Perceived influences on healthy eating among African American women of cidldbearing age: Interpretations using the pen-3 theoretical framework

Özet

The specific objective of the current study is to explore the healthy eating perceptions of African American (AA) women of childbearing age residing in zip code areas of Genesee county, Flint, Michigan with the highest documented rates of infant mortality. The PEN-3 model (Airhihenbuwa et al, 1992) guided the development of the focus group panel discussion guide that was composed of open ended questions. Community members served on the project steering committee and helped with recruitment of the study sample, selection of sites, moderation of the focus groups and interpretation of the results. A series of four focus group panels were facilitated in loca] community centers, and lasted 2 hours each, during which AA women 19-30 years (n=l l) participated (11 x 4 = 44). Sessions were audio recorded and transcribed. Transcripts were reviewed and broader themes were developed based on the three dimensions of the PEN-3 theoretical framework (Airhihenbuwa et al , 1992). The first dimension of the PEN-3 model health education encompasses persons, extended family and neighborhood as potential targets for nutrition education. The second dimension educational diagnosis of health guided the interpretations of results related to perceptions, enablers and nurturers. The third dimension, cultural appropriateness of health behavior, identified factors that influence healthy eating from a cultural standpoint and includes positive, exotic and negative factors. Young women in the early years of childbearing were perceived to benefit from pre-pregnancy nutrition programs addressing micronutrients such as iron, calcium and folic acid. Family members are thought to influence women's food habits by both providing nutrition advice, and during food preparation at gatherings. Participants discussed neighborhood concerns within the community such as limited access to community nutrition programs, as well as concerns (location, size, access, variety, quality) about neighborhood grocery stores. In addition eating out was described as a common practice among young women and frequency of foods eaten away from home by young women were perceived to be linked to the nutritional quality of their food selections and birth outcomes experienced. Several strategies were suggested to help overcome the physiological barrier of lactose intolerance that some women may experience and help meet their daily needs for calcium including the importance of label reading activities. Factors such as access and quality of prenatal care, time limitations, budget restrictions and stressful experiences are considered major influences on healthy eating. Family members act as nurturers by providing women with recipes and influencing their decisions to seek prenatal care. Positive influences such as modifications to food preparation methods need to be encouraged and exotic factors such as vegetable and milk preferences for consumption should be incorporated into nutrition education programs. Negative practices include items added such as salt and butter to prepared vegetables as well as common misconceptions about food restrictions during pregnancy need to be addressed to improve eating habits. In summary, the constructs of the PEN- 3 model were confirmed applicable to the development of the focus group panel discussion guide as well as for the interpretations of results within the current study. Nutrition educators are encouraged to take into account perceived influences identified using the PEN-3 model while designing interventions targeting young AA women in the community of Genesee County, Flint.
