Özet
Amaç Anatomik akciğer rezeksiyonu planlanan hastalarda preoperatif dönemde uygulanan bilevel positive airway pressure (BPAP) desteğinin, postoperatif pulmoner komplikasyonlar, hastanede kalış süresi ve solunum fonksiyon testleri (FEV₁, FVC) üzerindeki etkilerini değerlendirmek. Yöntem Bu çalışma, Pamukkale Üniversitesi Göğüs Cerrahisi Kliniği'nde Eylül 2024 – Eylül 2025 tarihleri arasında anatomik akciğer rezeksiyonu uygulanan toplam 66 hastayı kapsamaktadır. Tüm hastalara cerrahiden 1 hafta önce standart preoperatif değerlendirme kapsamında arteriyel kan gazı, solunum fonksiyon testleri (FEV₁, FVC) ve DLCO ölçümleri yapılmıştır. Hastalar iki gruba ayrılmıştır: BPAP grubu (n = 33): Ameliyat öncesindeki 1 haftalık dönemde belirlenen protokole göre BPAP desteği uygulanan hastalar. Kontrol grubu (n = 33): Standart preoperatif hazırlık uygulanan hastalar. Her iki grupta da cerrahiden 1 gün önce aynı testler (FEV₁, FVC, DLCO ve arteriyel kan gazı) tekrar edilmiştir. Her iki grupta postoperatif pulmoner komplikasyonlar, hastanede kalış süresi ile tedavi sonrası kontrol döneminde ölçülen DLCO ve solunum fonksiyon testlerindeki değişimlerin karşılaştırması yapılmıştır. Bulgular BPAP uygulanan grupta FEV₁ ve FVC değerleri anlamlı biçimde arttı. Buna karşın postoperatif komplikasyon oranları, dren miktarı ve hastanede kalış süresi açısından iki grup arasında anlamlı fark saptanmadı. Sonuç Preoperatif kısa süreli BPAP uygulaması, anatomik akciğer rezeksiyonu geçirecek hastalarda solunum fonksiyonlarını belirgin şekilde iyileştiren bir yöntemdir. Bununla birlikte, çalışmanın örneklem büyüklüğü sınırlı olduğundan, BPAP'ın postoperatif komplikasyonlar ve hastanede kalış süresi üzerindeki etkisinin daha net ortaya konulabilmesi için daha geniş örneklemli çalışmalar gereklidir.
Özet (Çeviri)
Aim To evaluate the effects of preoperative bilevel positive airway pressure (BPAP) support on postoperative pulmonary complications, length of hospital stay, and pulmonary function test parameters (FEV₁, FVC) in patients scheduled for anatomical lung resection. Methods This study included 66 patients who underwent anatomical lung resection at the Department of Thoracic Surgery, Pamukkale University, between September 2024 and September 2025. All patients underwent standard preoperative assessment 1 week before surgery, including arterial blood gas analysis, pulmonary function tests (FEV₁, FVC), and DLCO measurements. Patients were divided into two groups: BPAP group (n = 33): Patients who received BPAP support according to the predetermined protocol during the 1-week preoperative period. Control group (n = 33): Patients who underwent standard preoperative preparation without BPAP. In both groups, the same assessments (FEV₁, FVC, DLCO, and arterial blood gas) were repeated 1 day before surgery. Postoperative pulmonary complications, length of hospital stay, and changes in DLCO and pulmonary function test parameters measured at the control assessment after preoperative preparation were compared between the two groups. Results The BPAP group demonstrated a significant improvement in FEV₁ and FVC values. However, no significant differences were observed between the groups regarding postoperative complication rates, drainage volume, or length of hospital stay. Conclusion Short-term preoperative BPAP application is an effective method for improving pulmonary function in patients undergoing anatomical lung resection. Nevertheless, due to the limited sample size, further studies with larger cohorts are needed to clarify the impact of BPAP on postoperative complications and hospital stay.
