Özet

62 ÖZET Bu çalışmada psoriasis vulgarisli hastalarda dar bant UVB tedavisinin etkinliği, yan etkileri ve remisyon süreleri üzerine etkisi araştırılmak istenmiştir. Çalışmaya Dicle Üniversitesi Tıp Fakültesi Dermatoloji polikliniğine başvuran, vücudun %10'undan fazlasında lezyon bulunan yada daha önceden en az 2 ay topikal tedavi alıp iyileşmeyen 16' sı kadın, 17' si erkek toplam 33 psoriasisli hasta alındı. Minimal eritem dozunun %70'i verilerek tedaviye başlandı ve her seansta 311nm UVB dozu %20 artırıldı. Başlangıç PASI skorlarında % 70 iyileşme elde edilene kadar haftada 3 seans, sonraki 1 ay haftada 2 ve 1 ay da haftada 1 seans dar bant UVB uygulandı. Belirgin iyileşme olmayan olgulara 15 günde bir 2 ve ayda bir 2 seans daha verilerek tedavi sonlandırıldı. Yan etki olarak 13 hastada eritem, 5 hastada pruritus, 2 hastada subakut fototoksisite, 2 hastada lentigo, 1 hastada punktat keratoz, 2 hastada Köbner fenomeni, 1 hastada vezikül gelişimi, 2 hastada alt ekstremitelerde ödem ve 1 hastada yüz bölgesinde demodikozis gözlendi. Hastaların 30'unda (% 90) tam veya parsiyel iyileşme elde edildi. Üç hastada (% 10) ise hiç iyileşme görülmedi. Hastaların 9'unda (% 27) idame tedavisi esnasında, 7' sinde (% 21) tedavi kesildikten ortalama 2 ay sonra nüks görüldü. Ondört hastanın (% 42) ise 6 'sı 1 yıldan beri, 8'i ise 6 aydan beri remisyondadır. Sonuç olarak dar bant UVB fototerapisi psoriasis tedavisinde etkili, güvenli ve tölerabilitesi yüksek olan bir tedavi alternatifidir.

63 SUMMARY The aim of this study is to evaluate the effectiveness of narrow band UVB phototherapy, its adverse effects and its effects on the duration of remission in the patients with psoriasis vulgaris. Thirty three patients (16 female, 17 male) whose more than 10% of then- bodies covered with psoriatic lesions or who could not recover despite a topical therapy for at least two months and applied to the outpatient Clinics of Dermatology in the Medicine Faculty of Dicle University, were included to the study. Therapy was started with the 70% of MED, and in each session the dose of the 311nm UVB was increased 20% and it was given 3 times in a week until a 70% decrease in the PASI score was obtained at the beginning of the study. Narrow band UVB therapy was applied twice a week in the first month, and once a week in the following month. Extra dose was applied to the patients who did not have significant clearence, twice a month and once a month for the following two months. The adverse effects of the therapy were; erythema on 13 patients, pruritus on 5 patients, subacute phototoxicity on 2 patients, lentigo on 2 patients, punctate keratosis on 1 patient, Kobner reaction on 2 patients and demodicosis on face on 1 patient. Complete or partial clearence was obtained from 30 patients (90%). There was no response from 3 patients (10%). There was a recurrence on 9 patients (27%) during the main therapy and on 7 patients (21%) 2 months after the therapy. Six patients have been in remission for 1 year and 8 of them have been for 6 months. As a result narrow band UVB is an effective, tolerable, safe and alternative treatment modality for psoriasis vulgaris.
