Özet

ÖZET Fresenius Comtec Aferez Cihazları İle Uygun Donörlerden Çift Doz Trombosit Süspansiyonu Elde Edilmesinin Değerlendirilmesi Donör kazanım programlarının öneminin artmakta olduğu her geçen gün, bir donörden mümkün olan en fazla ürünü elde etmekte bir o kadar önem kazanmıştır. Özellikle elde edilmesi diğer kan komponentlerine oranla daha zor olan ve saklama süresi açısından daha kısa ömürlü olan trombosit süspansiyonları için bu konu daha da önemlidir. Aferez cihazlarının icadı ve geliştirilmesi ile tek donörden, konvansiyonel metodlara göre 6 ile 8 donörden elde edilen trombosit ürünü sağlanabilmiş, şimdi de bu miktarın iki kat daha yükseltilmesi gündeme gelmiştir. Uygun olan her donörden birden fazla yahut çift doz trombosit sağlamak, hem hasta yakınlarının daha fazla donör bulma ihtiyacını ortadan kaldıracak, hem aferez ünitesinin maliyetlerini aşağıya çekecek, hem de ünitenin daha karlı ve stoklu çalışmasını sağlayacaktır. Eğer çalışılan sistem ürün içerisindeki lökosit kontaminasyonunu belirli sınırların altına çekmeyi başarabiliyorsa filtre kullanımı ihtiyacı da ortadan kalkacaktır. Günümüzde uygun olan tüm donörlerden çift doz trombosit toplanması iyice önem kazanmıştır. Tüm aferez cihazı üreticileri uygun olan tüm donörlerden çift doz trombosit elde etmenin mümkün olduğunu iddia etseler de, işlem süratinin ve elde edilen ürün içindeki lökosit kontaminasyonun belirli sınırlar içerisinde olup olmadığının tespit edilmesi şarttır. Fresenius Comtec aferez cihazları ile bu konu üzerinde şu ana kadar yapılmış belirgin bir çalışma yoktur. Yapılacak araştırma ve denemeler ile Comtec cihazlarının çift doz trombosit toplama üzerine efektivitesi saptanmıştır. Çalışma esnasında ağırlığı > 75 kg, trombosit ön sayımlan > 250xl09/L olan donörlerde Fresenius Comtec aferez cihazları ile çift doz trombosit toplanmasına çalışılmıştır. 25Bu araştırma ile Fresenius Comtec aferez cihazlarının çift doz trombosit süspansiyonu toplamadaki etkiliği yukarıda bahsi geçen birçok yönden değerlendirilmiş ve kısa sürelerde, cihazın öngörülen tüm işlemleri lökosittten arındırılmış olarak tanımlanan standartlara uygun elde edebildiği tespit edilmiştir.

SUMMARY Evaluation of the Fresenius Comtec Blood Cell Seperator in the Collection of Double Dose Apheresis Platelets from Suitable Donors Donor recruitment programs are becoming very important every day and it also increases the importance of getting the maximum amount of products from single donors. Especially when we talk about platelets, because of difficulties of getting and storing them, it is more and more important. With the invention and improvement of blood cell separators, it becomes possible to get the equal amount of 6 to 8 units of random donor platelets from a single donor with an apheresis device and nowadays everyone is trying to get the double of this amount. If it is possible to get double units of platelets from suitable donors, it will be very helpful for both the patients' and the patients relatives' side, because there would be less needed number of donors. On the other side, apheresis unit will have high stocks of platelets and their cost will drop down dramatically. At the same time, if the number of leukocytes in the product is under some limitations, it will reduce the filter usage with the platelet suspensions. This will also reduce the costs of the apheresis unit. All apheresis devices and disposables manufacturers claims that, from every suitable donor it is possible to get double units of platelets, but the total time of the procedure and the amount of leucocytes in the product bag should be mentioned and examined. Up to now, there had not been any study with the Fresenius Comtec Cell Separators concerning these issues. With this new study we will evaluate the results of Comtec. During the study, we try to get double units of platelets from donors, which have a platelet pre-count >250xl09/L and have a weight of >75 kg. 27As a conclusion, Fresenius Comtec Cell Seperators are evaluated for collection of double dose platelets and found out that in very short times the device is capable of getting the expected amount of products in determined leuko-reduced standards.
