Özet
ÖZET Şizofrenik hastaların % 20'si klasik antipsikotiklere cevap vermemektedir. Araştırmalarda, klozapinin tedaviye dirençli bu hastalarda % 30-61 oranında iyileşme sağladığı gösterilmiştir. Ancak yüksek etkinlik, düşük ekstrapiramidal yan etkilerine karşın agranulositoz riski nedeniyle kullanımı sınırlı olmaktadır. Çalışmada 29 tedaviye dirençli şizofren hastada Klozapinin etkinliği ve yan etki profilinin araştırılması amaçlanmıştır. Hastalar kısa psikiyatrik değerlendirme ölçeği (BPRS), klinik gobal izlenimler ölçeği (CGI), negatif semptomları değerlendirme ölçeği (SANS) ile UKU yan etki skalası, vital bulgular takibi, EEG, tam kan, periferik yayma, biyokimyasal ve hormonal ölçümlerle izlendi. Olgular 30 hafta süre ile takip edildi. Ölçümler tedavi öncesi; 1., 2., 3., 4., 5., 6., 10., 14., 18., 22., 26. ve 30. haftalarda sadece EEG, biyokimyasal ve hormonal takipler tedavi öncesi ve tedavi sonunda yapıldı. Klozapin 12.5 mg/gün ile başlanıp 450 mg/gün doza çıkarıldı. Çalışmayı 26 olgu tamamladı. Bir olgu akatizi ve ağır sedasyon, bir olgu ağır sedasyon nedeniyle, bir olgu da takip için kan vermeyi reddettiği için çalışma dışında kaldı. 30. Hafta sonunda tedavi öncesine göre BPRS skorlarında % 36.2, CGI skorlarında % 29.92, SANS skorlarında %21.67 oranında azalma meydana geldi. Sıklık sırasına göre en çok izlenen yan etkiler hipersalivasyon (% 46.2), sedasyon (% 42.3) ve çarpıntı (% 38.4) olarak saptandı. Bir olguda konvulsif nöbet gözlendi. En önemli yan etki olarak bildirilen agranülositoza çalışma grubunda rastlanmadı. 64

SUMMARY EFFICACY AND ADVERSE EFFECTS OF CLOZAPINE IN TREATMENT- RESISTANT OF SCHIZOPHRENIA The 20 % of the schizophrenic patients are refractory to treatment with conventional neuroleptics. The studies in those treatment-resistant schizophrenic patient showed that the improvement rate with clozapine was 30-61 %. On the other hand, although it has high efficacy and low extrapyramidal side effects because of the risk of agranulocytosis, its usage is restricted. In this study it has been intended to research the efficacy and the side effect profile of clozapine in 29 treatment-resistant schizophrenic patients. Drug efficacy was assessed by means of Brief Psychiatric Rating Scala (BPRS), Clinical Global Impression (CGI), and Scala for the Assessment of Negative Symptoms (SANS), and side effects were rated on UKU side effect scala, vital signs foolow-up, EEG, whole blood counting, peripheric spread, and biochemecal and hormone analysis. The patients had been followed-up 30 weeks. Most of the ratings had been done before the treatment and the weeks of 1st, 2nd, 3rd, 4th, 5th, 6th, 10th, 14th, 18th, 22nd, 26th, and 30th, only EEG, biochemical and hormone analysis were before and at the end of the treatment. Clozapine was initiated at a dose 12.5 mg/day and increased at a rate of 450 mg/day. Twenty-six patients completed the study. Three patients dropped out for the following reasons: One for achatisia and extreme sedation, one for extreme sedation, one for refusing to give blood sample. At the end of the 30 weeks there had been a decrease of 36.2 % in BPRS score, 29.92 % in CGI score, and 21.67 % in SANS score, compared with before the treatment. The mostly seen side effect were, in frequency order, hypersalivation (46.2 %), sedation (42.3 %), and palpitation (38.4). One patient had convulsive seizure. The agranulocytosis that wa notified in the reports as the most significiant side effect was not observed in this study group. 65
