Özet

Amaç: Multipl skleroz (MS) genetik ve çevresel etmenlerin neden olduğu, merkezi sinir sisteminde demiyelinizasyonla karakterize nöron ve akson kaybıyla ilişkili otoimmün bir hastalıktır. Hastalığın seyrine göre kendi içinde gruplandırılmaktadır. Günümüzde her geçen gün aydınlatılan moleküler mekanizmalarla birlikte hastalığa ilişkin tanı ve tedavi yöntemleri artmaktadır. Genetik ve çevresel etmenlerin yanı sıra immün homeostazi hastalığın patogenezinde önemli rol oynamaktadır. İmmün kontrol noktası inhibitörleri olarak bilinen PD-1 ve PD-L1 homeostazinin sağlanmasında immün kaskadın önemli parçalarından biridir. PI3K-AKT-mTOR yolağı hücre döngüsünün düzenlenmesinde rol oynayan önemli sinyal yolaklarından biridir. Yapılan çalışmalarda PD1-PD-L1 sinyal yolağının PI3K-AKT-mTOR yolağı ile birlikte T hücrelerinde otoimmüniteyi düzenleyebildiği bildirilmiştir. Fingolimod, T hücrelerini lenf nodunda tutarak aşırı immün cevabı önleyen S1P reseptör modülatörüdür. Bu çalışmada RRMS (tekrarlayan-kötüye giden MS) hastalarında fingolimodun PD-1/PD-L1 ve PI3K/Akt/mTOR yolakları ile ilişkili genlerin ekspresyonu ve sitokin seviyeleri üzerine etkisinin araştırılması amaçlandı. Materyal ve Metot: Çalışmada, her biri 25 bireyden oluşan dört grup oluşturuldu: sağlıklı bireylerden oluşan kontrol grubu, naif RRMS hasta grubu, fingolimod tedavisi öncesi RRMS hasta grubu, fingolimod tedavisi sonrası RRMS hasta grubu. Bireylerden alınan periferik kan örneklerinden PD-1/PD-L1 ve PI3K/Akt/mTOR yolaklarıyla ilişkili PTEN, PDK1, AKT1, mTOR, FOXP3, STAT3, PD1 (PDCD1), PD-L1 (CD274) ve GAPDH genlerinin ekspresyon seviyeleri Realtime PCR yöntemi ile analiz edildi.Ayrıca alınan serum örnekleri aracılığıyla IL-2, IL-6, IL-12, IL-17 ve IL-22 düzeyleri enzime bağlı immünosorbent test (ELISA) yöntemiyle analiz edildi. Elde edilen tüm bulgular istatistiksel açıdan değerlendirildi. İstatistiksel olarak anlamlılık seviyesi p<0.05 olarak kabul edildi. Bulgular: Değerlendirilen her bir parametrede istatistiksel olarak anlamlı bulgular tespit edildi. Fingolimod tedavisi sonrası RRMS hasta grubu ile naif RRMS hasta grubu karşılaştırıldığında fingolimod tedavisi sonrası grupta PDCD1, CD274, PTEN ve FOXP3 ekspresyonunda artış tespit edildi. Naif RRMS hasta grubu diğer gruplarla karşılaştırıldığında bu grupta AKT1, mTOR, PDK1 ve STAT3 gen ekspresyonlarında ve IL-2, IL-6, IL-12, IL-17 ve IL-22 sitokin düzeylerinde artış tespit edildi. Bu bulgular, fingolimod tedavisinin PD-1/PD-L1 yolağı üzerinden T hücre yanıtlarını baskılayarak ve FOXP3+ Treg hücre aktivitesini artırarak immün toleransı desteklediğini göstermektedir. Öte yandan, naif RRMS hastalarında PI3K/AKT/mTOR yolak aktivasyonunun ve proinflamatuvar sitokin düzeylerinin yüksek olması, hastalığın aktif döneminde proinflamatuvar T hücre yanıtlarının baskın olduğunu ortaya koymaktadır. Bu sonuçlar, fingolimodun özellikle PD-1/PD-L1 aksisi ve mTOR sinyallemesi üzerinden RRMS'deki immün regülasyona olan etkisini göstermektedir.

Aim: Multiple sclerosis (MS) is an autoimmune disease characterized by demyelination in the central nervous system and associated with neuronal and axonal loss, caused by both genetic and environmental factors. The disease is classified into subtypes based on its clinical course. With the increasing elucidation of molecular mechanisms, diagnostic and therapeutic methods for MS are expanding. In addition to genetic and environmental factors, immune homeostasis plays a significant role in the pathogenesis of the disease. PD-1 and PD-L1, known as immune checkpoint inhibitors, are crucial components of the immune cascade in maintaining homeostasis. The PI3K-AKT-mTOR pathway is one of the key signaling pathways involved in the regulation of the cell cycle. Studies have declared that the PD-1/PD-L1 signaling pathway, together with the PI3K-AKT-mTOR pathway, can regulate autoimmunity in T cells. Fingolimod is an S1P receptor modulator that prevents excessive immune responses by retaining T cells in the lymph nodes. This study aimed to investigate the effects of fingolimod on the expression of genes and cytokine levels associated with the PD-1/PD-L1 and PI3K–AKT–mTOR pathways in RRMS patients. Materials and Methods: The study included four groups, each consisting of 25 participants: a healthy individuals, a naïve RRMS patient group, a pre fingolimod treatment RRMS patient group and a post fingolimod treatment RRMS patient group. Peripheral blood samples obtained from individuals were used to analyze the expression levels of genes associated with the PD-1/PD-L1 and PI3K/Akt/mTOR pathways, including PTEN, PDK1, AKT1, mTOR, FOXP3, STAT3, PD-1 (PDCD1), PD-L1 (CD274), and GAPDH, using the realtime PCR method. In addition, serum samples were analyzed to determine the concentrations of IL-2, IL-6, IL-12, IL-17, and IL-22 via the enzyme-linked immunosorbent assay (ELISA) method. All data obtained were evaluated to statistical analysis, and a p-value of <0.05 was considered as a statistically significant. Results: Each parameter were identified as a statistically significant. When the post-fingolimod treatment patient group was compared with the naïve MS group, an increase in the expression of PDCD1, CD274, PTEN, and FOXP3 was observed. When the naïve MS group was compared with other groups, an increase in the expression of AKT1, mTOR, PDK1, and STAT3 and increase levels of IL-2, IL-12, IL-22, IL-17 and IL-6 were detected. These findings indicate that fingolimod attenuates immune tolerance by suppressing T cell responses through the PD-1/PD-L1 pathway and enhancing the activity of FOXP3+ Treg cells. On the other hand, the high activation of the PI3K/AKT/mTOR pathway and elevated levels of proinflammatory cytokines in naïve RRMS patients reveal that proinflammatory T cell responses dominate during the active phase of the disease. These results demonstrate fingolimod's impact on immune regulation in RRMS, particularly through the PD-1/PD-L1 axis and mTOR signaling.
