evidence that EMDR 2.0 can serve as an effective, scalable, and holistic mental health intervention in the aftermath of natural disasters.
