Özet
VI-TÜRKÇE ÖZET HAFİF, ORTA VE İLERİ HALLUKS VALGUS DEFORMİTESİNDE KLİNİK UYGULAMALARIMIZ: Bunionu yada bunionları olan bir hastada hangi prosedürün yapılacağına karar vermek için, cerrahi düzeltme gerektirecek spesifik patolojilerin dikkatli bir fizik muayene ve radyolojik inceleme sonrası ortaya konulması gerekir. Deformitenin artmış halluks valgus açısı, intermetatarsal açı, başparmağın pronasyonu, artmış distal metatarsal eklem açısı, genişlemiş medial eminens, sesamoidlerin subluksasyonu gibi tüm elementleri düzeltilmelidir. Bu deformitelerin herhangi birisinde düzeltilme yetersizliği rekürren deformite oluşumu için risk oluşturacaktır. Yapılan geniş seriler sonucu ortaya çıkan gerçek tüm bunion deformitelerin çözümünün tek bir metodla çözümlenemeyeceğidir Semptomatik orta ve ileri dereceli halluks valgus deformiteli hastalarda modifiye proksimal osteotomi+distal yumuşak doku prosedürü ile hafif ve orta dereceli halluks valgus deformiteli hastalarda modifiye distal chevron osteotomisi+Stofella DC halluks valgus splinti uygulamalarımızı değerlendirdik. GATA Ortopedi ve Travmatoloji Ana Bilim Dalında Temmuz 1999-Temmuz 2003 tarihleri arasında 69 hasta 79 ayak proksimal ve distal osteotomi tekniği ile cerrahi olarak tedavi edildi.Hastalanmızı 2 gruba ayırdıkJ.gruptaki hastalar orta ve ileri derece halluks valgusu olan ve uyumsuz ekleme sahip hastalardı ve bunlara modifiye proksimal kresentrik osteotomi+distal yumuşak doku prosedürü uygulandı.II.grup hastalar ise hafif ve orta dereceli, uyumlu ekleme sahip halluks valgus hastalan idi ve bu hastalara modifiye distal chevron osteotomisi uygulandı. Kaynak bilgiler ile birlikte değerlendirildiğinde, olgularımızdan edindiğimiz sonuçlar uyumsuz ekleme sahip orta ve ileri dereceli halluks valgus deformitesinde modifiye proksimal kresentrik osteotomi+distal yumuşak doku prosedürü;uyumlu ekleme sahip hafif ve orta dereceli halluks valgus deformitesinde modifiye distal Chevron osteotomisi+Stofella DC halluks valgus splinti uygulamalarının, halluks valgusu, metatarsus primus varusu düzeltmek, l.MTP eklemin normal mekaniğini sağlamak, ağrıyı yok etmek için uygun ve ideal tedavi yöntemleri olduğu ve tedavide seçilebilecek uygun alternatifler olduğu tesbit edildi. 52

Vn-İNGİLİZCE ÖZET: CLINICAL APPLICATIONS IN MILD AND MODERATE HALLUX VALGUS Before deciding to type of operative procedures must be applied in a patient with bunion or bunions, a detailed clinical and radiological examination must be performed in order to determine the spesific pathologic structures which will be corrected operatively. All elements of deformity including increased hallux valgus, intermetatarsal, distal metatarsal articular angles, pronation of hallux, enlargement of medial eminens and subluxation of sesamoids must be corrected. Inadequate correction of any of these deformities will increase rik of the recurrense. The results of many clinical research showed that, all bunion deformities can not be corrected with one type of operative method. We tried to asses our results of modified proximal osteotomy + distal soft tissue procedure in patients with moderate- severe hallux valgus and distal chevron procedure+ stofella DC in patients with mild - moderate hallux valgus deformity. Between July 1 999 - July 2003, 79 hallux of 69 patients were treated by proximal and distal osteotomy methods in GATA department of orthopaedics. we divided our patient into 2 groups. In group I there were patients with moderate and severe hallux valgus deformities and incongruent joint, who were treated by modified cresenteric osteotomy + distal soft tissue procedure. In group II there were patients with mild - moderate hallux valgus deformities and congruent joint, who were treated by modified chevron procedure. Overall, the assesment of our results by the help of references and literature showed that, modified cresenteric osteotomy + distal soft tissue applications in patients with moderate - severe hallux valgus deformity with the presence of incongruent joint structure, modified chevron procedure + stofella DC applications in patients with mild - moderate hallux valgus with the presence of congruent joint structure will be appreciate alternatives in the treatment of these deformities. 53
