Özet
Giriş: Postoperatif ağrı ve komplikasyonlar, meme kanseri cerrahisinde önemli bir sorun olmaya devam etmektedir. Rejyonal anestezi teknikleri, ameliyat sonrası ağrı ve komplikasyonlar üzerindeki olumlu etkilerinin anlaşılması sonrasında giderek daha fazla tercih edilmeye başlanmıştır. Özellikle interfasyal plan blokları başta olmak üzere çok sayıda teknik tanımlanmış olsa da etkinlik ve güvenilirlik açısından ideal tekniğin arayışı devam etmektedir. Pektoral sinir bloğu II (PECSII) rutin pratiğimizde tercih edilen, etkinlik ve güvenlik profili iyi bilinen bir tekniktir. Son zamanlarda, ikinci ve üçüncü kostal seviyelerde serratus posterior superior ve interkostal kaslar arasındaki düzlemi hedefleyen serratus posterior süperior interkostal plan (SPSIP) bloğu tanımlanmıştır. Bu çalışmanın amacı, randomize kontrollü bir çalışma tasarımında meme kanseri cerrahisi geçiren hastalarda PECSII ve SPSIP bloklarının perioperatif sonuçlarını karşılaştırmaktır. Gereç ve Yöntem: Bu randomize kontrollü çalışma Antalya Eğitim Araştırma Hastanesi'nde planlandı, etik kurul onayı (Karar No:1340 Tarih:26.12.2024) alındı. Çalışma ayrıca https://clinicaltrials.gov (NCT07001657) platformuna kaydedildi. Çalışmaya 18-65 yaş arası, ASA I-III, meme kanseri cerrahisi geçirecek hastalar dahil edildi. Ön çalışma sonucunda 0.90 güç ile çalışmaya dahil edilmesi gereken minimum hasta sayısı 94 olarak hesaplandı, 115 hasta çalışmaya dahil edildi. Hastalar randomize edilerek (https://www.randomizer.org); PECS grubu (n:57) ve SPSIP grubu (n:58) olarak ayrıldı. Her iki gruba da operasyondan 30 dk önce blok uygulaması yapıldı. Hastalara 30 ml %0,25 bupivakain uygulandı. Cerrahi işlem bitmeden 30 dk önce 15mg/kg parasetamol, 1mg/kg tramodol ve 0,15mg/kg ondansetron yapıldı. Tüm hastalara standart hasta kontrollü analjezi cihazı takıldı. Toplam opioid miktarı kaydedildi. Birincil sonlanımımız postoperatif 24 saatlik opioid tüketimi, ikincil sonlanımımız postoperatif VAS skorları olarak analiz edildi. Hastaların anestezi ve cerrahi süresi, blok uygulama süresi, iğne yönlendirme sayısı, intraoperatif ve postoperatif analjezik kullanım miktarı, ilk analjezik talep zamanı, intraoperatif xii hemodinamik veriler, postoperatif statik ve dinamik VAS skoru (1,2,6,12,24 saat), Quality of Recovery-15(QoR-15), bulantı ve kusma skorları, hasta ve cerrah memnuniyeti, hastanede kalış süresi ve blok ile ilişkili komplikasyonlar kayıt altına alındı. Bulgular: Hastaların ortalama yaşı 52,1±10,0 idi. Olguların %59,1'inin komorbid hastalığı vardı (hipertansiyon %42,6, diyabet %22,6, hipertiroidizm %11,3 vb.). PECSII ve SPSIP grupları, komorbiditeler, ASA skoru, ameliyat türü ve aksiller diseksiyon varlığı açısından benzerdi. Postoperatif ve intraoperatif opioid tüketimi gruplar arasında benzer düzeydeydi. Benzer şekilde, postoperatif VAS skorları, ilk analjezik talebine kadar geçen süre, hastanede kalış süresi, bulantı ve kusma, hasta ve cerrah memnuniyeti ve QoR-15 skorları gruplar arasında farklılık göstermemekteydi. Ancak, PECSII grubu ile karşılaştırıldığında, SPSIP grubunda blok uygulama süresi daha kısa (<0,0001), iğne yönlendirme sayısı daha azdı (p<0,001). Tartışma ve Sonuç: SPSIP bloğunun uygulama alanının operasyon bölgesinden uzak olması, işlem süresinin kısa, iğne yönlendirme sayısının az ve BMI yüksek hastalarda kolay uygulanabilir olması blok uygulama avantajı olarak görülebilir. SPSIP bloğu meme kanseri cerrahisi sonrasında PECSII bloğa benzer analjezik etkinlik ve uygulama kolaylığı ile alternatif bir blok olarak kullanılabilir.

Introduction: Postoperative pain and complications remain a significant concern in breast cancer surgery. Following the recognition of the beneficial effects of regional anesthesia techniques on postoperative pain and complications, their use has become increasingly widespread. Although numerous techniques—particularly interfascial plane blocks—have been described, the search for an ideal approach in terms of efficacy and reliability continues. The Pectoral Nerve Block Type II (PECSII) is a well-established technique in our routine practice, known for its favorable efficacy and safety profile. Recently, the serratus posterior superior intercostal plane (SPSIP) block has been described, targeting the plane between the serratus posterior superior and intercostal muscles at the second and third costal levels. The aim of this study was to compare the perioperative outcomes of PECSII and SPSIP blocks in patients undergoing breast cancer surgery within a randomized controlled trial design. Material and Methods: This randomized controlled study was conducted at Antalya Training and Research Hospital following approval from the ethics committee (Decision No: 1340, Date: December 26, 2024). The study was also registered on the https://clinicaltrials.gov platform (NCT07001657). Patients aged 18 to 65 years, classified as ASA physical status I–III, and scheduled for breast cancer surgery under general anesthesia were included. A preliminary power analysis determined that a minimum of 94 patients was required to achieve a power of 0,90; however, 115 patients were enrolled. Participants were randomly allocated using a computer-generated sequence into two groups: the PECS group (n = 57) and the SPSIP group (n = 58). Both groups received the respective regional block 30 minutes before surgery with 30 mL of 0,25% bupivacaine administered under ultrasound guidance. Additionally, 15mg/kg paracetamol, 1 mg/kg tramadol, and 0,15mg/kg ondansetron were administered 30 minutes before the end of surgery. All patients received standard patient-controlled analgesia, and total opioid consumption was recorded. The primary outcome was postoperative 24-hour opioid consumption, while secondary outcomes xiv included postoperative static and dynamic visual analogue scale (VAS) scores. Other recorded parameters were anesthesia and surgical duration, block performance time, number of needle passes, intraoperative and postoperative analgesic requirements, time to first analgesic request, intraoperative hemodynamic variables, postoperative VAS scores at 1, 2, 6, 12, and 24 hours, Quality of Recovery-15 (QoR-15) scores, incidence of nausea and vomiting, patient and surgeon satisfaction, length of hospital stay, and block-related complications. All data were collected and analyzed prospectively. Results: The average age of the patients was 52,1 ±10,0. 59,1% had comorbid conditions (Hypertension 42,6%, diabetes 22,6%, hyperthyroidism 11,3%). The PECSII and SPSIP groups were similar in terms of comorbidities, ASA score, type of surgery, and presence of axillary dissection. The SPSIP group demonstrated comparable postoperative and intraoperative opioid consumption to the PECSII group. Similarly, postoperative VAS scores, time to first analgesic request, length of hospital stay, nausea and vomiting, patient and surgeon satisfaction and QoR-15 scores were comparable between groups. However, compared to the PECSII group, the SPSIP group had a shorter block performance time (<0,001) and fewer number of needle passes (p<0,001). Discussion and Conclusion: The of the SPSIP block is distant from the surgical site, along with its shorter performance time, fewer needle passes, and better applicability in patients with higher BMI, can be considered advantages in terms of block performance. The SPSIP block may serve as an alternative to the PECSII block after breast cancer surgery, offering comparable analgesic efficacy and ease of application.
