Özet
Chiasma tendinum, m. flexor digitorum superficialis ve m. flexor digitorum profundus'un tendonlarının kesiştiği bölgede bulunup parmakların ince hareketleri ve stabilitesi açısından kilit öneme sahiptir. Literatürde çalışma konusu doğrudan chiasma tendinum olan yalnızca birkaç çalışma bulunmaktadır. Bu çalışmada chiasma tendinum yapısının detaylı bir incelemesini yapmak, chiasma ile ilgili komşu ve yüzeyel yapıların ilişkisini ortaya koymak ve bu incelemelerin dağılımlarını analiz etmek amaçlanmıştır. Çalışmada modifiye logan solüsyonu ile fikse 10 kadavranın (6 erkek, 4 kadın) üst ekstremiteleri kullanıldı. Parmaklarda yüzeyel referans noktalar belirlendikten sonra, orta hattan insizyon yapılarak deri ve tendon kılıfı açılmış, sonrasında chiasma tendinum ve vincula ile ilgili ölçümler gerçekleştirildi. Bu ölçümler sağ-sol parmaklara, cinsiyete ve taraflara göre analiz edildi. Chiasma tendinum'un tipleri; parmaklara, cinsiyete ve taraflara göre önemli farklılıklar göstermektedir. Chiasma tendinum'un yüzeyelindeki yapılar incelendiğinde, pulley ve chiasma tendinum pozisyonu parmak boyutuna göre değişmesine karşın, kalınlık veya uzunluk gibi parametrelerin parmak boyutuna göre değişmediği gözlendi. Chiasma tendinum'un tüm tiplerinin simetrik olmadığı gözlendi. Asimetrik chiasma tendinum'un fleksiyon sırasında parmak üzerinde bir döndürme etkisi oluşturabileceği düşünüldü. Pulley ve fleksör tendonların morfolojik yapısının kesiden önce yapılan gözlemlere göre yorumlanabilmesi, fleksör tendon cerrahilerinin planlanmasında katkı sağlayabilir. Chiasma tendinum ve pulley yapılarının parmak, taraf ve cinsiyete göre değişiminin bilinmesinin ilgili klinik uygulamalarda daha doğru yaklaşımlar sunulmasına yardımcı olabileceği düşünüldü. Bu çalışmadaki gibi, 5. parmakta chiasma tendinum'un bulunmadığı ve(ya) m. flexor digitorum superficialis'in iki kolundan birinin çok ince olduğu olgular, tendonun spontan hasara daha yatkın olmasına yol açıyor olabilir. Tendinous chiasm is situated where tendons of flexor digitorum superficialis and flexor digitorum profundus muscles cross each other and has utter importance in fine movements and stability of the fingers. There are rather few studies in the literature which focus solely on tendinous chiasm itself. This study aims to conduct a robust examination of the tendinous chiasm, reveal the properties of related adjoint and superficial structures and analyze the distributions of these properties. 10 (6 male, 4 female) formalin fixated cadaver upper limbs were bilaterally micro-surgically dissected. Skin and tendon sheath measurements were completed initially and through a midline excision, tendinous chiasm and vincula measurements were done. The measurements were analyzed with respect to fingers, genders and sides. Tendinous chiasm types exhibit significant differences in distribution with respect to fingers, genders and sides. Pondering the structures superficial to the tendinous chiasm, positions of pulleys and tendinous chiasm correlate with finger size, however the parameters such as width and thickness are independent of the finger size. Not all tendinous chiasm types are symmetrical. Asymmetrical tendinous chiasm types could cause a torsion effect on fingers. Acquiring information about morphological structures of pulleys and flexor tendons without incisions could provide valuable input for flexor tendon surgeons. Distribution of tendinous chiasm and pulley parameters with respect to finger, genders and sides could help develop better clinical approaches. Absence of tendinous chiasm or one of two limbs of flexor digitorum superficialis insertions observed especially at 5th digit implies that flexor tendon injury in this finger could occur without significant trauma.
