Özet

Çalışma down sendromlu çocuklarda orofasiyal tedavi ve terapatik Yoganın yutma fonksiyonelliği, uyku alışkanlıkları ve yaşam kalitesi üzerine etkilerini karşılaştırmak amacıyla yapıldı. Çalışmada Down Sendromu (DS) tanısı konmuş 5-12 yaş aralığında 51 çocuk, Fizyoterapi ve Rehabilitasyon Grubu (FTR) ( n=17), Orofasiyal Tedavi (OFT) Grubu: (n=17), Yoga grubu:(n=17) randomizasyon yöntemiyle üç gruba ayrıldı. FTR grubuna kuvvetlendirme, endurans, proprioseptif egzersizlerinden oluşan klasik fizyoterapi ve rehabilitasyon programı uygulanırken, OFT grubuna klasik fizyoterapi ve rehabilitasyon programına ek olarak orofasyal tedavi, Yoga grubuna ise klasik fizyoterapi ve rehabilitasyon programına ek olarak Yoga eğitimi verildi. Tüm gruplara 8 hafta, hafta iki kez, 30-45 dk olacak şekilde tedavi programları uygulandı. Çocukların genel bilgileri tanımlayıcı veri formu ile, yutma problemleri Pediatrik Yeme Değerlendirme Aracı (pEAT-10), uyku alışkanlıkları Çocuk Uyku Alışkanlıkları Anketi (ÇUAA), dil kuvvet ve endurans değerlendirmesi Iowa Oral Performans Cihazı (IOPI), salya kontrolü Salya Şiddet Skalası (SKŞS), baş postürü gonyometre, dil itme şiddeti Dilin İtme Refleksi Değerlendirme Skalası (DİRS), sağlıkla ilişkili yaşam kalitesi KIDSCREEN-27, ebeveynlerin uyku kalitesi ise Pittsburg Uyku Kalite İndeksi (PUKİ) ile tedavi öncesi, tedavi sonrası ve 4.hafta takip dönemlerinde değerlendirildi.DS'li çocuklarda yutma problemlerinin üç grupta da zamanla azaldığı (p<0.05), fakat elde edilen etkilerin sadece Yoga grubunda 4. Hafta devam ettiği bulundu (p<0.05). Dil kaslarının enduransının klasik fizyoterapi ve rehabilitasyon grubunda ve Yoga grubunda arttığı (p<0.05), orofasiyal tedavi grubunda değişmediği tespit edildi (p>0,05), dil kaslarının kuvvetinde ise zamanla üç grupta da artış gözlendi (p<0.05). DS'li çocuklarda salya kontrolü ve baş postüründe hiçbir grupta değişim gözlenmedi (p>0,05). Bu çocuklarda yaşam kalitesi tüm gruplarda zamanla iyileşirken (p<0.05), 4. haftada orofasiyal tedavi ve Yoga uygulanan grupların yaşam kaliteleri değişmezken klasik fizyoterapi uygulanan grupta azalma bulundu (p>0,05). DS'li çocuklarda uyku ile ilgili parametreler incelendiğinde sadece Yoga eğitimi verilen grupta hem çocuk hem de ebeveyn uyku değerlerinde iyileşme gözlendi (p<0.05). Diğer iki grupta ise çocuk ve ebeveyn uyku değerlerinde değişiklik saptanmadı (p>0,05).Çalışma sonucunda DS'li çocuklara uygulanan tedavi protokolleri sayesinde üç grupta da dil kaslarının kuvvetinde iyileşme görüldü.Yoga eğitimi sonrası,vücuttaki stres hormonlarının azalması, zihnin rahatlaması çocuklarda ve yetişkinlerde uykuya geçişi kolaylaştırarak uyku kalitelerinde iyileşme sağlamıştır.

The study was conducted to compare the effects of orofacial treatment and therapeutic Yoga on swallowing functionality, sleep habits and quality of life in children with Down syndrome. Fifty-one children between the ages of 5 and 12 diagnosed with Down Syndrome (DS) were randomly assigned to three groups (FTR Group: n=17, OFT Group: n=17, YOGA Group: n=17) and included in the study. While the FTR group received only rehabilitation treatment (strengthening, endurance, proprioceptive exercises), the OFT group received orofacial treatment (tongue exercises, lip exercises, chewing exercises, nasal breathing exercises, masako maneuver, shaker exercise, nasal hygiene and sleep hygiene education, accompanied by a physiotherapist) + rehabilitation treatment. The YOGA group received therapeutic Yoga (Yoga postures and breathing exercises accompanied by stories) + rehabilitation treatment. All groups received individual, face-to-face sessions of 30-45 minutes twice a week for 8 weeks, for a total of 16 treatment sessions. Children's descriptive data form anamnesis, parental report, and bruxism, snoring, difficulty swallowing, difficulty chewing, and mouth breathing were collected. In addition, children's swallowing problems were assessed using the Pediatric Eating Assessment Tool (pEAT-10), children's sleep habits were assessed using the Child Sleep Habits Questionnaire (CSHQ) for tongue strength and endurance assessment The ol Oral Performance Instrument (IOPI) device was used, for drooling control Saliva Control Saliva Severity Scale (SSSS), head posture was assessed using a goniometer. Children's health-related quality of life was assessed using the KIDSCREEN-27 scale. Parents' sleep quality was assessed using the Pittsburgh Sleep Quality Index (PSQI) for pre-treatment, post-treatment, and 4-week follow-up scores. After treatment, swallowing problems in children with Down Syndrome decreased over time in all three groups (p<0.05), but the effects persisted only in the Yoga group after week 4 (p<0.05). It was found that the endurance of the tongue muscles increased in the classical physiotherapy and rehabilitation group and the Yoga group (p<0.05), and remained unchanged in the orofacial treatment group (p>0.05), an increase in the strength of the tongue muscles was observed in all three groups over time (p<0.05). No changes were observed in saliva control or head posture in any group of children with Down syndrome (p>0.05). While the quality of life improved over time in all groups of these children (p<0.05), at week 4, the quality of life of the groups treated with orofacial treatment and yoga remained unchanged, while a decrease was observed in the group treated with classical physiotherapy (p>0.05). When sleep-related parameters were examined in children with Down Syndrome, improvement in sleep values was observed in both children and parents only in the group that received Yoga training (p<0.05). No changes were detected in child and parent sleep values in the other two groups (p>0.05). The study found that treatment protocols applied to children with Down syndrome resulted in improved tongue muscle strength in all three groups. Following yoga training, the reduction of stress hormones in the body and the relaxation of the mind have made it easier for children and adults to fall asleep, resulting in improved sleep quality.
