Özet
Amaç: Crowe Tip 2 ve 3 GKD zemininde gelişmiş koksartrozlu hastalarda yapılan çimentosuz total kalça artroplastisinde uygulanmış olan femur başı otogreftinde çift planlı vida fiksasyonu ile asetabular rekonstrüksiyon yapılmış olan hastaların takiplerinde uygulanan yöntemin radyolojik olarak değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Gelişimsel kalça displazisi sebebiyle total kalça artroplastisi uygulanan kriterlere uygun 9 hasta çalışmaya dahil edilmiştir. Hastaların minimum 24 ay olacak şekilde takip grafisinde greftin kaynama durumu, asetabular kap migrasyonunun varlığı, greftin rezorpsiyonu, greftin yer değiştirmesi ve asetabular kapın greft tarafından örtünüm oranları, lateral inklinasyon açısı standart pelvis anteroposterior grafileri üzerinden değerlendirildi. Bulgular: Hastaların takip grafileri değerlendirildiğinde hastaların tamamında greftte tam kaynama tespit edildi. Hiçbir hastada greft migrasyonu, asetabular kap migrasyonu görülmedi. Sonuç: Crowe Tip 2 ve Tip 3 gelişimsel kalça displazisi nedeniyle total kalça artroplastisi uygulanan hastalarda, femoral baş otogreftinin“çift planlı vidalı fiksasyon”tekniği ile asetabular augmentasyonu, radyolojik açıdan son derece başarılı sonuçlar vermiştir. Bu teknik, yüksek greft kaynama oranı, minimal greft rezorpsiyonu ve mükemmel asetabular komponent stabilitesi sağlayarak, displastik asetabulumun rekonstrüksiyonunda güvenilir ve etkili bir seçenek olarak öne çıkmaktadır. Özellikle orta ve ileri dereceli kemik defektlerde sağladığı mekanik avantaj, uzun dönem implant sağkalımı için umut vaat etmektedir.

Objective: The aim of this study was to radiologically evaluate the outcomes of acetabular reconstruction using femoral head autograft with double-plane screw fixation in patients with advanced osteoarthritis secondary to Crowe type II and III developmental dysplasia of the hip (DDH) who underwent cementless total hip arthroplasty. Methods: Nine patients who met the inclusion criteria and underwent total hip arthroplasty due to developmental dysplasia of the hip were included in the study. Follow-up radiographs obtained over a minimum period of 24 months were evaluated. Graft status, presence of acetabular cup migration, graft resorption, graft displacement, acetabuler cup coverage by the femoral head graft, and the lateral inclination angle were assessed on standard anteroposterior pelvic radiographs. Results: Evaluation of follow-up radiographs demonstrated complete graft union in all patients. No graft migration or acetabular cup migration was observed in any of the cases. Conclusion: In patients with Crowe type II and III developmental dysplasia of the hip undergoing total hip arthroplasty, acetabular augmentation using femoral head autograft with a double-plane screw fixation technique yielded highly successful radiological outcomes. This technique provides a high rate of graft union, minimal graft resorption, and excellent acetabular component stability, making it a reliable and effective option for reconstruction of the dysplastic acetabulum. The mechanical advantages offered by this method, particularly in moderate to severe bone defects, appear promising for long-term implant survival.
