Özet

Amaç: Bu çalışmanın amacı, subklavyen ven kollabsibilite indeksinin (SCV-CI); spinal anestezi sonrası gelişen hipotansiyonu (SAIH) öngörmedeki rolünü araştırmaktır. Yöntem: Bu prospektif gözlemsel karşılaştırmalı çalışmaya, 01.03.2025-01.07.2025 tarihleri arasında Dicle Üniversitesi Tıp Fakültesi Hastanesi ameliyathanesinde, 18-65 yaş arası, American Society of Anesthesiologists (ASA) skoru I-II olan, elektif cerrahi geçiren toplam 90 hasta dahil edildi. Örneklem büyüklüğü G*Power ile hesaplandı (n=90). SAIH gelişen (n=51) ve gelişmeyen (n=39) iki grup oluşturuldu. SCV-CI ölçümleri ultrasonografi ile yapıldı. Tüm olgulara standart monitörizasyon uygulandı ve spinal anestezi sonrası 30 dakika boyunca hemodinamik parametreler (kalp atım hızı (KAH), sistolik kan basıncı (SKB), diyastolik kan basıncı (DKB), ortalama arter basıncı (OAB) ve oksijen satürasyonu (SpO₂) beşer dakikalık aralıklarla kaydedildi. Bulgular: Çalışmaya dahil edilen 90 hastanın yaş, kilo, boy, vücut kitle indeksi, cinsiyet dağılımı, ASA skoru ve operasyon türü açısından gruplar arasında anlamlı fark saptanmamıştır (p>0,05). Ek hastalık varlığı, SAIH gelişen grupta daha yüksek bulunmuştur (p=0,045). Hemodinamik parametreler incelendiğinde, SAIH gelişen grupta başlangıç SKB, DKB, OAB ve KAH ortalamaları daha yüksek bulunmuş, ancak SAIH sonrası SKB, DKB, OAB ve KAH zamanla belirgin şekilde azalmıştır. SAIH gelişmeyen grupta ise bu parametreler daha stabil seyretmiştir. SpO₂ her iki grupta da normal ve stabil kalmıştır. Subklavyen ven (SCV) maksimum çap değerleri gruplar arasında farklılık göstermemiştir, ancak SCV minimum çap ve SCV-CI SAIH gelişen grupta anlamlı olarak daha yüksek bulunmuştur (p<0,001). SAIH gelişen grubun %9,8'inde komplikasyon gözlenmiş olup, SAIH gelişmeyen grupta komplikasyon izlenmemiştir. Sonuç: SCV-CI, SAIH riskini öngörmede pratik ve güvenilir bir araç olarak değerlendirilebilir.

Aim: The aim of this study is to investigate the role of the subclavian vein collapsibility index (SCV-CI) in predicting post-spinal hypotension (PSH). Methods: This prospective observational comparative study included a total of 90 patients aged 18-65 years with an American Society of Anesthesiologists (ASA) score of I-II who underwent elective surgery in the operating room of Dicle University Faculty of Medicine Hospital between March 1, 2025, and July 1, 2025. The sample size was calculated using G*Power (n=90). Two groups were formed: those who developed PSH (n=51) and those who did not (n=39). SCV-CI measurements were performed using ultrasonography. Standard monitoring was applied to all cases, and hemodynamic parameters (heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP) and oxygen saturation (SpO₂) were recorded at five-minute intervals for 30 minutes after spinal anesthesia. Results: No significant differences were found between the groups in terms of age, weight, height, body mass index, gender distribution, ASA score, and type of surgery among the 90 patients included in the study (p>0.05). The presence of additional diseases was higher in the group that developed PSH (p=0.045). When hemodynamic parameters were examined, the mean initial SBP, DBP, MAP, and HR were higher in the group that developed PSH; however, SBP, DBP, MAP, and HR decreased significantly over time after PSH. In the group that did not develop PSH, these parameters remained more stable. SpO₂ remained normal and stable in both groups. Subclavian vein (SCV) maximum diameter values did not differ between groups, but SCV minimum diameter and SCV-CI were significantly higher in the group that developed PSH (p<0.001). Complications were observed in 9.8% of the group with PSH development, while no complications were observed in the group without PSH development. Conclusion: SCV-CI can be considered a practical and reliable tool for predicting PSH risk.
