Özet
Amaç: Çalışmamızda perioperatif dönemde aktif sürveyans yapılarak kolonizasyon durumu ve mevcut kullanılan profilaksi uygulamaları ile cerrahi sonrası enfeksiyon gelişimi arasındaki ilişkilerin gözlemlenmesi, postoperatif dönemde enfeksiyon gelişimine neden olabilecek diğer risk faktörlerinin ve perioperatif kolonizasyon riskini artıran faktörlerin belirlenmesi amaçlanmıştır. Cerrahi sonrası hastane ve yoğun bakım yatış süresini etkileyen durumlar, tüm nedenlere bağlı 90 günlük mortalite ve profilaksisi ilişkili yan etki gelişimi incelenen konular arasında yer almıştır. Gereç ve Yöntem: Prospektif gözlemsel kohort olarak tasarlanan çalışmada, Eylül 2025-Şubat 2026 tarihleri arasında Ankara Üniversitesi Genel Cerrahi Anabilim Dalı tarafından elektif abdominal cerrahi işlem yapılması planlanan hastalar, preoperatif dönemde rektal ve inguinal bölgelerinden alınan tarama örnekleri sonuçlarına göre çoklu ilaca dirençli mikroorganizma (ÇİDM) kolonizasyonu olan ve olmayanlar olarak iki grupta incelenmiştir. Hastalar postoperatif dönemde 90 gün boyunca cerrahi sonrası gelişen enfeksiyonlar, yatış süresi, mortalite ve profilaksi ilişkili yan etki gelişimi açısından takip edilmiştir. Postoperatif 5. günden sonra hastane yatışı devam eden hastalardan kontrol tarama örnekleri alınmıştır.Perioperatif tarama örneklerine ön zenginleştirme yöntemi sonrası standart ekim yöntemleri ve fenotipik direnç testleri uygulanarak kolonizasyon durumu tespit edilmiştir. Veriler SPSS 26.0 ile analiz edilmiştir. Parametrik iki grup karşılaştırmalarında varyans homojenliği Levene testi ile değerlendirilmiştir ve homojenlik sağlanamadığında Welch düzeltmesi uygulanmıştır. Anlamlı karşılaştırmalar için uygun etki büyüklüğü ölçütleri kullanılmıştır. Bulgular: Çalışmaya 59 hasta dahil edilmiştir. Preoperatif kolonize olmayan grup 33 hasta ( %55,9) ve kolonize olan grup 26 hasta ( %44,1) olarak tespit edilmiştir. Yapılan cerrahi işlemler içerisinde ilk sırada kolorektal bölge cerrahileri yer almıştır (n = 35; %53,8). Cerrahi sonrası 90 günlük takipte enfeksiyon gelişim oranı %49,1 olarak saptanmış (n = 29). En sık gelişen enfeksiyon türü %48,5 oranında (n = 16) cerrahi alan enfeksiyonu olmuştur. 90 günlük tüm nedenlere bağlı mortalite %3,4 oranında (n = 2) belirlenmiştir. ÇİDM kolonizasyon varlığı ile cerrahi sonrası enfeksiyon gelişimi, uzamış hastane yatışı ve 90 günlük mortalite arasında bağımsız bir ilişki gösterilememiştir (p > 0,05). En sık kullanılan profilaktik antibiyotik sefazolin olmuştur ve profilaksi ilişkili advers olay gelişim oranı %6,7 saptanmıştır. Perioperatif antibiyotik profilaksi uygulamalarında kullanılan antibiyotik grubu, dozlama süresi ve postoperatif dönem profilaksinin devamı enfeksiyon gelişimi ile ilişkili bulunmuştur (p <0,05). Cerrahi sonrası profilaksinin devamının enfeksiyon gelişimini önlemede ek yarar sağlamadığı gözlenmiştir. Ayrıca hasta ve cerrahi ilişkili bazı faktörler ile enfeksiyon gelişimi arasında ilişki gösterilmiştir (p <0,05). Ek olarak perioperatif aktif sürveyans; kolonizasyon durumunun zaman içerisinde değişkenlik gösterebileceğini ve rektal tarama örneklerinin inguinal tarama örneklerine göre özellikle dirençli gram negatif mikroorganizma kolonizasyonunu saptamada daha yüksek duyarlılığa sahip olduğunu göstermiştir. Sonuç: Çalışmamız perioperatif kolonizasyon, antibiyotik profilaksi uygulamaları ve enfeksiyon risk faktörlerini aynı hasta popülasyonu içerisinde birlikte değerlendirmiştir. Elde edilen bulgular, cerrahi sonrası enfeksiyon gelişimi önleme stratejilerinin bütüncül değerlendirilmesinin gerekli olduğunu göstermektedir. Ancak gelecekte yapılacak çok merkezli, daha geniş örneklemli ve moleküler yöntemlerin kullanıldığı çalışmalarla, ÇİDM kolonizasyonunun sonuçlar üzerine etkisini daha net ortaya koyabileceği ve yüksek riskli hasta gruplarında uygulanacak enfeksiyon önleme stratejilerinin geliştirilmesine katkı sağlayabileceği düşünülmektedir.

Background: The aim of this study was to observe the relationships between colonization status, currently used prophylactic practices, and the development of postoperative infections through active surveillance during the perioperative period, as well as to identify other risk factors that may contribute to the development of postoperative infections and factors associated with an increased risk of perioperative colonization. Conditions affecting postoperative hospital and intensive care unit length of stay, 90-day all-cause mortality, and the development of prophylaxis-related adverse events were also among the outcomes investigated. Materials and Methods: In this prospective observational cohort study, patients scheduled to undergo elective abdominal surgery by the Department of General Surgery at Ankara University İbni Sina and Cebeci Hospitals between September 2025 and February 2026 were classified into two groups according to the results of preoperative rectal and inguinal screening samples: those with and those without multidrug-resistant microorganism (MDRO) colonization. Patients were followed for 90 days postoperatively for the occurrence of postoperative infections, length of hospital stay, mortality, and prophylaxis-related adverse events. Follow-up screening samples were obtained from patients whose hospitalization continued beyond the fifth postoperative day. Colonization status was determined by applying standard culture methods and phenotypic resistance testing following a pre-enrichment procedure to perioperative screening samples. Data were analyzed using SPSS version 26.0. Homogeneity of variances in parametric two-group comparisons was assessed using Levene's test, and Welch's correction was applied when homogeneity assumptions were not met. Appropriate effect size measures were used for statistically significant comparisons. Results: A total of 59 patients were included in the study. The preoperatively non-colonized group consisted of 33 patients (55.9%), while the colonized group consisted of 26 patients (44.1%). Colorectal surgical procedures constituted the most common type of surgery performed (n = 35; 53.8%). During the 90-day postoperative follow-up period, the overall infection rate was 49.1% (n = 29). Surgical site infection was the most frequently observed infection type, accounting for 48.5% of all infections (n = 16). The 90-day all-cause mortality rate was 3.4% (n = 2). No independent association was demonstrated between the presence of MDRO colonization and postoperative infection development, prolonged hospital stay, or 90-day mortality (p > 0.05). Cefazolin was the most commonly used prophylactic antibiotic, and the rate of prophylaxis-related adverse events was 6.7%. The antibiotic class used for perioperative antibiotic prophylaxis, timing of administration, and continuation of prophylaxis into the postoperative period were found to be associated with infection development (p < 0.05). Continuation of prophylaxis in the postoperative period did not provide additional benefit in preventing postoperative infections. Furthermore, several patient-related and surgery-related factors were associated with infection development (p < 0.05). In addition, active perioperative surveillance demonstrated that colonization status may vary over time and that rectal screening samples have higher sensitivity than inguinal screening samples, particularly for detecting colonization with resistant Gram-negative microorganisms. Conclusion: This study evaluated perioperative colonization, antibiotic prophylaxis practices, and infection-related risk factors simultaneously within the same patient population. The findings indicate that strategies aimed at preventing postoperative infections should be assessed using a comprehensive approach. However, future multicenter studies with larger sample sizes and the incorporation of molecular methods may provide a clearer understanding of the impact of MDRO colonization on clinical outcomes and contribute to the development of infection prevention strategies for high-risk patient populations.
