Özet
ÇalıĢma, farklı fonksiyonel seviyedeki Duchenne Musküler Distrofi (DMD)‟li çocuklarla benzer yaĢtaki sağlıklı çocukların yorgunluklarını ve enerji tüketimlerini karĢılaĢtırmak amacıyla planlandı. ÇalıĢmaya yaĢları 6-11 yıl arasında değiĢen, Brooke Alt Ekstremite Fonksiyonel Sınıflandırması‟na göre Seviye 1‟den 18, Seviye 2‟den 17, Seviye 3‟ten 17 olmak üzere DMD tanısı almıĢ 52 çocuk ve benzer yaĢtaki 17 sağlıklı çocuk dahil edildi. Olguların demografik bilgileri kaydedildi. Olguları değerlendirmek için alt ekstremite limitasyon ve kısalıkları değerlendirmesi, 6 Dakika Yürüme Testi (6DYT), Northstar Ambulasyon Değerlendirmesi (NSAA), Fizyolojik Harcama Ġndeksi (FHĠ), Brooke fonksiyonel sınıflaması, Süreli Performans Testleri, Pediatrik YaĢam Kalitesi Envanteri (PedsQL) YaĢam Kalitesi yöntemleri kullanıldı. Yorgunluk için Seviye 1 ile 3, Seviye 2 ile 3, Seviye 3 ile sağlıklı grup arasında farklar saptandı (p<0.0083). Yürüme mesafesi, on metre koĢma, merdiven inme-çıkma, sırtüstü yerden kalkma aktivitelerinde tüm gruplarda farklar saptandı (p<0.0083). FHĠ olarak Seviye 1 ile 3 ve Seviye 2 ile 3 arasında farklar saptanırken (p<0.0083) diğer gruplar arası bir fark saptanmadı. Tüm seviyelerde ayak bileği limitasyon açısından farklar saptandı (p<0.016). Gruplar arasında yaĢam kalitesi açısından bir fark saptanmadı. ÇalıĢmamızda DMD‟li çocukların enerji tüketimi ve yorgunluk seviyesinin sağlıklı yaĢıtlarına göre daha fazla olduğu ve fonksiyonel seviye azaldıkça bu farkın daha da arttığı belirlendi.

The aim of this study is designed to compare the fatigue and energy consumption of children with DMD in different functional levels with the healthy children of similar age. Sixty nine children between the ages of 6 and 11 were included in this study where 52 of them were diagnosed with Duchenne Musclar Dysthropy (DMD) in Level I (18 patients), Level II (17 patients), and Level III (17 patients) according to Brooke Functional Classification Scale and 17 of them healthy children were included. The demographic characteristics of the children were recorded. Lower limb limitation and shortness assessment, 6 Minute Walk Test (6MWT), Northstar Ambulatory Assessment Scale (NSAA), Physiological Cost Ġndex (PCI), Brooke functional classification, Timed performance tests, The Pediatric Quality of Life Inventory (PedsQL) life quality methods were used to assess the children. Fatigue, in between Level I and III, Level II and III, the healthy group and Level 3 differences were found (p<0.0083). Difference in all groups was found in walking distance, ten meters running, stair descending-climbing, standing up lying on your back in activities (p<0.0083). In between Level I and Level III, Level II and III groups differences were found for PCI (p<0.0083), the other groups had no significant difference. For all levels in terms of ankle limitation were found differences (p<0.016). There was no difference in all groups in terms of quality of life. In our study, energy and fatigue levels of children with DMD were found to be more than their healthy peers and determined that a reduction in the functional levels increased further in this difference.
