Özet
Şark Çıbanı kendiliğinden iyileştiğinde vücudun çeşitli bölgelerinde yara izi bırakan ve Dünya Sağlık Örgütü tarafından ihmal edilmiş tropikal hastalıklardan biri olarak sınıflandırılan vektör kaynaklı bir bulaşıcı hastalıktır. Doksan iki ülkede endemik kabul edilen bu hastalık yıllık 700.000 ile 1.200.000 arasında değişen bir vaka sayısına sahiptir. Türkiye'nin güneydoğu bölgelerinde sık görülen hastalık, en çok Şanlıurfa'da yayılım göstermektedir. Hastalık hem yaşam kalitesini etkilemekte hem de bireyler, aileler, sağlık sistemi ve toplum açısından ciddi bir ekonomik yük oluşturmaktadır. Şark Çıbanı'nın hastaların yaşam kalitesine etkilerinin incelenmesi ve hastalık maliyetinin hesaplanması, bu alanda yürütülecek politika ve uygulamalar için önemlidir. Bu araştırmanın amacı; şark çıbanının hasta ve hasta yakını, hizmet sunucusu ve toplumsal perspektiflerden maliyetini hesaplamak, hastaların yaşam kalitesini ölçmek ve maliyet ile yaşam kalitesi arasındaki ilişkiyi incelemektir. Araştırma, Şanlıurfa ilinde Şark Çıbanı Tanı ve Tedavi Merkezi'nde 2024 yılı Ocak-Eylül döneminde şark çıbanı tanısı konulmuş 136 hasta üzerinde prospektif olarak gerçekleştirilmiştir. Hastalar; bebekler (0-5 yaş), çocuklar (6-17 yaş) ve yetişkinler (17+) olmak üzere üç gruba ayrılmış; hasta ve hasta yakını, hizmet sunucusu ve toplumsal perspektiflerden hastalık maliyeti hesaplanmıştır. Hastaların yaşam kalitesi ise, Dermatolojik Yaşam Kalitesi İndeksi, Çocuklarda Dermatolojik Yaşam Kalitesi İndeksi ve Bebeklerde Dermatolojik Yaşam Kalitesi İndeksi kullanılarak değerlendirilmiştir. Araştırmada kapsamında hasta ve hasta yakını perspektifinden toplam hastalık maliyeti 297.037 TL (9.196,19 $), ortalama hastalık maliyeti 2.184,10 TL (67,62 $); hizmet sunucusu perspektifinden toplam hastalık maliyeti 257.440,35 TL (8.717,80 $), ortalama hastalık maliyeti 1.893,40 TL (58,62 $) ve toplumsal perspektiften toplam hastalık maliyeti 554.477,35 TL (17.166,48 $), ortalama hastalık maliyeti 4.077,04 TL (126,22 $) olarak hesaplanmıştır. Araştırmada hasta ve hasta yakını perspektifinden gerçekleşen giderler, hizmet sunucusu perspektifinden gerçekleşen giderlerden daha yüksek çıkmıştır. Araştırmada yaşam kalitesi değerlendirmelerinde; DLQI genel puanı (17+yaş) 21,95±6,15; CDLQI genel puanı (6-17 yaş) 15,63±5,47; IDLQI genel puanı (0-5 yaş) 9,54±3,79 olarak hesaplanmıştır. Hastalık yaşam kalitesi üzerinde orta ve büyük etki yaratmaktadır. Semptomlar, duygusal yaşam, kognitif işlevler, okul ve tatiller en çok etkilenen yaşam kalitesi alt boyutları olmuştur. Araştırmada yaşanılan yer, ulaşım tipi, sağlık kuruluşuna olan uzaklık, kür tedavisi sayısı, hasta grubu, lezyon sayısı, lezyon bölge sayısı, zamanında gidilmeyen tedavi durumu değişkenleri ile hastalık maliyeti perspektifleri arasında ve aylık hane geliri, ulaşım tipi, gidilen kurum sayısı, kür tedavisi sayısı, yaşanılan yer değişkenleri ile yaşam kalitesi puanları arasında anlamlı ilişki bulunmuştur. Ayrıca hastalık maliyeti arttıkça yetişkinlerde yaşam kalitesinin düştüğü, bebek hasta grubunda ise yaşam kalitesinin yükseldiği sonucuna ulaşılmıştır. Hastaların özellikle üretken günlerini kaybetmesi, gelir kaybı yaşaması, ulaşım maliyetlerine katlanması, belirti, duygu ve kognitif boyutlarda yaşam kalitesinin düşmesi, iş ve okul devamsızlığı, hizmet sunucusunun katlandığı maliyetlerin önemli bir bölümünün personel giderlerinden oluşması önem arz etmektedir. Bu kapsamda şark çıbanı hastalığına ilişkin erken tanı çalışmaları, kırsal bölgelere yönelik önceliklendirme, birinci basamak sağlık hizmetlerinin güçlendirilmesi, ekonomik destek paketlerinin oluşturulması, hastalığın takibi ve bildirilmesine yönelik bilinçlendirme çalışmaları, lezyon sayılarına ve bölgelerine yönelik tedavi protokollerinin geliştirilmesi, hekim istihdamı, koruyucu ekipmanların dağıtımı vb. çalışmalar önerilmektedir.

Cutaneous leishmaniasis is a vector-borne infectious disease that, when it heals spontaneously, leaves scars on various parts of the body and is classified as one of the neglected tropical diseases by the World Health Organization. This disease, which is considered endemic in ninety-two countries, has an annual case number ranging from 700,000 to 1,200,000. Prevalent especially in the southeastern regions of Turkey, the disease is most widespread in Şanlıurfa. The disease affects quality of life and imposes a significant economic burden on individuals, families, the healthcare system, and society. Examining the effects of cutaneous leishmaniasis on patients' quality of life and calculating the cost of the disease are important for policies and practices to be developed in this field. The aim of this study is to calculate the cost of cutaneous leishmaniasis from the perspectives of patients and their relatives, service providers, and society; to measure the quality of life of the patients; and to examine the relationship between cost and quality of life. The research was conducted prospectively on 136 patients diagnosed with cutaneous leishmaniasis at the Cutaneous Leishmaniasis Diagnosis and Treatment Center in Şanlıurfa during the January–September 2024 period. The patients were divided into three groups: infants (0–5 years), children (6–17 years), and adults (17+); and the cost of illness was calculated from the perspectives of patients and their relatives, service providers, and society. Patients' quality of life was assessed using the Dermatology Life Quality Index, Children's Dermatology Life Quality Index, and Infants' Dermatology Life Quality Index. Within the scope of the study, total cost of illness was calculated as 297.037 TL (9.196,19 $), average cost of illness as 2.184,10 TL (67,62 $) from the perspective of the patient and the patient's relative; total cost of illness as 257.440,35 TL (8.717,80 $), average cost of illness as 1.893,40 TL (58,62 $) and total cost of illness as 554.477,35 TL (17.166,48 $), average cost of illness as 4.077,04 TL (126,22 $) from the perspective of the societal. Expenses incurred from the perspective of the patient and the patient's relative in the study were higher than the expenses incurred from the perspective of the service provider. In the quality of life assessments in the study; DLQI general score (17+ years) 21,95±6,15; CDLQI general score (6-17 years) 15,63±5,47; The overall IDLQI score (0-5 years) was calculated as 9,54±3,79. The disease has a moderate to significant impact on quality of life. Symptoms, emotional life, cognitive functions, school, and holidays were the most affected quality of life subscales. In the study, significant relationships were found between the perspectives of disease cost and the variables of place of residence, type of transportation, distance to the health facility, number of course treatments, patient group, number of lesions, number of lesion regions, and the situation of not receiving timely treatment; and between the quality of life scores and the variables of monthly household income, type of transportation, number of institutions visited, number of course treatments, and place of residence. Moreover, it was concluded that as the cost of illness increased, the quality of life decreased in adults, while the quality of life improved in the infant patient group. It is noteworthy that especially the loss of productive days, income loss, bearing transportation costs, decrease in quality of life in symptom, emotional and cognitive dimensions, absenteeism from work and school, and the fact that a significant portion of the costs incurred by the service provider consisted of personnel expenses was observed in patients. In this context, early diagnosis studies related to cutaneous leishmaniasis, prioritization for rural areas, strengthening of primary healthcare services, creation of economic support packages, awareness-raising activities for disease monitoring and notification, development of treatment protocols according to the number and regions of lesions, employment of physicians, and distribution of protective equipment are recommended.
