Özet

VII. ÖZET AMAÇ: Bu çalışmanın amacı el ekzemalı erkek ve kadın hastalarda seksüel fonksiyonu değerlendirmek ve birlikte olan depresyonun cinsel fonksiyon üzerinde negatif etkisi olup olmadığını incelemektir. GEREÇ VE YÖNTEM: Toplam 91 kadın ( 43 el ekzemalı hasta ve 48 sağlıklı kontrol) ve 79 erkek (45 el ekzemalı hasta ve 34 sağlıklı kontrol) çalışmaya alındı. Hasta grupta el ekzema şiddet indeksinin değerlendirilmesinde el elzema şiddet indeksi kullanıldı. Kadınlarda seksüel fonksiyonu değerlendirmek için“The Female Sexual Function Index”(FSFI), erkeklerde seksüel fonksiyonu değerlendirmek için“The International Index of Erectile Function”(IIEF) kullanıldı. Yaşam kalitesi“Dermatolojik Yaşam Kalite İndeksi”(DYKİ) ile değerlendirildi. Depresyon tanısı“Structured Clinical Interview for DSM-IV (SCID-I)”görüşme sonucu konuldu ve derecelendirme içi“Hamilton Depresyon Derecelendirme ölçeği (HAM-D)”kullanıldı. BULGULAR: 43 kadın hastanın 26' sında (60.46%) ve 45 erkek hastanın 11' inde (24.44%) depresyon bulundu. Depresyonlu el ekzemalı kadın grupta kontrol grubuna göre toplam FSFI skorunda belirgin azalma saptandı (20.84±9.19, 24.04±3.40, p0.05). Depresyonlu el ekzemalı kadın grupta depresyonsuz el ekzemalı kadın gruba göre toplam FSFI skorunda belirgin düşüş saptandı (20.84±9.19 ve 22.23±5.82, p0.05). Korelasyon analizi ile depresyonsuz el ekzemali kadın hastalarda FSFI skoru ve HAM-D ölçeği arasında negatif korelasyon saptandı (r=-0.592, p=0.012). FSFI skoru, El Ekzema Şiddet İndeksi ve DYKİ ile korele değildi (p>0.05). Depresyonlu el ekzemali kadın hastalarda FSFI skoru, El Ekzema Şiddet İndeksi, DYKİ ve HAM-D ile korele değildi (p>0.05). El ekzemali erkek hastalarda IIEF skoru, El Ekzema Şiddet İndeksi, DYKİ ve HAM-D ile korele değildi (p>0.05). Alt gruplar açısından seksüel fonksiyondaki bozulma kadınlarda ağrı dışındakilerde anlamlı, erkeklerde ise sadece ilişki başarısında anlamlı bulundu. SONUÇ ve ÖNERİLER: Çalışma sonucunda el ekzemali hastalarda kontrol grubuna kıyasla belirgin seksüel fonksiyon bozukluğu saptandı ve depresyonun varlığı el ekzemali hastalarda seksüel fonksiyon bozukluğunu negatif etkiledi. El ekzemali hastalar daha iyi bir yaşam kalitesi sağlamak için seksüel fonksiyon ve depresyon açısından değerlendirilmelidir. 43

VIII. SUMMARY OBJECTIVE: The aim of this study was to investigate sexual function in females and males with hand eczema and evaluate whether the coexistent depression has an additional negative effect on sexual function in these patients. MATERIALS and METHODS: A total of 91 female subjects ( 43 with hand eczema patients and 48 healthy volunteers as a control group) and 79 male subjects (45 with hand eczema patients and 34 healthy volunteers as a control group) were enrolled in the study.The hand eczema severity index (HECSI) was used to determine the severity of hand eczema for the patient groups. The Female Sexual Function Index (FSFI) was used to assess female sexual function and the international index of erectile function (IIEF) was used to evaluate male sexual function. Quality of life was assessed with the Dematology Life Quality Index (DLQI). The diagnosis of depression was made according to the SCID-I interview and Hamilton Depression Rate Scale (HDRS) was used for grading depression. RESULTS: From the 43 female subjects with hand eczema, 26 of them were found to have depression (60.46%) and from the 45 male subjects with hand eczema, 1 1 of them were found to have depression (24.44%) and FSFI total score was found to be significantly decreased in female hand eczema patients plus depression compared with healthy controls (20.84±9.19, 24.04±3.40, p0.05). FSFI total score was found to be significantly decreased in female hand eczema 44patients plus depression compared with hand eczema patients without depression (20.84±9.19 ve 22.23±5.82, p0.05). Correlation analysis revealed a negative correlation between FSFI score and HDRS in patients with female hand eczema without depression (r=-0.592, p=0.012). FSFI score was not correlated with HECSI and DLQI score (p>0.005). FSFI score was not correlated with HECSI, DLQI and HDRS scores in female patients with hand eczema plus depression (p>0.05). IIEF score was not correlated with HECSI, DLQI and HDRS scores in male patients with hand eczema (p>0.05). CONCLUSIONS and SUGGESTIONS: The results of the study demonstrated that patients with hand eczema, have distinct sexual dysfunction compared with healthy controls, and coexistent depression has additional negative effect on sexual dysfunction in our patients. Patients with hand eczema should be evaluated in terms of sexual function and depression in order to provide a better quality of life. 45
