Özet
Amaç: Bu çalışmanın temel amacı; şizofreni tanısı almış bireylerde dışavurumcu sanat terapisi temelli bir programın umut üzerine etkisini belirlemektir. Metaryal ve Metot: Tek kör randomize tipte olan bu araştırma 22 Temmuz 2025- 30 Ekim 2025 tarihleri arasında Samsun Ruh Sağlığı ve Hastalıkları Hastanesinde tedavi gören şizofreni tanılı bireylerden, dahil edilme kriterlerine uyan ve gönüllü olan 50 hastadan elde edilmiştir. Araştırmada basit randomizasyon ile örneklem grubu deney (n=25) ve kontrol (n=25) gruplarına atanmıştır. Gerekli izinler ve onamlar alındıktan sonra katımcıların ön testleri yapılmış, ardından deney grubunu oluşturan bireylere dışavurumcu sanat terapi programı uygulanmıştır. Araştırmanın verileri; Sosyodemografik Bilgi Formu ve Herth Umut Ölçeği ile toplanmıştır. Verilerin istatistiksel analizinde tanımlayıcı istatistikler; kikare, Fisher Freeman-Halton testi, Fisher's Exact testi, bağımsız gruplar t testi, iki faktörlü ANOVA Sphericity Assumed testi kullanılmıştır. Araştırmada gruplar arasındaki anlamlı çıkan farkın büyüklüğü eta kare (𝜂2) ile verilmiştir. Bulgular: Müdahale öncesinde deney ve kontrol grubundaki bireylerin Herth Umut Ölçeği toplam ve alt boyut puan ortalamaları arasında istatistiksel olarak anlamlı bir fark saptanmamıştır (p>0.05). Müdahele sonrası ölçümlerde deney gruplarındaki bireylerin Herth Umut Ölçeği toplam ve alt boyut puan ortalamalarının kontrol grubundaki bireylere kıyasla anlamlı düzeyde yüksek olduğu belirlenmiştir (p<0.001). Dışavurumcu sanat terapesi temelli program şizofreni tanılı bireylerin Herth Umut Ölçeği toplam ve Gelecek , Olumlu Hazıroluşluk ve Beklenti , Kendisi ve Çevresindekilerle Arasındaki Bağlar alt boyut puanlarının anlamlı ve geniş bir etki düzeyine sahip olduğu belirlenmiştir (p<0.001). Sonuç: Dışavurumcu sanat terapinin şizofreni tanılı bireylerde umut düzeyini artırmada etkili olduğu, müdahele olarak kullanılabileceği ve psikiyatri hemşireliği uygulamalarına entegre edilebileceği sonucuna ulaşılmıştır.

Objective: The primary objective of this study is to determine the effect of an expressive arts therapy-based programme on hope in individuals diagnosed with schizophrenia. Materials and Methods: This single-blind randomised study was conducted between 22 July 2025 and 30 October 2025 at the Samsun Mental Health and Diseases Hospital with 50 patients diagnosed with schizophrenia who were undergoing treatment, met the inclusion criteria, and volunteered to participate. In the study, the sample group was assigned to an experimental (n=25) and control (n=25) group using simple randomisation. After obtaining the necessary permissions and consent, the participants were pre-tested, and then the expressive arts therapy programme was applied to the individuals forming the experimental group. The data for the study were collected using the Sociodemographic Information Form and the Herth Hope Scale. Descriptive statistics, chi-square, Fisher-Freeman-Halton test, Fisher's Exact test, independent groups t-test, and two-factor ANOVA Sphericity Assumed test were used in the statistical analysis of the data. The magnitude of the significant difference between the groups in the study was given by eta square (𝜂2). Findings: No statistically significant difference was found between the mean total and subscale scores of the Herth Hope Scale in the experimental and control groups before the intervention (p>0.05). Post-intervention measurements revealed that the mean total and subscale scores of the Herth Hope Scale for individuals in the experimental groups were significantly higher than those for individuals in the control group (p<0.001). The expressive arts therapy-based programme was found to have a significant and broad effect on the total Herth Hope Scale scores and the Future, Positive Readiness and Expectation, and Connections with Self and Others subscale scores of individuals diagnosed with schizophrenia (p<0.001). Conclusion: It was concluded that expressive art therapy is effective in increasing hope levels in individuals diagnosed with schizophrenia, can be used as an intervention, and can be integrated into psychiatric nursing practices.
