Özet
Amaç: Korneal ektazili gözlerde Rose K2 ve Orbiflex K korneal sert lenslerin güvenilirlik ve etkinliğinin değerlendirilmesi Gereç-Yöntem: Ankara Üniversitesi Tıp Fakültesi Göz Hastalıkları Anabilim Dalı Kornea ve Kontakt Lens Servisi'ne Ocak 2008- Ağustos 2015 tarihleri arasında korneal ektazi tanısı ile gelen ve kliniğimizde en az 2 takibi olan olgular dahil edildi. Tüm hastalar; kornea ektazi tipleri, demografik verileri, görme keskinlikler, manifest refraksiyon değerleri, biyomikroskopik muayene bulguları, keratometri, korneal topografi açısından değerlendirildi. Bulgular:Çalışmaya 483 olgunun 860 gözü dahil edildi. Çalışmaya dahil edilen olguların %54'ü erkek, %46'sı kadındı. Çalışmaya hastaların korneal sert lens uygulanan gözleri dahil edildi ve dahil edilen gözlerin %49.9'ü sağ göz, %50.1'i sol göz idi. Hastaların ortalama yaşı 26.7±9.9 idi. 855 gözde keratokonus, 1 gözde keratoglobus, 3 gözde PMD, 1 gözde post-graft ektazi mevcuttu.Hastalar Amsler-Krumeich evresine göre sınıflandırıldıklarında; evre 1 KC -274 göz (%31.9), evre 2 KC -302 göz (%35.1), evre 3 KC -95 göz (%11), evre 4 KC -189 göz (%22) olarak gruplandırılmıştır. Hastaların düzeltilmemiş görme keskinlikleri, gözlükle düzeltilmiş görme keskinlikleri, korneal sert lens ile düzeltilmiş görme keskinlikleri ve korneal sert lens reçete edildikten sonra takiplerindeki en son muayenelerindeki görmeleri sırasıyla; 0.99 ± 0.60 (0-2.6), 0.39 ± 0.39 (0-2.6) , 0.05 ± 0.33 (0-9), 0.08 ± 0.54 (0-9) logMAR'dır. Hastalara uygun duruş bulunanana kadar denenen lens sayısı ortalama 2.01±1.0 (1-6) idi. İlk denenen lensin BCR'ı ile uygun duruş elde edilen lensin BCR'ı arasında ortalama 0.15±0.10 fark vardır. Olguların 793'ünde korneal sert lens başarılı şekilde uygulanmış ve reçete edilmiştir. Altmış yedi gözde (%7.8) korneal sert lens uyumsuzluğu tespit edilmiş ve korneal sert lens reçete edilmemiştir. Bu hastalara; INTACS , PPK , Kerasoft ve Skleral lens önerilmiştir. Hastaların izlemleri boyunca 107 göze yeniden reçete yazılma gereksinimi duyulmuştur. Reçete yazılan hastalardan 162 (%18.8) olgu lensini hiçbir zaman almamıştır. 91 (%10.6) olgu ise lensini almış kontrole gelmiş fakat takiplerinde lensini bırakmıştır. Olguların 749'unda (%87.1), takip süreleri boyunca reçete edilen korneal sert lens'nin BCR veya dioptri değerleri değiştirilmemişken, 83 olguda (%9.7) reçete 1 kere, 21 olguda (%2.4) 2 kez, 5 olguda (%0.6) 3 kez ve 2 hastada (%0.2) 4 kez değiştirilmiştir. Ortalama takip süresi 14.3 ± 18.1 (6-127) aydı. Takip boyunca 1 olgu gözündeki steril infiltrat dışında komplikasyonla karşılaşılmadı. Sonuç:Korneal sert lensler KC'da her evrede refraktif kusuru etkili olarak düzeltirler. Uygulamaları ve bakımları kolay olan bu lensler uzun dönem takipte ciddi komplikasyonlara neden olmaksızın güvenle kullanılabilirler.

Purpose: To evaluate the safety, efficacy, and on-eye performance of Rose K2 and Orbiflex K contact lens fitting in ectatic corneal diseases Materials and Methods:The charts of patients who were fit with rigid gas permeable lenses in Ankara University Medical Faculty at January 2008- August 2015 were reviewed retrospectively. The reason for lens fit, the number of trial lenses required for ideal fit, centration and stabilization of the contact lens, uncorrected, spectacle-corrected, and contact lens-corrected visual acuities, slit lamp biomicroscopy findings and topographic findings are recorded. Results: Eight hundred and sixty eyes of 483 patients were included in the study. The mean patient age was 26.7±9.9. 54% of the cases were male and 46% of the patients were female. Indications included, 855 eyes with various degrees of keratokonus, 1 eye with keratoglobus, 3 eyes with pellucid marginal degeneration, 1 eye with postgraft ectasia. Among those patients 274 eyes (%31.9) had mild, 302 eyes (%35.1) had moderate, 95 eyes (%11) had severe and 189 eyes (%22) had advanced keratoconus. The mean uncorrected, best spectacle corrected and the best contact lens corrected visual acuities were 0.99 ± 0.60 (0-2.6), 0.39 ± 0.39 (0-2.6) , 0.05 ± 0.33 (0-9), 0.08 ± 0.54 (0-9) logMAR respectively. The mean trial lens count until succesfull fit was 2.01±1.0 (1-6). The mean difference between the first diagnostic lens and the lens prescription was 0.15±0.10.In 793 eyes adequate fit with rigid gas permeable lenses were achieved and prescribed. Sixty-seven eyes had (%7.8) RGP lens failure and couldn't get a presciption. To these patients, INTACS, keratoplasty, Kerasoft lenses and Scleral lenses had been recommended. Through their follow-ups 107 eyes needed re-prescribtion. From those who have been prescribed, 162 eyes (%18.8) had never purchased the lens. 91 cases (%10.6) had bought their lenses and came for their control visits but discontiuned after purchasing. In 749 cases (%87.1), the prescribtion remained same throughout theşr follow-ups, however in 83 cases (%9.7) prescription changed 1 time, 21 cases (%2.4) 2 times, 5 cases (%0.6) 3 times ve 2 cases (%0.2) 4 times.The mean follow up period was 15.4 ±19.2 months (6 -127 months) after contact lens fit. There was no complications encountered except 1 sterile infiltrate in 1 case. Conclusions: Rigid gas permeable contact lenses may be successfully fit in keratoconic eyes regardless of the keratoconus grade. Easy to fit and care, these lenses may be used for long term with no serious complications.
