Özet
Amaç: Üriner sistem taş hastalığının giderek artan insidansı ve yüksek nüks oranları, rezidü taşların yönetimini klinik açıdan önemli bir sorun haline getirmektedir. Rezidü fragmanların izlem, medikal tedavi, ESWL (Extracorporeal Shock Wave Lithotripsy) veya cerrahi ile yönetimine ilişkin standart bir protokol bulunmamaktadır. Bu çalışmanın amacı, sekiz bitkisel bileşen ekstresinden oluşan Tutukon® adlı bitkisel gıda takviyesinin, retrograd intrarenal cerrahi (RIRC) sonrası taşsızlık oranlarına ve postoperatif dönem klinik sonuçlarına olan etkisini araştırmaktır. Gereç ve Yöntem: Bu prospektif, tek kör, plasebo kontrollü çalışma, Ondokuz Mayıs Üniversitesi Tıp Fakültesi Üroloji Anabilim Dalı'nda böbrek taşı nedeniyle RIRC uygulanan 130 hastayı (65 çalışma grubu, 65 kontrol grubu) kapsamaktadır. Hastalar bilgilendirilerek onamları alınmış ve çalışma (bitkisel gıda desteği) ve kontrol (plasebo) gruplarına ayrılmıştır. Çalışma grubuna, litolitik, spazmolitik, antibakteriyel, antiinflamatuar, diüretik, antioksidan ve nefroprotektif özelliklere sahip, sekiz bitkisel ekstreden oluşan gıda desteği (Tutukon®) günlük 3x45 ml, toplam 1 ay süreyle verilmiş; kontrol grubuna ise sadece yardımcı maddeler içeren plasebo uygulanmıştır. Hastaların demografik, preoperatif, operatif ve postoperatif verileri (tam idrar tetkiki (pH, eritrosit, lökosit), beyaz küre (WBC) sayıları, serum kreatinin düzeyleri, spot idrarda kalsiyum, kreatinin, oksalat ve sitrat düzeyleri) kaydedildi. İkinci haftada Türkçe valide edilmiş Üreteral Stent Semptomları Sorgulama Formu (USSQ) ile stent ilişkili semptomları değerlendirildi; birinci ayda kontrastsız bilgisayarlı tomografi (BT) ile rezidü fragmanlar incelenmiş, stentsiz dönem semptomları değerlendirilmiştir. Birinci ay sonunda çalışma ve kontrol gruplarının taşsızlık oranları, postoperatif komplikasyonlar, rehospitalizasyon gereksinimi, böbrek fonksiyonları, WBC ve idrar parametrelerindeki farklar, stentli ve stentsiz dönem semptomları karşılaştırılmıştır. Bulgular: Gruplar arasında yaş, cinsiyet, vücut kitle indeksi, taş lokalizasyonu, preoperatif taş hacmi, Hounsfield ünitesi ve intraoperatif parametreler açısından anlamlı fark saptanmadı. Birinci ay kontrastsız BT değerlendirmesinde rezidü taş hacmi Tutukon grubunda daha düşük saptandı (medyan 10 vs 36 mm³, p=0,026). Taşsızlık durumu gruplar arasında anlamlı farklılık gösterdi (p<0,001); klinik anlamlı rezidü oranı Tutukon grubunda %27,7, plasebo grubunda %55,4 iken tam taşsızlık oranları sırasıyla %38,5 ve %13,9 olarak bulundu. Lojistik regresyon analizinde Tutukon® kullanımı taşsızlık için bağımsız bir belirleyici olarak saptandı (multivarient OR 5,888; %95 GA 2,388–14,518; p<0,001). Postoperatif değerlendirmede Tutukon grubunda lökosit değişiminde daha belirgin azalma (p=0,037) ve idrar sitrat düzeyinde artış (p=0,044) izlenirken kreatinin, idrar pH, eritrosit, lökosit, kalsiyum ve oksalat değişimleri açısından anlamlı fark saptanmadı. Hastanede yatış süresi ve komplikasyon oranları benzerdi. Semptom değerlendirmesinde Tutukon grubunda Stentli VAS skoru (p=0,043) ve ağrı skorları (p=0,023) daha düşük, stent çıkarıldıktan sonra ise işeme semptomları (p=0,002), ağrı (p=0,005) ve iş performansı(p=0,023) skorları anlamlı olarak daha iyi bulundu. Sonuç: Bu prospektif çalışmada, RIRC sonrası Tutukon kullanımının taşsızlık oranlarını artırdığı, klinik anlamlı rezidü fragman oranını azalttığı ve postoperatif semptom yükünü iyileştirdiği gösterilmiştir. İdrar sitrat düzeyinde artış ve inflamatuar yanıtın azalması ile uyumlu bulgular elde edilmiş olup böbrek fonksiyonları üzerinde olumsuz bir etki izlenmemiştir. Bu bulgular, Tutukon'un RIRC sonrası dönemde güvenli ve etkili bir destek tedavi seçeneği olabileceğini düşündürmekle birlikte, sonuçların doğrulanması için daha geniş örneklemli, uzun dönemli ve çok merkezli çalışmalara ihtiyaç vardır.

Objective: The increasing incidence and high recurrence rates of urinary system stone disease make the management of residual stones a clinically significant problem. There is no standard protocol for the management of residual fragments through observation, medical treatment, ESWL (Extracorporeal Shock Wave Lithotripsy), or surgery. The aim of this study is to investigate the effect of Tutukon®, an herbal food supplement composed of eight herbal component extracts, on stone-free rates and postoperative clinical outcomes after retrograde intrarenal surgery (RIRS). Material and Methods: This prospective, single-blind, placebo-controlled study included 130 patients (65 study group, 65 control group) undergoing RIRS for kidney stones at the Department of Urology, Ondokuz Mayıs University Faculty of Medicine. Patients were informed and consented to participate in the study (herbal food supplement) and control (placebo) groups. The study group received a food supplement consisting of eight herbal extracts with litholytic, spasmolytic, antibacterial, anti-inflammatory, diuretic, antioxidant, and nephroprotective properties (3x45 ml daily, for a total of 1 month); the control group received a placebo containing only excipients. Demographic, preoperative, operative, and postoperative data (complete urinalysis (pH, erythrocytes, leukocytes), white blood cell (WBC) counts, serum creatinine levels, spot urine calcium, creatinine, oxalate, and citrate levels) were recorded for each patient. In the second week, stent-related symptoms were assessed using the Turkish validated Ureteral Stent Symptoms Questionnaire (USSQ); at one month, residual fragments were examined with non-contrast enhanced computed tomography, and symptoms in the stent-free period were evaluated. At the end of the first month, stone-free rates, postoperative complications, need for rehospitalization, differences in renal function, WBC and urine parameters, and symptoms in the stent and stent-free periods were compared between the study and control groups. Results: The groups were similar in terms of age, sex, body mass index, stone localization, preoperative stone volume, Hounsfield unit, and intraoperative parameters. At the first month, the residual stone volume on non-contrast enhanced computed tomography (CT) evaluation was found to be lower in the Tutukon group (median 10 vs 36 mm³, p=0.026). Stone-free status differed significantly between the groups (p<0.001); the clinically significant residual stone rate was 27.7% in the Tutukon group and 55.4% in the placebo group, while the complete stone-free rates were 38.5% and 13.9%, respectively. In logistic regression analysis, Tutukon® use was identified as an independent predictor of stone-free status (multivariate OR 5.888; 95% CI 2.388–14.518; p<0.001). Postoperative evaluation showed a more pronounced decrease in leukocyte turnover (p=0.037) and an increase in urinary citrate levels (p=0.044) in the Tutukon group, while no significant differences were found in creatinine, urinary pH, erythrocytes, leukocytes, calcium, and oxalate changes. Hospital stay and complication rates were similar. In symptom assessment, the Tutukon group had lower VAS and pain scores during the stent period (p=0.043 and p=0.023), while after stent removal, voiding symptoms (p=0.002), pain (p=0.005), and work performance (p=0.023) scores were significantly better. Conclusion: In this prospective study, the use of Tutukon following RIRS was associated with higher stone-free rates, lower rates of clinically significant residual fragments, and improvement in postoperative symptom burden. An increase in urinary citrate levels and findings consistent with reduced inflammatory response were observed, without any adverse impact on renal function. These findings suggest that Tutukon may be a safe and effective adjunctive therapy in the postoperative management of patients undergoing RIRS; however, larger studies with longer follow-up and multicenter designs are needed to confirm these results.
