Özet

Opioid Kullanım Bozukluğu (OKB), bireylerin fiziksel ve psikolojik işlevlerini etkileyen, önemli ölçüde sosyal ve ekonomik sorunlara yol açan kronik bir rahatsızlıktır. Opioidlerin uzun süreli kullanımı, beyin kimyasında değişikliklere neden olarak toleransın ve yoksunluğun ortaya çıkmasına zemin hazırlar. Oksidatif stresin madde kullanım bozukluğunda önemli bir rol oynadığı ve yaşam tarzı ile genetik faktörlere bağlı olarak, serbest radikaller ve antioksidanlar arasındaki dengesizliğin bozulması sonucu meydana geldiği bilinmektedir. Bu süreçte meydana gelen serbest radikallerin temizlenmesinde süperoksit dismutaz enzimi (SOD) çok önemli bir rol üstlenir. SOD genlerinde meydana gelen polimorfizmler ise, enzimin yapısını, işlevini ve ekspresyon düzeylerini etkileyerek OKB'ye yatkınlığı değiştirebilir. Bu çalışmada SOD-1 50 bp Ins/Del ve SOD-2 rs5746136 polimorfizmlerinin OKB riski üzerindeki etkisi değerlendirilmiştir. Bu amaçla, Ankara Eğitim ve Araştırma Hastanesi- Alkol ve Madde Bağımlılığı Tedavi ve Eğitim Merkezi'ne (AMATEM) başvuran DSM-5 kriterlerine göre OKB tanısı konmuş (n=101) bireyler ve hayatları boyunca herhangi bir psikoaktif madde kullanmamış sağlıklı bireylerden (n=104) kan örnekleri toplanmıştır. Kan örneklerinden izole edilen DNA'lar, SOD-1 50 bp Ins/Del ve SOD-2 rs5746136 polimorfizmlerinin tanımlanması amacıyla PCR ve/veya PCR-RFLP yöntemleri ile genotiplendirilmiştir. Bireylerden Klinik Opioid Yoksunluk Ölçeği (KOYÖ), Madde Aşerme Ölçeği (MAÖ), Beck Anksiyete Envanteri (BAE) ve Beck Depresyon Envanteri-II (BDE-II) doldurmaları istenmiştir. Elde edilen tüm veriler istatiksel olarak analiz edilmiştir. SOD-1 50 bp Ins/Del polimorfizmi için Del alel frekansı OKB'li bireylerde ve sağlıklı bireylerde sırasıyla %8 ve %15 olarak, SOD-2 rs5746136 polimorfizmi için varyant alel (T) frekansı OKB'li bireylerde ve sağlıklı bireylerde sırasıyla %36 ve %44 olarak hesaplanmıştır. Her iki polimorfizmin de genotip frekanslarının Hardy-Weinberg dengesinde olduğu belirlenmiştir (p>0,05). Her iki polimorfizme ait genotip dağılımları incelendiğinde (SOD-1 için Del/Del+Ins/Del ile Ins/Ins; SOD-2 için ko-dominant (CC, CT, TT), T-dominant (CC ile CT+TT), C-dominant modellerde (TT ile CC+CT)), SOD-1 Del/Del+Ins/Del genotiplerinin OKB'li bireylerde frekansı sağlıklı bireylere kıyasla istatiksel olarak anlamlı düzeyde düşük bulunmuştur (p = 0.049). Ancak SOD-2 kalıtım modelleri arasında anlamlı bir fark bulunamamıştır (sırasıyla ko-dominant, T-dominant ve T-resesif modeler için p=0,261, p=0,490 ve p= p=0,099). OKB'li ve sağlıklı bireylerde genotip alt gruplarına göre ölçek puanlarının karşılaştırılması sonucunda BAE ve BDE-II değerlerinin eroin kullanan bireylerde (BAE: 20,0; BDE-II: 27,0) sağlıklı bireylere (BAE: 2,0; BDE-II: 4,0) kıyasla daha yüksek olduğu bulunmuştur (p=0,001.). Ek olarak, OKB tanısı konmuş bireylerde genotip alt gruplarına göre KOYÖ, MAÖ, BAE ve BDE-II ölçeklerinden elde edilen puanlar da analiz edilmiştir. Her iki polimorfizme ait genotip alt grupları arasında söz konusu ölçek puanları açısından anlamlı bir fark saptanmamıştır (p > 0,05). Sonuç olarak, bu tez çalışmasından elde edilen bulgular, Türk popülasyonunda SOD-1 50 bp Ins/Del polimorfizminin OKB ile ilişkili olabileceğini, Del alelinin sağlıklı bireylerde daha sık görülmesi nedeniyle koruyucu bir rol üstleneceğini göstermiştir. SOD-2 rs5746136 polimorfizmi ise genotip ve alel dağılımları açısından OKB ile anlamlı bir ilişki göstermemiştir. Her iki polimerfizme ait bu bulguların daha büyük örneklem grupları ve farklı etnik kökenler üzerinde yapılacak çalışmalarla desteklenmesi önerilmektedir.

Opioid Use Disorder (OCD) is a chronic disorder that affects individuals' physical and psychological functions and leads to significant social and economic problems. Long-term opioid use causes changes in brain chemistry, paving the way for tolerance and withdrawal. Oxidative stress is known to play a significant role in substance use disorders and results from an imbalance between free radicals and antioxidants, which can be caused by lifestyle and genetic factors. The superoxide dismutase enzyme (SOD) plays a crucial role in scavenging the free radicals produced in this process. Polymorphisms in the SOD genes may alter susceptibility to OCD by affecting the enzyme's structure, function, and expression levels. This study evaluated the impact of the SOD-1 50 bp Ins/Del and SOD-2 rs5746136 polymorphisms on OCD risk. For this purpose, blood samples were collected from individuals diagnosed with OCD according to DSM-5 criteria (n=101) and healthy individuals (n=104) who had not used any psychoactive substances in their lifetimes and who presented to Ankara Education and Research Hospital-Alcohol and Substance Addiction Treatment and Training Center (AMATEM). DNA isolated from the blood samples was genotyped using PCR and/or PCR-RFLP to identify the SOD-1 50 bp Ins/Del and SOD-2 rs5746136 polymorphisms. Subjects were asked to complete the Clinical Opioid Withdrawal Scale (COWS), Substance Craving Scale (SCS), Beck Anxiety Inventory (BAI), and Beck Depression Inventory-II (BDI-II). All data were statistically analyzed. The Del allele frequency for the SOD-1 50 bp Ins/Del polymorphism was calculated as 8% and 15% in individuals with OCD and healthy individuals, respectively, and the variant allele (T) frequency for the SOD-2 rs5746136 polymorphism was calculated as 36% and 44% in individuals with OCD and healthy individuals, respectively. The genotype frequencies of both polymorphisms were determined to be in Hardy-Weinberg equilibrium (p>0.05). When the genotype distributions of both polymorphisms were examined (Del/Del+Ins/Del and Ins/Ins for SOD-1; co-dominant (CC, CT, TT), T-dominant (CC with CT+TT), and C-dominant models (TT with CC+CT)), the frequency of the SOD-1 Del/Del+Ins/Del genotypes was found to be statistically significantly lower in individuals with OCD compared to healthy individuals (p = 0.049). However, no significant difference was found between the SOD-2 inheritance models (p=0.261, p=0.490 and p=0.099 for co-dominant, T-dominant and C-dominant models, respectively). As a result of the comparison of scale scores according to genotype subgroups in OCD and healthy individuals, it was found that BAI and BDI-II values were higher in heroin users (BAI: 20.0; BDI-II: 27.0) compared to healthy individuals (BAI: 2.0; BDI-II: 4.0) (p=0.001). In addition, the scores obtained from the COWS, MAS, BAI and BDI-II scales according to genotype subgroups in individuals diagnosed with OCD were also analyzed. No significant difference was found in terms of these scale scores between the genotype subgroups of both polymorphisms (p>0.05). In conclusion, the findings from this thesis suggest that the SOD-1 50 bp Ins/Del polymorphism may be associated with OCD in the Turkish population, and that the Del allele, being more common in healthy individuals, may play a protective role. The SOD-2 rs5746136 polymorphism, however, did not show a significant association with OCD in terms of genotype and allele distribution. It is recommended that these findings for both polymorphisms be supported by studies conducted with larger samples and diverse ethnic backgrounds.
