Özet

Bu çalışmada elektif açık inguinal herni onarımı yapılan geriatrik hastalarda, preemptif intravenöz (IV) ibuprofenin postoperatif erken bilişsel değişim, gecikmiş nörobilişsel iyileşme (dNCR), ağrı şiddeti ve iyileşme kalitesi üzerindeki etkilerinin değerlendirilmesi amaçlandı. Prospektif, randomize, çift kör çalışmaya 65-84 yaş aralığında 76 erkek hasta dahil edildi. İndüksiyondan 30 dakika önce Grup 1'e (n=38) 800 mg (200 mL) preemptif IV ibuprofen, Grup 2'ye (n=38) plasebo (200 mL IV %0,9 NaCl) uygulandı. Tüm hastalarda standart analjezi protokolü uygulandı. Postoperatif VAS skoru ≥4 olan hastalara 1 mg/kg ek IV tramadol verildi. Bilişsel fonksiyonlar Montreal Bilişsel Değerlendirme Ölçeği (MoCA) ile değerlendirildi; 1. günde ≥2 puanlık düşüş postoperatif erken bilişsel değişim, 7. günde ≥2 puanlık düşüş ise dNCR olarak kabul edildi. Postoperatif 1. günde erken bilişsel değişim insidansı Grup 2'de %36,8 (n=14) iken, Grup 1'de %0 (n=0) olarak saptandı (p<0,001). Postoperatif 7. günde her iki gruptaki hiçbir hastada dNCR saptanmadı (p=1,000). Grup 1'in 24 saatlik toplam tramadol tüketimi (147,37±50,60 mg vs 234,21±53,41 mg) ve VAS ağrı skorları anlamlı derecede düşüktü (p<0,001). QoR-15 skorları Grup 1'de daha yüksek bulundu (p<0,001). Çalışmamız, geriatrik hastalarda preemptif IV ibuprofen uygulamasının, dNCR'nin öncülü kabul edilen postoperatif erken bilişsel değişimi anlamlı düzeyde azalttığını göstermektedir. Bu bulgular; preemptif IV ibuprofenin multimodal analjezi stratejileri içinde nörobilişsel korunma ve opioid tasarrufuna katkı sağlayan bir ajan olarak değerlendirilebileceğini göstermektedir.

This study aimed to evaluate the effects of preemptive intravenous (IV) ibuprofen on postoperative early cognitive change, delayed neurocognitive recovery (dNCR), pain severity, and quality of recovery in geriatric patients undergoing elective open inguinal hernia repair. Seventy-six male patients aged 65–84 years were included in this prospective, randomized, double-blind study. Thirty minutes before induction, Group 1 (n=38) received 800 mg (200 mL) preemptive IV ibuprofen, whereas Group 2 (n=38) received placebo (200 mL IV 0.9% NaCl). A standard analgesia protocol was applied to all patients. For patients with a postoperative VAS score ≥4, 1 mg/kg supplemental IV tramadol was administered. Cognitive functions were assessed using the Montreal Cognitive Assessment (MoCA) scale; a decrease of ≥2 points on the 1st postoperative day was defined as early cognitive change, and a decrease of ≥2 points on the 7th postoperative day was defined as dNCR. The incidence of early cognitive change on the 1st postoperative day was 36.8% (n=14) in Group 2, whereas it was 0% (n=0) in Group 1 (p<0.001). On the 7th postoperative day, no patient in either group developed dNCR (p=1.000). Group 1 exhibited significantly lower 24-hour total tramadol consumption (147.37±50.60 mg vs. 234.21±53.41 mg) and VAS pain scores (p<0.001). QoR-15 scores were significantly higher in Group 1 (p<0.001). Our study demonstrates that preemptive IV ibuprofen administration significantly reduces postoperative early cognitive change, which is considered a precursor to dNCR, in geriatric patients. These findings suggest that preemptive IV ibuprofen can be considered as an agent that contributes to neurocognitive protection and opioid-sparing effects within multimodal analgesia strategies.
