Özet
Giriş: Beklenen yaşam süresinin uzaması, hastaların giderek artan beklentileri, ve genç yaşta daha fazla uygulanan total kalça protezleri (TKA) sonucu revizyon total kalça artroplastisi (rTKA) ihtiyacı da buna paralel olarak artmaktadır. Bu çalışmada kliniğimizde, septik ve aseptik sebeplerle rTKA uygulanan hastalardaki revizyon nedenlerinin ve uygulanan cerrahi tedavilerin sonuçları geriye dönük olarak değerlendirildi. Gereç ve Yöntem: Kliniğimizde 2014–2025 yılları arasında rTKA uygulanan 50 hastanın 51 kalçası geriye dönük olarak değerlendirildi. Erken dönemde vefat eden 3 hasta klinik değerlendirmeye dâhil edilmedi. Hastaların demografik verileri, primer TKA endikasyonları, revizyon nedenleri, uygulanan cerrahi yöntemler ve kullanılan implantlar kaydedildi. Klinik sonuçlar HHS, OHS, VAS ağrı skoru ve SF-36 ile değerlendirildi. Radyolojik değerlendirme pelvis AP ve ortoröntgenogram görüntüleri üzerinden yapıldı. Elde edilen veriler istatistiksel olarak analiz edildi ve p<0.05 değeri anlamlı kabul edildi. Bulgular: Hastaların ort. yaşı 71,6 13,1 yıl ve VKİ'si ort. 26,1 ± 5,48 kg/m² (min–maks: 18,3–39,5) kg/m² idi. Hastaların 19'u (%37,3) erkek, 32'si (%62,7) kadındı. Revizyon nedenleri arasında en sık olarak septik gevşeme (%31,4), dislokasyon (%27,5) ve aseptik gevşeme (%21,6) saptandı. Travma grubunda dislokasyon ve periprostetik kırık daha sık, travma dışı grupta ise aseptik gevşeme anlamlı düzeyde daha yüksek bulundu (p<0.05). HHS 30,35'ten 84,12'ye (p<0,001), OHS 14,29'dan 40,97'ye (p<0,001) yükseldi; VAS' 7,77'den 1,89'a geriledi (p<0.001). SF-36'nın tüm alt parametrelerinde anlamlı iyileşme saptandı. Septik ve aseptik revizyonlar karşılaştırıldığında klinik ve radyolojik sonuçlar benzer olmakla birlikte yalnızca sosyal fonksiyon skorunda septik grupta daha düşük değer saptandı (p=0.032). Rotasyon merkezi kaybı tüm olgularda 0 mm olup herhangi bir değişim saptanmadı. BUE ameliyat öncesi 10,25 mm'den ameliyat sonrası 3,90 mm'ye geriledi (p<0,001). Ort. 42 aylık (min – maks: 6- 140 ay) takipte femoral komponent çökmesinin ort. 1,03 mm (min–maks: 0–15) olduğu belirlendi. Sonuç: rTKA uygulanan hastalarda klinik ve yaşam kalitesi göstergelerinde anlamlı iyileşme elde edildiği gösterilmiştir. HHS, OHS, VAS ve SF-36 skorlarındaki belirgin düzelme, farklı endikasyonlarla uygulanan revizyon cerrahisinin fonksiyonel kazanım açısından etkili bir tedavi seçeneği olduğunu desteklemektedir. Uygun endikasyon, dikkatli preoperatif hazırlık ve doğru cerrahi strateji ile uygulanan rTKA, komplikasyon riskine rağmen tatmin edici klinik ve fonksiyonel sonuçlar sağlayabildiğinden rTKA uygulamalarının cesaretle yapılmasını önermekteyiz.

Introduction: The increase in life expectancy, rising patient expectations, and the more frequent use of total hip arthroplasty (THA) at a younger age have led to a parallel increase in the need for revision total hip arthroplasty (rTHA). In this study, the reasons for revision and the outcomes of the surgical treatments applied to patients who underwent rTHA for septic and aseptic causes in our clinic were retrospectively evaluated. Materials and Methods: A total of 51 hips of 50 patients who underwent revision total hip arthroplasty (rTHA) between 2014 and 2025 in our clinic were retrospectively evaluated. Three patients who died in the early postoperative period were excluded from the clinical evaluation. Patients' demographic data, primary THA indications, reasons for revision, surgical techniques, and the implants used were recorded. Clinical outcomes were assessed using the Harris Hip Score (HHS), Oxford Hip Score (OHS), Visual Analog Scale (VAS) for pain, and the SF-36 questionnaire. Radiological evaluation was performed using anteroposterior pelvic and orthoradiographic images. The obtained data were statistically analyzed, and a p value of <0.05 was considered statistically significant. Results: The mean age of the patients was 71.6 ± 13.1 years, and the mean body mass index (BMI) was 26.1 ± 5.48 kg/m² (range: 18.3–39.5 kg/m²). Nineteen patients (37.3%) were male and 32 (62.7%) were female. The most common reasons for revision were septic loosening (31.4%), dislocation (27.5%), and aseptic loosening (21.6%). Dislocation and periprosthetic fracture were more frequent in the trauma group, whereas aseptic loosening was significantly more common in the non-trauma group (p < 0.05). The Harris Hip Score (HHS) improved from 30.35 to 84.12 (p < 0.001), and the Oxford Hip Score (OHS) increased from 14.29 to 40.97 (p < 0.001). The Visual Analog Scale (VAS) pain score decreased from 7.77 to 1.89 (p < 0.001). Significant improvements were observed in all subdomains of the SF-36 questionnaire. When septic and aseptic revisions were compared, clinical and radiological outcomes were similar; however, only the social functioning score was significantly lower in the septic group (p = 0.032). The loss of the center of rotation was 0 mm in all cases, with no detected change. The bone–implant interface distance decreased from 10.25 mm preoperatively to 3.90 mm postoperatively (p < 0.001). At a mean follow-up of 42 months (range: 6–140 months), the mean femoral component subsidence was 1.03 mm (range: 0–15 mm). Conclusion: Significant improvements in clinical outcomes and quality-of-life measures were demonstrated in patients who underwent revision total hip arthroplasty (rTHA). The marked improvements in HHS, OHS, VAS, and SF-36 scores support revision surgery as an effective treatment option for functional recovery across different indications. With appropriate patient selection, careful preoperative planning, and the use of proper surgical strategies, rTHA can provide satisfactory clinical and functional outcomes despite the risk of complications; therefore, we recommend performing rTHA with confidence.
