Özet

Bu araştırma, fizyoterapi alan serebral palsili çocuklarda, refleksoloji uygulamasının, çocukların spastasite, kaba motor fonksiyonları, bağımlılık düzeyi ve yaşam kalitesine etkisinin belirlenmesi amacıyla yapılmıştır. Plasebo-kontrollü deneysel araştırma olarak yapılan araştırma, 23 Aralık 2014- 2 Şubat 2016 tarihleri arasında Melikgazi Özel Eğitim ve Rehabilitasyon Merkezi'nde eğitim gören 15 deney, 15 plasebo, 15 kontrol grubunda olmak üzere 45 çocukla gerçekleştirilmiştir. Klinik Araştırmalar Etik Kurulu'ndan onay, çocuklardan ve ebeveynlerden yazılı bilgilendirilmiş gönüllü olur alınmıştır. Veriler Çocuk Tanıtım Formu, Kaba Motor Fonksiyon Sınıflama Sistemi (KMFSS), Kaba Motor Fonksiyon Ölçütü (KMFÖ), Modifiye Ashworth Skalası (MAS), Modifiye Tardieu Skalası, Pediatrik Fonksiyonel Bağımsızlık Ölçeği (PFBÖ), Pediatrik Yaşam Kalitesi Ölçeği (PedsQL) kullanılarak toplanmıştır. Uygulama öncesi deney, plasebo ve kontrol grubunun ölçümleri yapılmıştır. Deney grubundaki çocuklara haftada iki kez, yirmi dakika, toplam yirmi dört seans refleksoloji uygulaması, plasebo grubundaki çocuklara da haftada iki kez, yirmi dakika, toplam yirmi dört seans plasebo uygulaması yapılmıştır. Uygulama sonrası ölçümler tekrarlanmıştır. Kontrol grubuna herhangi bir uygulama yapılmamış, üç ay sonra ölçümler tekrarlanmıştır. Normal dağılım göstermeyen değişkenler için Wilcoxon T Testi ve Kruskal Wallis testi kullanılmıştır. Kategorik değişkenlerin karşılaştırılmasında Ki-kare ve Mcnemar testi uygulanmıştır. Tüm değerlendirmelerde p<0.05 değeri istatistiksel olarak anlamlı kabul edilmiştir. Çalışmada sağ taraf MAS Gastrokinemius Kası ve M. Soleus Kaslarında (p<0.001) ve sol taraf MAS M. Soleus Kasında gruplar arası farkın istatistiksel olarak önemli olduğu bulunmuştur (p<0.05). Sağ taraf Tardeu Kalça Addüktörleri V3 (p<0.05), Gastrokinemius Kası V1, Gastrokinemius Kası V3, M. Soleus Kası V1 ve M. Soleus Kası V3 değerlerinin gruplar arası farkın önemli olduğu belirlenmiştir (p<0.001). Sol taraf Tardeu Kalça Addüktörleri V1 (p<0.05), Hamstringler V1, Hamstringler V3, Gastrokinemius Kası V1, M. Soleus Kası V1 ve M. Soleus Kası V3 değerlerinin gruplar arası farkın önemli olduğu belirlenmiştir (p<0.001). KMFÖ toplam ölçek puanlarında ve oturma alt başlığında; PFBÖ toplam ölçek puanlarında, kendine bakım ve iletişim alt başlıklarında gruplar arası farkın önemli olduğu saptanmıştır (p<0.05). Yaşam kalitesi ölçeğinde gruplar arası farkın önemli olmadığı bulunmuştur (p>0.05). Çalışmada fizyoterapi alan serebral palsili çocuklarda refleksolojinin alt ekstremitede spastisiteyi azalttığı, kaba motor fonksiyonlarda ilerleme sağladığı, bağımlılık düzeylerini azalttığı, yaşam kalitesinde ise herhangi bir değişim oluşturmadığı bulunmuştur. Refleksoloji uygulamasının sıklığı ve süresi arttırılarak gelecek çalışmaların yapılması önerilmektedir.

The current study was done in order to determine the effect of reflexology upon spasticity, functional dependence level and quality of life among children with cerebral palsy who received physiotherapy. The study, designed in placebo-controlled experimental model, was done with 45 children -15 children assigned to experimental group, 15 to placebo group and 15 to control group- who were trained at Melikgazi Special Education and Rehabilitation Center between the 23rd of December 2014 and the 2nd of February 2016. The ethical suitability of the research was approved by Ethical Council of Clinical Researches and written informed consents of children and their parents were obtained. The data were collected using Descriptive Information Form, Gross Motor Function Measure Classification System, Gross Motor Function Measure, Modified Ashworth Scale (MAS), Modified Tardieu Scale, Pediatric Functional Independence Scale, Pediatric Quality of Life Scale. Before the implementation of reflexology; measurements of the participants in experimental, placebo and control groups were obtained. To the children in the experimental group; a 20 min.-reflexology was performed twice a week in a total 24 sessions while to the children in the placebo group; a 20 min.-placebo was performed twice a week in a total 24 sessions. Following the implementations; measurements were retaken. To the children in the control group; no intervention was done. Measurements were retaken after three months. For the data that did not follow a normal distribution; Wilcoxon T Test and Kruskal Wallis test were used. For the comparisons of the categorical variables; Chi-square and Mcnemar tests were employed. The accepted level of significance for all analyses was p< 0.05. In the study; it was identified that the difference between right MAS gastrocnemius muscle and M. Soleus muscles (p<0.001) and left MAS M. Soleus muscle was significant among the groups (p<0.05). Also; there was significant difference between right Tardieu hip adductor V3 (p<0.05), gastrocnemius muscle V1, gastrocnemius muscle V3, M. Soleus muscle V1 and M. Soleus muscle V3 measurements among the groups (p<0.001). Also; there was significant difference between left Tardeu hip adductors V1 (p<0.05), Hamstring V1, Hamstring V3, gastrocnemius muscle V1, M. Soleus muscle V1 and M. Soleus muscle V3 measurements among the groups (p<0.001). It was noted that there was significant difference in Gross Motor Function Measure total scores and sitting position; in Pediatric Functional Independence Scale total scores, self-care and communication subscales among the groups (p<0.05). It was seen that there was no statistically significant difference in terms of Pediatric Quality of Life Scale among the groups (p>0.05). It was found in the study that reflexology reduced spasticity in lower extremities, improved gross motor functions, decreased dependency but led to no change in quality of life among children with cerebral palsy who received physiotherapy. It is recommended that prospective studies be replicated with increased frequency and duration of reflexology.
