Özet
Amaç: All-on-4 konseptinde alt ve üst çeneye hazırlanan titanyum bar destekli monolitik zirkonya restorasyonların farklı oklüzyon türlerinde kortikal kemik, trabeküler kemik, implant, abutment ve altyapıda oluşturduğu stres değerlerinin sonlu elemanlarstres analizi ile incelenmesi amaçlandı. Materyal ve Metot: Bu çalışmada, dişsiz alt ve üst çenede all-on-4 konseptinde titanyum altyapılı ve Cr-Co altyapılı restorasyonlara ait modeller oluşturuldu. Bilateral balanslı, grup fonksiyonu ve kanin koruyuculu oklüzyon türü için sentrik kapanış, lateral hareket ve protrüziv hareket sırasında dişlerde oluşacak temas noktalarına maksimum ısırma kuvvetlerini temsil eden kuvvetler uygulandı. Kemik, implant, abutment ve altyapıda oluşan stresler iki protez türü arasında karşılaştırmalı olarak değerlendirildi. Bulgular: Altyapı materyalleri karşılaştırıldığında titanyum altyapılı restorasyon, oklüzyon türleri arasında ise kanin koruyuculu oklüzyon türü en düşük stres değerlerini gösterdi. Stres değerlerinin hiçbir modelde kortikal kemik ve trabeküler kemiğin dayanma sınırını ve implant, abutment ve altyapı materyalinin yorulma direncini aşmadığı gözlendi. İki restorasyon türüne ait stres değerlerinde en büyük fark altyapılar arasında gözlendi. Sonuç: Bu çalışmanın sınırları dahilinde, kemik, implant, abutment ve altyapıda oluşan stresler göz önünde bulundurulduğunda titanyum bar destekli monolitik zirkonya restorasyonlar tercih edilebilir. Bu restorasyon türünde ise kanin koruyuculu oklüzyon kullanılması önerilebilir. En uygun protez ve oklüzyon türünün uzun dönemli klinik çalışmalarla araştırılması gerekmektedir.

Aim: This study aimed to investigate the stress distribution in cortical bone, trabecular bone, implants, and the substructure caused by titanium bar-supported monolithic zirconia restorations prepared for the maxillary and mandibular arches within the All-on-4 concept under different occlusal schemes, using finite element analysis. Material and method: In this study, models of titanium-based and Co-Cr-based restorations were created within the All-on-4 concept for edentulous maxillary and mandibular arches. For bilateral balanced, group function, and canine-guided occlusal schemes, forces representing maximum bite loads were applied to the contact points occurring during centric occlusion, lateral movements, and protrusive movements. The resulting stress distributions in the bone, implants, and substructure were comparatively evaluated between the two types of prostheses. Results: When comparing the substructure materials, the titanium-supported restoration exhibited the lowest stress values. Among the occlusal schemes, the canine-guided occlusion demonstrated the most favorable stress distribution. In all models, the stress values did not exceed the strength limits of cortical and trabecular bone, nor the fatigue resistance thresholds of the implant and substructure materials. The most significant difference in stress values between the two types of restorations was observed in the substructures. Conclusion: Within the limitations of this study, considering the stress distribution in the bone, implant, and substructure, titanium bar-supported monolithic zirconia restorations may be considered a preferable option. Among the occlusal schemes, canine-guided occlusion appears to be the most favorable for this type of restoration. However, the most appropriate prosthesis and occlusal design should be further investigated through long-term clinical studies.
