Özet
De Quervain hastalığı el bileğinin radial tarafı üzerinde ağrı, hassasiyet ve ödemle karakterizedir. Abduktor pollicis longus (APL), ekstansör pollicis brevis (EPB) tendonları ve bu tendonların fibroosseöz tendon kılıfları arasındaki mekanik impingiment nedeniyle ortaya çıkmaktadır. De Quervain tenosinovitinin tedavisinde bir uzlaşı sağlanamamıştır. Sıklıkla tercih edilen tedavi modaliteleri içerisinde cerrahi dışı yöntem olan el-elbileği aktivitelerinin modifikasyonu, analjezik ilaçlar, kortikosteroid enjeksiyonları, ortezleme, sıcak, soğuk, TENS, lazer ve masaj tedavisi gibi fizik tedavi modaliteleri yer almaktadır. Bu tedavilerin gerçek etkinliği tam olarak bilinmemektedir. Kinezyobant (KB) rehabilitasyon programlarında kullanılan yeni teknikler içerisindedir. Ortopedik ve spor ilişkili durumlarda yaygın bir şekilde kullanılmasına rağmen, diğer kas iskelet sistemi hastalıklarında yardımcı tedavi olarak giderek daha sık tercih edilmektedir. Bu çalışmada de Quervain hastalığı olanlarda KB'nin ağrı, el fonksiyonları ve uyku kalitesi üzerindeki etkinliğinin el-el bilek orteziyle karşılaştırılması amaçlanmıştır. Çalışmaya de Quervain tanısı olan, on sekiz yaş üzeri kırk iki hasta dahil edildi. Prospektif randomize dizayndaki bu klinik çalışmada KB grubunda 21 hasta, el-el bileği ortezi grubunda 21 hasta değerlendirildi. KB grubunda, haftada dört defa iki adet KB uygulanırken, ortez grubuna baş parmağı nötral pozisyonda tutan hazır üretim el- el bileği ortezi önerildi. Hastaların yaş, cinsiyet ve vücut kitle indeksi (VKİ) analizlere dahil edildi. Tedaviden önce ve tedaviden 4 hafta sonra, ağrı şiddeti 10 puanlı bir vizüel analog skala ile (VAS), el fonksiyonları Quick-Disability of the Arm, Shoulder and Hand Questionnaire (qDASH) ölçeği ile, uyku kalitesi Pittsburg Uyku Kalite İndeksi (PUKİ) ile değerlendirildi. KB grubunun yaş ortalaması 40,0 ± 16,6 yıl, ortez grubunun yaş ortalaması 46,3 ± 13,3 yıldı. KB grubunun %85,7'si kadın, %14,3'ü erkekti. Ortez grubunun %76,2'si kadın, %23,8'i erkekti. VKİ ortalaması KB grubunda 27,6 ± 6,0 kg/m2 , ortez grubunda 29,5 ± 6,3 kg/m2 'ydi. Gruplar arasında yaş (p=0,184), cinsiyet (p=0,697) ve VKİ (p=0,314) açısından anlamlı farklılık yoktu. Tedavi sonrasında hem KB hem de ortez grubunda VAS, qDASH ve PUKİ skorlarında anlamlı gelişme izlendi. Tedaviden önce VAS (p=0,570), qDASH (p=0,364) ve PUKİ (p=0,479) skorları benzer düzeydeydi. Tedaviden sonra, KB grubunun VAS (p=0,038) ve qDASH (p=0,033) skorları ortez grubundan anlamlı derecede daha düşüktü, ancak PUKİ (p=0,980) skorları benzer düzeydeydi. De Quervain tenosinoviti tedavisinde hem ortez hem de KB ağrı, el fonksiyonları ve uyku kalitesi üzerinde etkilidir. Bununla birlikte, KB tedavisi ağrı ve el fonksiyonları açısından daha iyi gelişme sağlamaktadır.

Comparison of conservative treatment's effect on pain and sleep quality in de Quervain's Disease De Quervain's disease is a disorder that is characterized by pain, tenderness and edema over the radial side of the wrist. It is caused by mechanical impingement between the tendons of abductor pollicis longus (APL) and extensor pollicis brevis (EPB) and their narrowed fibroosseous tendon sheet.There is no consensus in the treatment of de Quervain's tenosynovitis.Currently the common treatment modalities for de Quervain's disease are nonoperative and consist of modification of wrist activities, analgesic drugs, corticosteroid injection, bracing, casting and physical therapy modalities such as heat, cold, TENS, laser, massage, etc. The exact efficacy rate of these therapies is not clear. Kinesiotaping (KT) is a relatively new technique used in rehabilitation programs. Although it has been commonly used in orthopedic and sports settings, it is gradually becoming an adjunct treatment option for other musculoskeletal impairments. The purpose of this study is to compare the effects of KT application on pain, hand functions and sleep quality in subjects with de Quervain's disease with wrist-hand orthosis. Fourty two patients over the age of 18, with a diagnosis of Quervain disease were included in the study.In this prospective randomized clinical study, 21 patients were enrolled into the KT group, and 21 other patients were enrolled into the wrist-hand orthosisgroup. In the KT group, 2 pieces of KT were used 4 times weekly, and in the orthosisgroup fabricated wrist-hand orthosis holding the thumb in neutral position. Age, gender and body mass index (BMI) information of the patients were included in the analysis. Pain severity was evaluated with a 10-point visual analog scale (VAS), hand-wrist functions with Quick-Disability of the Arm, Shoulder and Hand Questionnaire (qDASH), and sleep quality with the Pittsburg Sleep Quality Index (PSQI) before and after four week treatment. The mean age of the KT group was 40.0 ± 16.6 years and the orthosis group was 46.3 ± 13.3 years. 85.7% of the KT group were women, 14.3% were men, and 76.2% of the orthosis group were women and 23.8% were men. The mean BMI of the KT group was 27.6 ± 6.0 kg/m2 , and 29.5 ± 6.3 kg/m2 for the orthosis group. There was no difference in age (p=0.184), gender (p=0.697) and BMI (p=0.314) between the groups.Significant improvement was observed in VAS, qDASH and PSQI scores after treatment in both the KT XIII and orthosis groups.VAS (p=0.570), qDASH (p=0.364) and PSQI (p=0.479) scores were similar between the KT and orthosis group before treatment. After treatment VAS (p=0.038) and qDASH (p=0.033) scores were lower in the KT group than in the orthosis group, but PSQI scores were similar (p=0.980). Both orthosis and KT are effective in pain, hand functions and sleep quality in patients with de Quervain's tenosynovitis, However, KT treatment has yielded better results in terms of pain and hand functions.
