Özet

Amaç: Düşük akım anestezinin (DAA) çevre kirliliğini azaltmak, maliyet düşüşü sağlamak, solunum devresindeki havanın ısı ve neminin korunması ile solunum yolu fonksiyonlarının iyileşmesine katkı sağlamak gibi birçok faydaları vardır. Günümüzde gelişen teknoloji ile anestezi makinelerinde çok önemli gelişmeler olmaktadır ve hasta güvenliğini artırıcı sistemler makinelere eklenmektedir. Biz bu prospektif çalışmamızda, Perseus A500 (Drager, Almanya) anestezi makinesinde bulunan ekonometre yazılımını kullanarak, TGA (taze gaz akım) hızı ekonometre ile ayarlanmış pediyatrik hasta grubunda DAA'nin anestezi derinliği ve inspiratuar oksijen konsantrasyonları üzerine etkisini araştırdık. Ayrıca kısa süreli (30dk -1 saat) ve uzun süreli (1 saat üstü) pediyatrik cerrahi olgularında ekonometre kullanımın inhalasyon anesteziği sarfiyatı üzerine etkilerini de karşılaştırdık. Son olarak DAA sırasında laringeal maskenin (LMA) kullanılabilirliğini de araştırdık. Materyal metod: Standart monitörizasyon ve anestezi indüksiyonundan sonra hastaların hava yolu güvenliği ETT veya LMA ile sağlandı. Her iki grupta TGA hızı 4 l/dk. ve taze gaz içinde %80 O2/Hava olacak şekilde ayarlandı. DAA grubunda MAK 0.8 olduğunda TGA ekonometre önerilerine göre ayarlandı. YAA grubunda TGA 2 l/dk. olarak ayarlandı. Kan basıncı, kalp hızı, periferik O2 satürasyonu, vücut sıcaklığı, inhale ekshale edilen gaz içindeki oksijen, karbondioksit, anestezi gaz miktarı ve TGA hızı, BİS değeri entübasyon sonrası ilk 20 dk.'da her 5 dk.'da bir, sonrasında 10 dk.'da bir kayıt edildi. Operasyon sonunda toplam tüketilen ve hasta tarafından alınan sevofluran miktarı anestezi cihazı içindeki yazılım sayesinde otomatik olarak hesaplandı ve kaydedildi. İstatistik analiz için ANOVA, t test ve ki kare testleri kullanıldı. Bulgular: Ekonometre önerileri uygulanan DAA grubundaki olguların tümünde TGA 0.5 l/dk.'ya düşüldü. YAA uygulanan olgularda sevofluranın tüketim miktarları 3 kattan daha fazlaydı (p < 0.0001). DAA uygulanan grupta uygulanan sevofluranın %30'u hasta tarafından alınırken, YAA uygulanan olgularda bu oran %0.8 idi (p < 0.0001). Her iki grupta saatlik sevofluran alım miktarları benzerdi. DAA ve YAA'de bir saatten uzun süren ve bir saatten kısa süren olgular,kendi aralarında karşılaştırıldığında her iki zaman dilimi için de sevofluran tüketimi YAA grubunda 2 anlamlı düzeyde daha yüksekti (p < 0.0001). DAA uygulanan hiçbir olguda ekspiratuar oksijen konsantrasyonu %30'un altına, SpO2 değeri de % 95'in altına düşmedi. %3 civarında açılan sevofluran vaporizatörü, yeterli alveolar konsantrasyonu sağlayabilmek için TGA'nın düşmesiyle daha yüksek konsantrasyonlara ayarlandı ve bu artış 10. dakikadan itibaren istatistiksel olarak anlamlı düzeye çıktı (p < 0.01). Olguların MAK değerleri anestezi süresi boyunca istenen aralıkta ve stabil olarak seyretti. DAA uygulanan olgular entübasyon şekillerine göre LMA (n = 33) ve ETT (n = 39) kullanılanlar olarak iki gruba ayrıldıklarında,ETT grubunda anestezi süresi, cerrahi süre, toplam sevofluran tüketimi ve alımı daha yüksekti. LMA grubundaki bütün olgularda DAA uygulaması herhangi bir ekstra müdahaleye gerek kalmaksızın tamamlandı. Sonuç: Ekonometre önerilerine uyularak pediyatrik olgularda TGA hızı 0.5 l/dk.'ya düşürüldüğü zaman, %50 civarında inspiratuar oksijen, hipoksik gaz karışımı riskinden korurken, %3'ün biraz üzerindeki sevofluran vaporizatör ayarı ile de yeterli anestezi derinliği sağlanabilmektedir. DAA bir saaten kısa süren olgularda da anlamlı anestezik gaz tasarrufuna neden olmaktadır. Bulgularımız DAA ugulamasının LMA ile de güvenle yapılabileceğini göstermektedir. Bu bulguların ışığında, pediyatrik olgularda ekonometre önerilerine uyularak DAA'nın güvenli bir şekilde uygulanabileceğini düşünmekteyiz.

Objective: Low-flow anesthesia (LFA) have many benefits such as reducing environmental pollution, reducing costs, protecting air and temperature in respiratory circuit, and contributing to the improvement of respiratory function. Today, with the developing technology, very important developments are taking place in anesthesia machines and systems for increasing patient safety are being added to the machines. In this prospective study, we investigated the effect of LFA on anesthesia depth and inspiratory oxygen concentrations in pediatric surgery patients adjusted with a fresh gas flow (FGF) rate econometer using the econometer software on the Perseus A500 (Drager, Germany) anesthesia machine. We also compared the effects of the use of an economometer on the consumption of inhalation anesthesia in short-term (30 min-1 hour) and long-term (1 hour) pediatric surgery cases. Finally, we also investigated the use of laryngeal mask airway (LMA) during LFA. Materials and Methods: Following standard monitorization and anesthesia induction, patients airway safety was provided by ETT or LMA. FGF rate was 4 L/min in both groups. And set to 80% O2/Air in fresh gas. When the MAC was 0.8 in the LFA group, the FGF was adjusted to the econometer recommendations. In the HFA (high flow anesthesia) group FGF was 2 L/min. Blood pressure, heart rate, peripheral O2 saturation, body temperature, oxygen concentration in inhaled and exhaled gas, carbon dioxide, anesthetic gas volume and TGA rate were recorded every 5 min for the first 20 min after the intubation, then followed by every 10 min. At the end of the operation, the total amount of sevoflurane consumed and taken by the patient was calculated automatically and recorded by the software in the anesthesia device. ANOVA, t test and chi square tests were used for statistical analysis. Results: The FGF was reduced to 0.5 L/min for all cases in the LFA group with the econometer recommendation. In high flow anesthesia patients, consumption of sevoflurane more than 3 times (p <0.0001). While 30% of the sevoflurane administered in the LFA group was taken by the patient, this rate was 0.8% in the HFA cases (p <0.0001). The hourly amounts of sevoflurane intake were similar in both groups. Sevoflurane consumption of more than one hour and less than one hour in LFA 4 and HFA, compared to their own consumption of two timeslots, was significantly higher (p <0.0001) in the HFA group.In any case where LFA was applied, the expiratory oxygen concentration was below 30% and the SpO2 value did not go below 95%. The sevoflurane vaporizer opened at around 3% was set to higher concentrations with a decrease in FGF to provide adequate alveolar concentration, and this increase was statistically significant from the 10th minute (p <0.01). MAC values of the cases remained stable and within the desired range throughout the duration of the anesthesia. In the cases applied by intubation to as LMA (n = 33) and ETT (n = 39) when divided into two groups as those used, the duration of anesthesia, duration of surgery, total sevoflurane consumption and intake were higher in the ETT group. In all cases in the LMA group, the LFA management was completed without any additional intervention. Conclusion: When TGA rate is reduced 0.5 l/min in pediatric cases by following to the recommendations of the econometric , 50% of the inspiratory oxygen is kept at the risk of hypoxic gas mixture, while adequate anesthesia depth can be achieved with a setting of sevoflurane vaporiser slightly above 3%. LFA provides significant anesthetic gas savings even in surgeries with shorter than 1 hour. Our findings show that LFA management can be performed safely with LMA. In the light of these findings, we think that LFA can be safely applied in pediatric cases by following the econonomic recommendations.
