Özet
ÖZET Bu çalışma, izofluran ve sevofluranın genel anestezi sırasında; kullanılan kas gevşetici miktarına, hemodinamiye, stres yanıtlarına, plazma elektrolitlerine, böbrek ve karaciğer fonksiyon testlerine, postoperatif dönemde ise sedasyon derecesine, analjezik gereksinim zaman ve miktarına etkilerini değerlendirerek karşılaştırmak amacıyla yapıldı. 42 bayan olgu (ASA I- II) 21'er kişilik 2 gruba ayrıldı (1. İzofluran grubu, 2. Sevofluran grubu). Anestezi indüksiyonunda tüm olgulara 2,5 mg/kg. propofol ve 0,1 mg/kg. veküronyum verildi. Anestezi, 1, gruba % 0.5 - % 1,5 izofluran + %30 O2+ % 70 azotprotoksit, 2. gruba % 0.5 - % 2 sevofluran + % 30 02 + % 70 azotprotoksit verilerek sürdürüldü. Tüm olgularda; kullanılan kas gevşetici miktarı, ortalama arter basıncı (OAB), nabız sayısı (NS) ve kandaki glukoz, insulin, adrenokortikotropik hormon (ACTH), alkalen fosfataz (AP), laktat dehidrogenaz (LDH), aspartat aminotransferaz (AST), alanin aminotransferaz (ALT), gama glutamil transferaz (GGT), kan üre azotu (BUN), kreatinin (Cre), klor (CI), potasyum (K), sodyum (Na) değerleri ile postoperatif sedasyon derecesi, analjezik gereksinim zamanı ve miktarı saptanarak gruplar arası ve grup içi karşılaştırmalar yapıldı. NS değerleri, Sevofluran grubunda 120. dk ve postoperatif lO.dk'da İzofluran grubundan daha düşüktü (p<0.05). OAB değerleri ise 60. dk'da Sevofluran grubunda daha yüksekti (p<0.01). Plazma glukoz, insülin ve ACTH değerleri her iki grupta da insizyondan hemen önceki değerlere göre insizyon sonrası 5., 30., 60. dakikalarda yükselme gösterdi (p<0.05, p<0.001,p<0.001). Anestezi bitiminde ALT, postoperatif 6. ve 48. saatte BUN İzofluran grubunda, 24. saatte ise AP Sevofluran grubunda daha düşüktü (p<0 01). Klor (Cl), postoperatif 48 saatte İzofluran grubunda daha yüksekti (p 0.001). İki grup arasında istatistiksel farklılık gösteren bütün bu değerler, her iki grupta da normal sınırlar içindeydi. Analjezik gereksinim miktarı iki grupta aynı, gereksinim zamanı ise Sevofluran grubunda daha kısa idi (p- 0.01). Sevofluran grubunda postoperatif uyanma daha hızlı, sedasyon derecesi daha az bulundu (p<0 001). IIISonuç olarak; İzofluran ve Sevofluran grupları arasında; kas gevşetici gereksinim miktarı, OAB, NS, postoperatif karaciğer ve böbrek fonksiyonları, serum elektrolitleri yönünden bazı istatistiksel farklılıklara karşın, bu değerlerin normal sınırlar içinde olmaları bakımından benzerlik olduğu, en belirgin fark olarak ise, Sevofluran grubunda postoperatif uyanmanın daha çabuk olması ve bu nedenle de hızlı derlenme gerektiren olgularda sevofluranın izoflurana tercih edilebileceği kanısına varıldı.

ABSTRACT Biochemical and Clinical Comparison of Isoflurane and Sevoflurane Anaesthesia in Adult Patients This study was designed to evaluate total neuromuscular blocking agent dose, hemodynamic and stress response to surgery, levels of serum electrolytes, renal and hepatic function tests at preoperative, perioperative and postoperative periods during isoflurane and sevoflurane anesthesia. In addition, sedation levels, first analgesic request time and total analgesic consumption were recorded at postoperative period too. 42 female patients (ASA I-II) were randomly allocated to two groups: Group 1 (Isoflurane group) and Group 2 (Sevoflurane group). A standardised general anesthetic technique was used, consisting of propofol (2.5 mg.kg'1), vecuronium (0.1 mg.kg"1) in induction of anaesthesia and ventilation with nitrous oxide (70%) and isoflurane (0.5-1.5 %) (Group 1) or sevoflurane (0.5-2 %) (Group 2) in oxgen (30%) for maintenance. The amount of vecuronium dose, mean arterial pressure (MAP), heart rate (HR), plasma levels of glucose, insulin and adrenocorticotropic hormon (ACTH), alcalen phosphatase (AP), lactate dehidrogenase (LDH), aspartate aminotranspherase (AST), alanine aminotranspherase (ALT), gamma glutamil transpherase (GGT), blood urine nitrogene (BUN), creatinine (Cre), chlorine (CI), potasium (K), sodium (Na), postoperative sedation levels, first analgesic request time and total analgesic consumption were recorded. The results were compared both between the two groups and in the groups. HR at 120 minutes after induction and 10 minutes after the operation in Sevoflurane group were significantly lower than Isoflurane group (p<0.05). MAP at 60 minutes after induction in Sevoflurane group were higher (p<0.01). Plasma glucose, insulin and ACTH at 5, 30, 60 min after incision were higher than the values just before incision (p<0.05, p<0.001, p<0.001 respectively) in both two groups. The ALT at the end of anesthesia, the BUN at 6 and 48 hours after operation in the Isoflurane group and Vthe AP at 24 hours after the operation in the Sevoflurane group were significantly less than the other values (p<0.05, p<0.01). CI at 48 hours after the opertaion were higher in Isoflurane group (p<0.001). These statitisticaly significant differences between the two groups were in normal ranges. Total analgesic consumption was similar between the two groups, but first analgesic request time was shorter in Sevoflurane group (p<0.01). Postoperative recovery was faster in Sevoflurane group and sedation levels were less in this group (p<0.001). In conclusion; there were some statistically significant differences between Sevoflurane and Isoflurane groups. Because these differences were in normal range, we concluded that these two agents have similar effects on MAP, NS, renal and hepatic functions, intraoperatif neuromuscular agent dose, postoperative analgesic request. Only one difference was very clear; postoperative recovery was faster in Sevoflurane group. For this reason, it was decided that sevoflurane can preferred to isoflurane, if fast recovery is needed.
