Özet
Amaç: Redüksiyon mamoplasti operasyonları geniş doku diseksiyonu ve bilateral uygulanmaları nedeniyle belirgin postoperatif ağrıya neden olabilen cerrahi girişimlerdir. Günümüzde multimodal analjezi yaklaşımları kapsamında ultrasonografi (USG) rehberliğinde interfasyal plan blokları, meme cerrahilerinde giderek daha yaygın kullanılmaktadır. Bu çalışmada redüksiyon mamoplasti cerrahisi uygulanan hastalarda ultrasonografi rehberliğinde uygulanan Tip II Pektoral Sinir Bloğu (PECS II) ile Erektör Spina Plan Bloğunun (ESPB) postoperatif analjezi etkinliklerinin karşılaştırılması amaçlanmıştır. Gereç ve Yöntem: Bu çalışma Kahramanmaraş Sütçü İmam Üniversitesi Tıp Fakültesi Klinik Araştırmalar Etik Kurulu onayı ile yürütülen prospektif, iki gruplu karşılaştırmalı klinik bir araştırmadır. Çalışmaya elektif redüksiyon mamoplasti cerrahisi planlanan, 18–55 yaş aralığında ve ASA I–III risk grubunda yer alan kadın hastalar dahil edilmiştir. Hastalar PECS II blok uygulanan grup (Grup P) ve ESPB uygulanan grup (Grup E) olmak üzere iki gruba ayrılmıştır. Tüm hastalara standart genel anestezi uygulanmış ve postoperatif analjezi amacıyla hasta kontrollü analjezi (HKA) cihazı aracılığıyla tramadol verilmiştir. Ağrı değerlendirmesi Numerik Derecelendirme Skalası (NRS) kullanılarak derlenme odasında 30. dakikada ve postoperatif 1., 2., 6., 12. ve 24. saatlerde yapılmıştır. İlk 24 saat içerisindeki toplam tramadol tüketimi, kurtarıcı analjezik ihtiyacı, postoperatif yan etkiler ve hasta memnuniyeti kaydedilmiştir. İstatistiksel analizler SPSS 25.0 yazılımı kullanılarak gerçekleştirilmiş ve p<0.05 değeri istatistiksel olarak anlamlı kabul edilmiştir. Bulgular: Çalışmaya toplam 46 hastanın verileri dahil edilmiştir. Demografik özellikler (yaş, boy, kilo ve ASA sınıfı) açısından gruplar arasında istatistiksel olarak anlamlı fark saptanmamıştır (p>0,05). Postoperatif farklı zaman noktalarında değerlendirilen NRS ağrı skorları açısından PECS II ve ESPB grupları arasında istatistiksel olarak anlamlı fark bulunmamıştır (p>0,05). Bununla birlikte postoperatif ilk 24 saat içerisinde tüketilen toplam tramadol miktarının ESP grubunda PECS grubuna göre anlamlı derecede daha düşük olduğu saptanmıştır (PECS: 31,35 ± 15,06 ml, ESPB: 19,96 ± 8,12 ml; p=0,013). Ayrıca hasta memnuniyeti skorlarının ESPB grubunda anlamlı derecede daha yüksek olduğu belirlenmiştir (p=0,005). Postoperatif bulantı, kusma, solunum depresyonu, pnömotoraks veya lokal anestezik sistemik toksisitesi gibi komplikasyonlar açısından gruplar arasında istatistiksel olarak anlamlı fark saptanmamıştır (p>0,05). Sonuç: Redüksiyon mamoplasti cerrahisinde ultrasonografi rehberliğinde uygulanan hem PECS II blok hem de Erektör Spina Plan Bloğu (ESPB) etkili ve güvenli postoperatif analjezi yöntemleridir. Bununla birlikte ESPB'nin postoperatif opioid tüketimini azaltma açısından daha avantajlı olabileceği görülmektedir. Bu blokların multimodal analjezi stratejileri içerisinde kullanılması postoperatif ağrı kontrolünü iyileştirebilir ve opioid gereksinimini azaltabilir.

Objective: Reduction mammoplasty procedures may result in significant postoperative pain due to extensive tissue dissection and bilateral surgical manipulation. In recent years, ultrasound-guided interfascial plane blocks have gained increasing popularity as part of multimodal analgesia strategies in breast surgery. The aim of this study was to compare the postoperative analgesic efficacy of ultrasound-guided Type II Pectoral Nerve Block (PECS II) and Erector Spinae Plane block (ESPB) in patients undergoing reduction mammoplasty surgery. Materials and Methods: This prospective, two-group comparative clinical study was conducted with the approval of the Clinical Research Ethics Committee of Kahramanmaraş Sütçü İmam University Faculty of Medicine. Female patients aged 18–55 years with ASA physical status I–III who were scheduled for elective reduction mammoplasty were included in the study. Patients were divided into two groups: the PECS II block group (Group P) and the erector spinae plane block group (Group E). Standard general anesthesia was administered to all patients. Postoperative analgesia was provided with tramadol via patient-controlled analgesia (PCA). Pain intensity was assessed using the Numerical Rating Scale (NRS) at the 30th minute in the recovery room and at the 1st, 2nd, 6th, 12th, and 24th postoperative hours. Total tramadol consumption within the first 24 hours, rescue analgesic requirement, postoperative adverse effects, and patient satisfaction were recorded. Statistical analyses were performed using SPSS 25.0 and p<0.05 was considered statistically significant. Results: Data from a total of 46 patients were analyzed. No statistically significant differences were observed between the groups regarding demographic characteristics (p>0.05). Postoperative NRS pain scores at the evaluated time points were also similar between the groups (p>0.05). However, total tramadol consumption during the first 24 postoperative hours was significantly lower in the ESP group compared with the PECS group (PECS: 31.35 ± 15.06 mL, ESP: 19.96 ± 8.12 mL; p=0.013). In addition, patient satisfaction scores were significantly higher in the ESP group (p=0.005). No significant differences were observed between the groups in terms of postoperative complications (p>0.05). Conclusion: Both ultrasound-guided PECS II block and erector spinae plane block are effective and safe analgesic techniques for reduction mammoplasty surgery. However, ESP block appears to provide an advantage by reducing postoperative opioid consumption. Incorporating these techniques into multimodal analgesia strategies may improve postoperative pain control and decrease opioid requirements.
