Özet
ABSTRACT A STUDY ABOUT STAFFING AND STAFF MOVEMENT OF NURSES AND MIDWIFES AT İZMİR ATATÜRK TRAINING AND RESEARCH HOSPITAL The shortage of health personnel is a problem of great importance. One of the important problems experienced is the maldistribution of personnel, the other is the movement inside and outside the hospital which affects the productivity of the staff in a negative way The assessment of the present condition in the planning of health manpower will be a good guide for the future plans to be made. From this point of view, the aim of this study is to examine the distribution of nurses and midwifes among the units, their staffing profile, working hours, patient-nurse ratio on each shift, nursing hours, their movement trends inside and outside the institution and affecting factors. Data were obtained from 460 nurses and midwifes staffed in a teaching hospital during the May of 2001. The questionnaire included items about demographics, working conditions and movement profile of the sampling. The data for the staffing profile obtained from the monthly patient-nurse records in every unit. The number of nurses working at clinics ranged from 6-8. The number of patients per nurse ranged from 4.5 and 8.8 during the day shift and 13.6 and 30.9 during the night shift in medical and surgical units. Nursing hours per patient day ranged from 1.4 and 2.7 hours for medical and surgical clinics and ranged from 4.8 and 24.0 for intensive care units. The time spent for direct care was less than the indirect care in the day shifts. It has been determined that 15.6% of the nurses have been moved once a year, 1.5% of them have been moved twice a year in the institution. There has been no significant effect of demographic characteristics and working conditions on movement in the institution. Results showed that 83.7% of the sampling have been moved 1-7 times between the health care institutions throughout working life. The reasons of movement outside the institutions were marital status, individual desire, willing to be close to their family, education, desire to live in big cities, difficult working conditions in villages, obligatory appointment, resignation, temporarily appointment. The movement number has been found high among the married with 2 or more children, among the graduates baccalaureate and associate degree programs, among the staff older than 30. Implications for planners, nurse managers and researchers from the research are recommended.
