Özet
Giriş- Amaç: Meme kanseri cerrahisinde postoperatif multimodal analjezi yönetimi önemli bir araştırma konusu olup son yıllarda periferik bloklar ön plana çıkmaktadır. Çalışmamızda preemptif uygulanan PECS-II (Modifiye Pektoral Sinir) ve ESP (Erektör Spina Plan) bloklarının postoperatif ağrı yönetiminde birbirlerine üstünlüğü olup olmadığını; opioidsiz bir analjezi stratejisi oluşturarak araştırmayı amaçladık. Gereç-Yöntem: Çalışma etik kurul onayı sonrası; prospektif, gözlemsel, tek merkezli olarak Haydarpaşa Numune Eğitim ve Araştırma Hastanesinde gerçekleştirildi. Unilateral elektif meme cerrahisi planlanan, 18-80 yaş aralığında, ASA (American Society of Anesthesiologists) riski I-III olan 70 hasta dahil edildi. Dışlamalar ve takipten çıkanlar sonrasında 62 hasta analiz edildi. Bloklarının her ikisi; preoperatif standart ASA monitörizasyonu altında, steril şartlarda, ultrasonografi lineer prob ile 22 gauge periferik blok iğnesi kullanılarak, in-plane teknikle ve hidrodiseksiyon doğrulamasıyla uygulandı. ESPB grubunda(n=31) yüzüstü pozisyonda T3 seviyesinden 30 ml %0,25 bupivakain enjekte edildi. PECS-II grubunda(n=31) sırtüstü pozisyonda önce derindeki interfasyal düzleme 20 ml, sonrasında yüzeyel düzleme 10 ml %0.25 bupivakain enjeksiyonu uygulandı. Temel sonuç değişkeni postoperatif ilk 24 saatteki toplam parasetamol tüketimiydi. İkincil sonuç değişkenleri postoperatif ağrı için NRS (Numeric Rating Scale) skorları, ilk analjezik talebi zamanı, ek analjezik ihtiyacı dozları ve zamanları, postoperatif hemodinamik verileri, bulantı kusma durumu ve hasta memnuniyetiydi. İstatistiksel analizler için“IBM SPSS Statistics 22.0”programı kullanıldı ve p<0,05 altındaki değerler istatistiksel olarak anlamlı kabul edildi. Bulgular: İki grup arasında postoperatif 24 saatteki toplam parasetamol dozları ve tüm saatlerde NRS skorları arasında anlamlı fark bulunmamıştır. Postoperatif ilk analjezik talebi için geçen zaman ortalaması ESPB grubunda 175 dk, PECS-II grubunda 435 dk kaydedildi ve istatistiksel olarak anlamlı fark bulunmuştur (p=0.015). PECS-II grubunda kurtarıcı analjezik olarak sadece parasetamol yeterli gelirken, ESPB grubunda 3 hastanın tramadol ihtiyacı kaydedildi. Postoperatif hemodinamik parametrelerde ve hasta memnuniyetinde gruplar arasında anlamlı fark bulunmamıştır. Hastaların hiçbirinde komplikasyon gelişmedi. Bulantı, kusma ve diğer yan etkiler açısından gruplar arasında istatistiksel olarak anlamlı fark saptanmamıştır. Sonuç: Onkolojik meme cerrahisi hastalarında ultrason kılavuzluğunda preemptif uygulanan PECS-II ve ESP bloklarının postoperatif analjezik etkinlikleri arasında anlamlı fark gözlenmemiştir. Daha uzun etkisi süresi PECS-II blok için tercih sebebi olabilir. Her iki blok uygulaması multimodal analjezinin bir parçası olarak uygulanabilecek etkin ve güvenli tekniklerdir.

Introduction – Aim: The optimization of postoperative multimodal analgesia in breast cancer surgery remains a critical area of investigation, with an increasing focus on peripheral nerve blocks in recent years. This study aimed to evaluate the comparative efficacy of preemptively administered PECS-II (Modified Pectoral Nerve) and ESP (Erector Spinae Plane) blocks in postoperative pain management and to explore the feasibility of an opioid-free analgesic strategy. Materials And Methods: Following ethical committee approval, this prospective, observational, single-center study was conducted at Haydarpaşa Numune Training and Research Hospital. A total of 70 patients, aged 18–80 years, classified as ASA (American Society of Anesthesiologists) physical status I–III, and scheduled for unilateral elective breast surgery, were enrolled. After exclusions and loss to follow-up, 62 patients were included in the final analysis. Both regional blocks were performed preoperatively under standard ASA monitoring, adhering to sterile conditions, utilizing a linear ultrasound probe, a 22-gauge peripheral nerve block needle, the in-plane technique, and hydrodissection for confirmation. In the ESPB group (n=31), 30 ml of 0.25% bupivacaine was administered at the T3 level in the prone position. In the PECS-II group (n=31), 20 ml was injected into the deep interfascial plane, followed by 10 ml into the superficial plane of 0.25% bupivacaine in the supine position. The primary outcome measure was the total paracetamol consumption within the first 24 hours postoperatively. Secondary outcomes included Numeric Rating Scale (NRS) pain scores, time to first analgesic request, the requirement for rescue analgesia, postoperative hemodynamic parameters, incidence of postoperative nausea and vomiting (PONV), and overall patient satisfaction. Statistical analyses were conducted using IBM SPSS Statistics 22.0, with a significance threshold set at p<0.05. Results: No statistically significant difference was observed between the two groups in terms of total paracetamol consumption within the first 24 hours postoperatively or NRS pain scores at all time points. However, the mean time to first analgesic request was significantly longer in the PECS-II group (435 minutes) compared to the ESPB group (175 minutes) (p=0.015). While paracetamol alone was sufficient as rescue analgesia in the PECS-II group, three patients in the ESPB group required tramadol. No significant intergroup differences were found regarding postoperative hemodynamic stability or patient satisfaction. No complications were reported in any of the patients. Furthermore, no statistically significant differences were observed between the groups in terms of PONV or other adverse effects. Conclusion: Preemptively administered ultrasound-guided PECS-II and ESP blocks demonstrated comparable efficacy in postoperative analgesia following oncologic breast surgery. The prolonged duration of analgesia observed with the PECS-II block may render it a preferable option in certain clinical contexts. Both techniques represent effective and safe components of a multimodal analgesic strategy.
