The aim of this study is to investigate the effect of deep water-based core strengthening exercises on balance and muscle strength in sedentary female individuals. For this purpose, 16 sedentary women (age: 31.81±3.33 years; body mass: 64.56±4.81 kg; height: 164.81±4.63 cm; BMI: 32.42±1.83 kg/m²) voluntarily participated in the study. Participants were randomly divided into experimental (n = 8) and control (n = 8) groups. While the experimental group was subjected to a deep water core strengthening exercise program three days a week for 12 weeks, no exercise intervention was applied to the control group. The participants were subjected to the Y balance test, Flamingo balance test, and one repetition maximum (1RM) strength tests for the shoulder, chest, and leg regions at four different periods: 0 (pre-test), 4th, 8th, and 12th weeks. In the study, the Friedman test was used for between-group comparisons of quantitative data that did not show normal distribution, and the Wilcoxon Signed Ranks test was used for post-hoc analyses. For quantitative data with normal distribution, two-way ANOVA test in repeated measures was used. According to the analysis results, following the 12-week deep water core exercise protocol, upper extremity strength significantly increased from 19.82±2.61 kg to 28.54±2.87 kg with bench press and from a shoulder press median of 28.12 (IQR 0.10) to 41.62 (IQR 2.24) (p < 0.001). In lower extremity strength leg press measurements, a continuous increase from 64.42±7.65 kg to 82.93±4.83 kg was detected (p < 0.001), and it was determined that strength development did not plateau, with significance maintained even between the 8th and 12th weeks. The dynamic balance composite Y balance score increased from 76.50±1.84 to 83.73±2.26 (p < 0.001), but with the increase being insignificant between the 8th and 12th weeks (p > 0.05), it was observed that the increase accelerated in the first 8 weeks entered a plateau phase in the last 4 weeks. The static balance Flamingo test median decreased from 7.00 (IQR 0.75) to 2.00 (IQR 0.75) (p < 0.001); however, the insignificance of the difference between the 8th–12th weeks (p > 0.05) showed a similar stagnation. In the control group, no general time effect was detected in either strength or dynamic and static balance measurements, and only limited differences were observed in some pairwise week comparisons (p > 0.05). As a result, it is seen that deep water core strengthening exercises significantly improve muscle strength measured by bench press, shoulder press and leg press, as well as dynamic and static balance performance in sedentary women. These findings reveal that water-based exercises are an effective method in preventing functional losses due to middle age and inactivity. Furthermore, offering an adaptable and long-term preferable protocol demonstrates that it constitutes a safe and sustainable physical activity alternative for sedentary individuals.
